Long-term clinical evaluation of laparoscopic management of large hiatal hernias.

BACKGROUND: Large hiatal hernias (LHH) account for 5-10% of all hiatal hernias. Surgery of LHH should be associated with low rates of postoperative complications and recurrences, to guarantee a favorable quality of life (QoL). Data on long-term results of laparoscopic repair of LHH are lacking. The objective of our study is to evaluate the long-term clinical outcomes of laparoscopic LHH management in a high-volume experienced center. METHODS: Patients who had undergone elective laparoscopic repair of LHH between January 1992 and December 2008 at the Center of Minimally Invasive Surgery of the

Treatment Response With Potassium-competitive Acid Blockers Based on Clinical Phenotypes of Gastroesophageal Reflux Disease: A Systematic Literature Review and Meta-analysis.

BACKGROUND/AIMS: Gastroesophageal reflux disease (GERD) is typically managed based on the clinical phenotype. We evaluated the efficacy and safety of potassium-competitive acid blockers (PCABs) in patients with various clinical GERD phenotypes. METHODS: Core databases were searched for studies comparing PCABs and proton pump inhibitors (PPIs) in clinical GERD phenotypes of erosive reflux disease (ERD), non-erosive reflux disease (NERD), PPI-resistant GERD and night-time heartburn. Additional analysis was performed based on disease severity and drug dosage, and pooled efficacy was calculated.

Mean nocturnal baseline impedance in gastro-esophageal reflux disease diagnosis: Should we strictly follow the Lyon 2 Consensus?

Clinical practice guidelines drive clinical practice and clinicians rely to them when trying to answer their most common questions. One of the most important position papers in the field of gastro-esophageal reflux disease (GERD) is the one produced by the Lyon Consensus. Recently an updated second version has been released. Mean nocturnal baseline impedance (MNBI) was proposed by the first Consensus to act as supportive evidence for GERD diagnosis. Originally a cut-off of 2292 Ohms was proposed, a value revised in the second edition. The updated Consensus recommended that an MNBI < 1500 Ohms

Distal esophageal spasm and gastroesophageal reflux disease: re-examining the association.

Distal esophageal spasm (DES) is a rare motility disorder presenting with dysphagia or chest pain. Although studies suggest a link between DES and gastroesophageal reflux disease (GERD), data supporting a distinct GERD-related phenotype are limited. This study aims to investigate demographic, symptomatic, and physiologic differences between DES subjects with and without GERD. A retrospective cohort analysis of DES patients determined by high resolution manometry (HRM) was conducted between February 2020 and January 2023. Demographics, medications, symptoms, and quantitative reflux testing data

Probable collagenous gastritis via Epstein-Barr virus reactivation in the setting of coronavirus disease 2019: a case report.

BACKGROUND: Recent biomedical research has shown the unusual, multisystem effects of coronavirus disease 2019 in humans. One specific sequela of a primary severe acute respiratory syndrome coronavirus 2 infection is the reactivation of latent viruses in various tissues, such as Epstein-Barr virus. Epstein-Barr virus has been identified in many inflammatory gastrointestinal lesions, such as microscopic gastritides and colitides. One subtype of these diseases is collagenous disease. "Long COVID" may be related to the reactivation of these latent viruses, and the following case describes a patien

Exploring the association between magnesium deficiency and chronic obstructive pulmonary diseases in NHANES 2005-2018.

Chronic Obstructive Pulmonary Disease (COPD) significantly impacts patients' quality of life and burdens healthcare systems. Magnesium is crucial for lung function and reducing respiratory disease risk. This study investigates the association between Magnesium Depletion Score (MDS) and COPD and explores whether inflammatory markers mediate this relationship. A cross-sectional analysis was conducted using data from 30,490 participants in the National Health and Nutrition Examination Survey (NHANES) from 2005 to 2018. MDS was calculated based on diuretic use, proton pump inhibitors, renal functi

A new classification of upper gastrointestinal toxicity induced by immunotherapy: from endoscopic and pathological insights to clinical management.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have become the standard of care in several solid tumours. Thus the action of ICIs may lead to the development of inflammatory damage in nontumoral tissues, defined as immune-related adverse events (irAEs). Scanty data describe upper gastrointestinal tract toxicity. PATIENTS AND METHODS: We conducted a monocentric retrospective study, enrolling patients with advanced cancer, who developed histology-proven immune-related oesophago-gastro-duodenitis, treated with at least one cycle of ICI between January 2016 and November 2022. RESULTS: We identi

Case report: Lichenoid esophagitis revealing an HIV infection.

Esophageal lichen planus is an underrecognized manifestation of lichen planus. It is typically diagnosed based on characteristic endoscopic findings, such as hyperkeratosis, trachealization, denudation and/or stenosis, along with the presence of a lichenoid infiltrate in histopathological examination. In cases where no other manifestation of lichen planus are found and direct immunofluorescence for fibrinogen along the basement membrane is negative, the term "lichenoid esophagitis" should be preferred. This distinction is critical, as it prompts a thorough evaluation for underlying diseases, i

Development and Validation of a Machine Learning System to Identify Reflux Events in Esophageal 24-Hour pH/Impedance Studies.

INTRODUCTION: Esophageal 24-hour pH/impedance testing is routinely performed to diagnose gastroesophageal reflux disease. Interpretation of these studies is time-intensive for expert physicians and has high inter-reader variability. There are no commercially available machine learning tools to assist with automated identification of reflux events in these studies. METHODS: A machine learning system to identify reflux events in 24-hour pH/impedance studies was developed, which included an initial signal processing step and a machine learning model. Gold-standard reflux events were defined by a

Endoscopic construction of an antireflux mucosal barrier for the treatment of GERD: a pilot study (with video).

BACKGROUND AND AIMS: Based on the porcine natural antireflux mechanism, we developed a novel endoscopic procedure to build an antireflux mucosal flap to block acid reflux and treat GERD. METHODS: The antireflux mucosal valvuloplasty (ARMV) procedure is performed by releasing and reconstructing three-fourths of the circumference of cardiac mucosa at the lesser curvature side into a double-layer mucosal flap. The mucosal flap works together with cardiac scarring to block reflux. We retrospectively reviewed 30 patients who underwent ARMV from 2019 to 2021. Subjective and objective data evaluatin
