Update on carcinoid syndrome.

The carcinoid syndrome is a result of the release of multiple carcinoid tumor factors, particularly serotonin, kinins, and histamine. These factors cause flushing, hemodynamic instability, right-sided heart disease, bronchospasm, and gastrointestinal symptoms. A through preoperative assessment concentrating on these areas is necessary. The use of invasive monitors perioperatively, with the avoidance of drugs and techniques that can exacerbate the syndrome, is beneficial. Octreotide, both prophylactically and acutely, provides the best medical therapy available.

The presence of platelet-activating factor-like activity in human spermatozoa.

Evidence is provided for the existence of platelet-activating factor (PAF)-like activity in the lipid extracts of human spermatozoa. The PAF content of human spermatozoa based on [3H]-serotonin release from washed rabbit platelets was noted to be 1.45 pmol/10(8) sperm cells in highly purified motile spermatozoa. No PAF activity was associated with the seminal fluid. Platelet-activating factor content of spermatozoa may be related to its fertility potential.

[Pharmacological treatment of neuropathic pain in primary care].

Neuropathic pain is difficult to treat. First line treatment includes tricyclic antidepressants (notably amitriptyline), mixed serotonin and norepinephrine reuptake inhibitors (duloxetine for diabetic peripheral neuropathic pain), antiepileptics (gabapentin and pregabalin) and topical agents (lidocaine plasters for postherpetic neuralgia). These drugs can be subsequently associated in case of partial response to monotherapy. Opioids and tramadol are only considered second/third line, alone or in combination with first line drugs.

Advances in the pharmacological treatment of pathological gambling.

In the present paper we discuss the current status of drug treatment for pathological gambling and the scientific rationales underlying the various pharmacological approaches. Specifically, we summarize the treatment study results of serotonin reuptake inhibitors, mood stabilizers, opioid antagonists, and atypical antipsychotics in pathological gambling. We also discuss dosage strategies, the duration of treatment, issues surrounding medication compliance, and approaches to treatment-refractory pathological gambling, such as pharmacological and behavioral augmentation.

[Body dysmorphic disorder as an obsessive-compulsive spectrum disorder].

Considerable evidence supports consideration of body dysmorphic disorder (BDD) and obsessive-compulsive disorder as related disorders. BDD may share common phenomenological and neurobiological features with OGD and other disorders characterized by repetitive thoughts and behaviors and mediated by serotonin dysfunction. Within this substrate, specific psychological and sociological factors may shape the specific BDD syndrome. Further research incorporating dimensional models of these disorders, may delineate specific similarities and differences.

[The assessment of the prognostic factors for the development of cancer in patients with hemorrhages in the presence of endometrial atrophy].

A total of 95 women aged 47 to 72 were examined with a history of bloody discharge in the postmenopause. Sex hormones, hydrocortisone, thyroid hormones, blood lipids and serotonin, noradrenaline and adrenalin excretion were assessed. Histologic examinations of endometrial scrapings off were carried out in all the cases. A table for prediction of endometrial diseases has been created on the basis of the findings. Cancer development risk factors were singled out.

Effect of voluntary retention of urine on plasma and urinary biogenic amines as well as circulatory and respiratory responses.

Stress caused by voluntary retention of urine in 25 normal healthy volunteers has been estimated by measuring the levels of biogenic amines (serotonin, catecholamine) in blood and urine. Estimations were done before and after voluntary retention of urine. Plasma and urine catecholamines and 5-HT significantly increased after retention. Simultaneously rise in BP, PR, RR were also observed. Inference has been drawn that retention of urine may induce stress.

[Exogenous psychosis in a patient with carcinoid syndrome following treatment with p-chlorophenylalanine (PCPA)].

A case is reported of a patient with carcinoid syndrome who developed a exogenous psychosis while under treatment with the serotonin-inhibitor p-chlorophenylalanine (PCPA). Partial symptoms similar to delirium and schizophrenia were exhibited. The attached literature survey focuses on the psychological side effects of PCPA treatment. A discussion follows concerning noteworthy phenomenological similarities between the case reported and certain forms of amphetamine psychosis. Common biochemical mechanisms are hypothetically stated.

Dose Dependent Course of Hyperprolactinemic and Normoprolactinemic Galactorrhea Induced by Venlafaxine.

Venlafaxine is a serotonergic and noradrenergic reuptake inhibitor which is used for the treatment of depression. We report a case of galactorrhea in a patient with major depressive disorder after starting treatment with venlafaxine. In particular, we discuss the course of hyper and normoprolactinemic galactorrhea. We managed this side effect initially by dose reduction and further by switching to essitalopram. Physicians should be aware of endocrinologic side effects such as galactorrhea during the serotonin and noradrenaline reuptake inhibitor treatment.

[When an antidepressant helps too much].

Initiation of an antidepressant may result in hypomania or mania. The risk is lower with selective serotonin reuptake inhibitors and bupropione than with venlafaxine and tricyclic antidepressants. With a mood-stabilizing medication, the risk of hypomania or mania will remain very low, along with its correspondingly lower efficacy. The therapy involves discontinuation of the antidepressant, initiation or dose elevation of a maintenance drug preventing hypomania and mania, and when needed, short-course use of a second generation antipsychotic.

Escitalopram induced mania.

This is a report of a case of recurrent depression with hypertension, ischemic heart disease and diabetes mellitus which switched to mania while on escitalopram. Escitalopram, one of the newer selective serotonin reuptake inhibitors (SSRIs), is considered to have fewer adverse effects and a lower propensity for drug interactions. However, escitalopram may induce mania at a maximum dose of 20 mg especially when given with Alprazolam which is known to boost efficacy of SSRIs.

Primary carcinoid tumor of the pancreas.

