Declining trend in osteoporosis management and screening following vertebral compression fractures - a national analysis of commercial insurance and medicare advantage beneficiaries.

BACKGROUND CONTEXT: Despite the increasing national incidence, osteoporosis and its associated comanagement, often remain an overlooked issue in the orthopedic world. Screening and associated management of osteoporosis is often only considered by providers when patients present with multiple fragility fractures. Current evidence with regard to the trends in screening and medical comanagement/antiosteoporotic therapy of osteoporotic vertebral compression fractures (VCFs) remains limited. PURPOSE: To understand trends, costs, and clinical impact associated the utilization of antiosteoporotic me

The prevalence of vertebral fracture amongst patients presenting with non-vertebral fractures.

INTRODUCTION: Despite vertebral fracture being a significant risk factor for further fracture, vertebral fractures are often unrecognised. A study was therefore conducted to determine the proportion of patients presenting with a non-vertebral fracture who also have an unrecognised vertebral fracture. METHODS: Prospective study of patients presenting with a non-vertebral fracture in South Glasgow who underwent DXA evaluation with vertebral morphometry (MXA) from DV5/6 to LV4/5. Vertebral deformities (consistent with fracture) were identified by direct visualisation using the Genant semi-quanti

Peripheral Regional Anesthesia for Awake Emergency Upper Limb Trauma Surgery in an Adult Patient With Fontan Physiology.

Background: The Fontan procedure is the principal technique used in the surgical palliation of a range of congenital heart defects involving a single functional ventricle. With improvements in surgical technique and medical management, patients with Fontan physiology are surviving longer and a growing number are now presenting for noncardiac surgery in adulthood. The Fontan physiology provides a unique challenge for anaesthesiologists managing their perioperative care. This case report explores this challenge further and emphasises the benefits which regional anaesthesia can provide in the man

Spontaneous rupture of the extensor pollicis longus tendon in a lacrosse player.

Spontaneous rupture of the extensor pollicis longus (EPL) tendon at the wrist has been reported with predisposing factors to include distal radius fractures, rheumatoid arthritis, systemic or local steroids, and repetitive abnormal motion of the wrist joint. We present a case of an 18-year-old college lacrosse player without history of known predisposing factors who presented with an acute inability to extend the interphalangeal (IP) joint of his right thumb. Preoperative musculoskeletal ultrasound demonstrated rupture of the EPL tendon proximal to the carpometacarpal (CMC) joint of the thumb.

[Genetic mutation and clinical features of osteogenesis imperfecta type V].

OBJECTIVE: To explore genetic mutations and clinical features of osteogenesis imperfecta type V. METHODS: Clinical record of five patients (including one familial case) with osteogenesis imperfecta type V were retrospectively analyzed. Peripheral blood samples of the patients, one family member, as well as healthy controls were collected. Mutation of IFITM5 gene was identified by PCR amplification and Sanger sequencing. RESULTS: A heterozygous mutation (c.-14C>T) in the 5-UTR of the IFITM5 gene was identified in all of the patients and one mother. The clinical findings included frequent frac

Intraoperative mechanical measurement of bone quality with the DensiProbe.

Reduced bone stock can result in fractures that mostly occur in the spine, distal radius, and proximal femur. In case of operative treatment, osteoporosis is associated with an increased failure rate. To estimate implant anchorage, mechanical methods seem to be promising to measure bone strength intraoperatively. It has been shown that the mechanical peak torque correlates with the local bone mineral density and screw failure load in hip, hindfoot, humerus, and spine in vitro. One device to measure mechanical peak torque is the DensiProbe (AO Research Institute, Davos, Switzerland). The device

Does Early Conversion to Below-elbow Casting for Pediatric Diaphyseal Both-bone Forearm Fractures Adversely Affect Patient-reported Outcomes and ROM?