Serotonin-secreting (carcinoid) tumors of the pancreas are very rare. There are only 13 cases reported since 1963. Liver metastases have not previously been described. We present two patients with primary carcinoid tumor of the pancreas, which metastasized to the liver. These patients differ in their clinical and radiological appearance. Carcinoid tumor should be considered in the differential diagnosis of a pancreatic mass in a patient with carcinoid syndrome, but lack of this syndrome does not exclude the diagnosis.

Opioid inhibition of dopamine release from nervous tissue of Mytilus edulis and Octopus bimaculatus.

Morphine and D-Ala2-Met-enkephalin as well as other opioids suppress potassium-stimulated release of 3H-labeled dopamine from neurons tissue of two marine invertebrates, Mytilus edulis and Octopus bimaculatus. Naloxone reverses the inhibitory effects in both species. Potassium-stimulated release of 3H-labeled serotonin is not altered by opioids. It is postulated that opiate receptors and their endogenous effectors play a prominent role in regulation of transmitter release in invertebrates.

Idiopathic thrombocytopenic purpura complicating chronic lymphocytic leukemia.

Three patients had severe, symptomatic idiopathic thrombocytopenic purpura (ITP) complicating chronic lymphocytic leukemia (CLL) that was refractory to prednisone treatment but that responded to splenectomy alone or in addition to immunosuppressive-cytotoxic treatment. Evidence of high titer of an antiplatelet factor, most likely an antibody, was corroborated in one case by the 14C-platelet serotonin release assay. This experience suggests that the treatment of ITP complicating CLL is similar to that of ITP not associated with lymphoproliferative disorders.

Lesions of central norepinephrine terminals with 6-OH-dopamine: biochemistry and fine structure.

Intracisternal injections of 6-hydroxydopamine produce rapid and long-lasting depletion of brain catecholamines without effects on serotonin concentrations. Depletion of norepinephrine is greatest in areas containing only nerve terminals and axons and least in areas containing monoamine cell bodies. The norepinephrine loss is accompanied by electron microscopic evidence of nerve terminal degeneration and decreased turnover. Dopamine loss is less marked and is not accompanied by degeneration or alteration of turnover rate.

The different roles of 5-HT1A/2A receptors in fluoxetine ameliorated pigmentation of C57BL/6 mouse skin in response to stress.

BACKGROUND: 5-HT1A receptor was participated in fluoxetine induced melanogenesis in melanocytes and in normal C57BL/6 mice, but we know little about whether other 5-HT receptors are involved in regulation of fluoxetine promotes pigmentation. OBJECTIVE: To investigate the role of 5-HT receptors in regulation of fluoxetine ameliorates chronic unpredictable mild stress (CUMS) and chronic restraint stress (CRS) induce hypopigmentation in C57BL/6 mice. METHODS: CUMS and CRS were used to induce depigmentation in mice and evaluate the effect of fluoxetine. Western blot, immunohistochemistry and Q-P

Pharmacological evidence of involvement of nitric oxide pathway in anti-pruritic effects of sumatriptan in chloroquine-induced scratching in mice.

Chloroquine (CQ) induces histamine-independent itch in human and mice. We recently reported the role of intradermal nitric oxide (NO)/cyclic guanosine monophosphate pathway in CQ-evoked scratching in mice. Chloroquine stimulates neuronal nitric oxide synthase (nNOS) activity to over-producing NO in the skin. Sumatriptan, a 5-hydroxytryptamine 1b/1d receptors (5-HTR1b/1d) agonist, is involved in pain and used to treat migraine and cluster headaches. According to previous studies, sumatriptan inhibits NOS activity. Thus, we aimed to investigate the effect of sumatriptan on CQ-induced scratching.

[On the Functional Cross-Talk between Brain 5-HT1A and 5-HT2A Receptors].

We have found that activation of 5-HT1A receptor with 8-OH-DPAT (0.1, 0.5 and 1.0 mg/kg, i. p.) considerably and dose-dependently reduced the number of 5-HT2A receptor-mediated head-twitches, whereas 5-HT1A receptor blockade with WAY-100635 (0.5 and 1.0 mg/kg, i. p.), on the contrary, pro- duced significant enhancement of this 5-HT2A receptor functional response. At the same time 5-HTA receptor activation with DOI (0.5 and 1.0 mg/kg, i. p.) abolished the 5-HT1A receptor-mediated hypothermic reaction, whereas 5-HT2A receptor blockade with ketanserin (1.0 and 2.0 mg/kg, i. p.) increased this 5-H

Early postnatal stress alters the 5-HTergic modulation to emotional stress at postadolescent periods of rats.

Several lines of evidence have shown that traumatic events during the early postnatal stage might precipitate long-lasting alteration in the functional properties underlying emotional expression. The aim of the present study was to examine whether the early postnatal stress alters the 5-HTergic mechanism underlying regulation of emotional stress, focusing on the 5-HT(1A) receptor-mediated synaptic responses in adult rats. Pups were exposed to aversive stimulus, footshock (FS) at the postnatal period of the second (2W) and the third week (3W). At postadolescent period (10-12-week-old), electrop

Memory of an operant response and of depressed mood retained in activation states of 5-HT(1A) receptors: evidence from rodent models.

Three series of studies were conducted to specify the role of 5-HT(1A) receptors in memory; using selective ligands that differentially activate 5-HT(1A) receptors, it was determined whether a change in the activation state of these receptors can lead to deficient retrieval, and whether a so-produced deficit can occur in an animal model of depression. First, in vitro studies of [35S]GTPgammaS binding responses identified ligands that differentially activate 5-HT(1A) receptors in rat hippocampus. WAY 100635, 8-OH-DPAT and flesinoxan induced 5-HT(1A) receptor activation that amounted to -2, +50