BACKGROUND: For distal forearm fractures in children, it has been shown that a below-elbow cast is an adequate treatment that overcomes the discomfort of an above-elbow cast and unnecessary immobilization of the elbow. For reduced diaphyseal both-bone forearm fractures, our previous randomized controlled trial (RCT)-which compared an above-elbow cast with early conversion to a below-elbow cast-revealed no differences in the risk of redisplacement or functional outcomes at short-term follow-up. Although studies with a longer follow-up after diaphyseal both-bone forearm fractures in children are

A comparison of the effect of autogenous vs. frozen homogenous grafts on the healing of non-union of forearm bones.

BACKGROUND: A large percentage of autogenous and homogenous grafts are resorbed and replaced by the recipient's own tissue. It is generally believed that autogenous grafts are superior to homogenous ones. The aim of our study was to compare the effectiveness of autografts and allografts in achieving the healing of a non-union of forearm bones. MATERIAL AND METHODS: Between 1976 and 2005, 56 patients with 68 non-unions of forearm bone shafts were operated on. The study group consisted of 45 male and 11 female patients aged 8 to 85. The patients were operated on using methods requiring autogeno

Impact of a community-based osteoporosis and fall prevention program on fracture incidence.

Associations between a 10-year community-based osteoporosis and fall prevention program and fracture incidence amongst middle-aged and elderly residents in an intervention community are studied, and comparisons are made with a control community. A health-education program was provided to all residents in the intervention community, which addressed dietary intake, physical activity, smoking habits and environmental risk factors for osteoporosis and falls. Both communities are small, semi-rural and situated in Ostergotland County in southern Sweden. The analysis is based on incidences of forearm

Evaluation of bone density and microstructure by DXA, TBS and HR-pQCT: an assessment of cortical porosity and its association with vertebral fractures in patients with acromegaly.

PURPOSE: Patients with acromegaly are at increased risk of fractures, regardless of areal bone mineral density (BMD). This study aimed to identify whether vertebral fractures (VFs) are related to decreases in bone volumetric density and microstructure via high-resolution peripheral quantitative computed tomography (HR-pQCT) and the trabecular bone score (TBS). METHODS: VF was evaluated in 49 patients with acromegaly (22 men and 27 women) via spine X-ray. Areal BMD, volumetric BMD, and bone microstructure were analyzed via dual-energy X-ray absorptiometry (DXA) and HR-pQCT. Lumbar spine trabec

Minimum 10-year follow-up after open reduction and internal fixation of radial head fractures Mason type II and III.

BACKGROUND: The treatment of complex radial head fractures remains controversial with open reduction and internal fixation (ORIF), radial head arthroplasty, and radial head excision being the most common treatment options. While ORIF is the preferred treatment strategy for Mason type II fractures, the optimal treatment of Mason type III fractures is debated. The purpose of this study was to report minimum 10-year outcomes after ORIF of Mason type II and type III radial head fractures. We hypothesized that both Mason Type II and Type III fracture patients would demonstrate satisfactory clinical

An analysis of causes and treatment outcome of chronic wrist pain after distal radial fractures.

Healed distal radial fractures are frequently complicated by chronic wrist pain which is multifactorial and can be debilitating. An accurate delineation of the pathoanatomy is the key for successful treatment. This study reviewed 22 patients who had surgical treatment between 1997 and 2001 for chronic wrist pain after distal radial fracture. Four patterns of pathoanatomy were identified: (1) ulnar impaction caused by radial malunion and shortening; (2) ulnar styloid non-union; (3) triangular fibrocartilage complex (TFCC) tears with or without distal radioulnar joint (DRUJ) instability; and (4)

Percutaneous versus subcutaneous titanium elastic nail tips: complication profiles by age group in pediatric forearm shaft fractures.

BACKGROUND: Titanium elastic nailing (TEN) is widely used for the treatment of unstable pediatric forearm fractures. Although subcutaneous nail tips remain the standard technique, superficial (exposed) nail tip placement has been described as an alternative strategy to facilitate implant removal. However, evidence regarding complication profiles associated with different TEN tip management techniques remains limited, particularly with respect to age-related differences. This study addresses three critical gaps: (1) comparative complication rates between superficial and subcutaneous TEN placeme
