Validation of the DynaPort MiniMod during sleep: a pilot study.

To measure activity during sleep, polysomnography and actigraphy are often used. The DynaPort MiniMod measures movement intensity and body position day and night. The goal was to examine the validity of the DynaPort MiniMod in assessing physical activity and body posture during sleep. In Study A, 10 healthy participants slept with the DynaPort MiniMod and the Actiwatch for one night. In Study B, 8 participants suspected of having Obstructive Sleep Apnoea Syndrome slept for one night with the DynaPort MiniMod and underwent complete polysomnography as part of the typical care protocol. In Study

Sleep and circadian rhythm disruption in psychiatric and neurodegenerative disease.

Sleep and circadian rhythm disruption are frequently observed in patients with psychiatric disorders and neurodegenerative disease. The abnormal sleep that is experienced by these patients is largely assumed to be the product of medication or some other influence that is not well defined. However, normal brain function and the generation of sleep are linked by common neurotransmitter systems and regulatory pathways. Disruption of sleep alters sleep-wake timing, destabilizes physiology and promotes a range of pathologies (from cognitive to metabolic defects) that are rarely considered to be ass

Sleep-related factors and mobility in older men and women.

BACKGROUND: To examine the association between sleep-related factors and measured and self-reported mobility in a representative sample of older adults. METHODS: This study included 2,825 men and women aged 55 years and older participating in a cross-sectional representative population-based Health 2000 Survey in Finland. Sleep duration, insomnia-related symptoms, and fatigue were inquired. Maximal walking speed was measured, and mobility limitation was defined as self-reported difficulties in walking 500 m or stair climbing. RESULTS: Insomnia-related symptoms and fatigue were prevalent amon

Molecular and anatomical signatures of sleep deprivation in the mouse brain.

Sleep deprivation (SD) leads to a suite of cognitive and behavioral impairments, and yet the molecular consequences of SD in the brain are poorly understood. Using a systematic immediate-early gene (IEG) mapping to detect neuronal activation, the consequences of SD were mapped primarily to forebrain regions. SD was found to both induce and suppress IEG expression (and thus neuronal activity) in subregions of neocortex, striatum, and other brain regions. Laser microdissection and cDNA microarrays were used to identify the molecular consequences of SD in seven brain regions. In situ hybridizatio

The influences of time-of-day and sleep deprivation on postural control.

The aim of this study was to check the combined and/or dissociated influences of time-of-day and sleep deprivation on postural control. Twenty subjects participated in test sessions which took place at 6:00 am, 10:00 am, 2:00 pm and 6:00 pm either after a normal night's sleep or after a night of total sleep deprivation. Postural control was evaluated by COP surface area, LFS ratio and Romberg's index. The results showed that postural control fluctuates diurnally according to three different periods, pronounced by sleep deprivation: (1) at 6:00 am, there was no modification by sleep deprivation

Sleep and rhythm consequences of a genetically induced loss of serotonin.

BACKGROUND: A genetic deficiency in sepiapterin reductase leads to a combined deficit of serotonin and dopamine. The motor phenotype is characterized by a dopa-responsive fluctuating generalized dystonia-parkinsonism. The non-motor symptoms are poorly recognized. In particular, the effects of brain serotonin deficiency on sleep have not been thoroughly studied. OBJECTIVE: We examine the sleep, sleep-wake rhythms, CSF neurotransmitters, and melatonin profile in a patient with sepiapterin reductase deficiency. PATIENT: The patient was a 28-year-old man with fluctuating generalized dystonia-par

Sleep apnea is common in patients with coronary artery disease.

Sleep-disordered breathing (SDB) has a prognostic impact in patients with cardiac diseases. We included 257 patients with preserved left ventricular function and angiographically proven coronary artery disease (CAD). All patients underwent cardiorespiratory polygraphy. In 251 patients high-sensitive C-reactive protein and fibrinogen were measured. SDB was documented in 188 patients (apnea-hypopnea-index [AHI] 16.4+/- 1.9/h): 58 patients presented central sleep apnea (CSA) and 130 patients obstructive sleep apnea (OSA). All patients (73%) with SDB had higher blood fibrinogen levels than those w

Identification of deep sleep and awake with computational EEG measures.

The objective of the present work was to examine identification of deep sleep and awake with computational analysis of sleep EEG traces from central brain regions. All-night EEG traces from a total of 56 male subjects, 22 healthy control subjects and 34 age-matched apnea patients, were examined. A spectral mean frequency measure, a Hilbert transform based EEG amplitude and a correlation coefficient method were compared. The EEG amplitude provided a good identification of deep sleep, reaching 86.25% but was relatively poor in the identification of wakefulness, reaching 39.06%. Mean frequency pr

The central motor conductivity of genioglossus in obstructive sleep apnoea.

BACKGROUND AND OBJECTIVE: During wakefulness, the electromyography (EMG) activities of upper airway dilator muscles are higher in OSA syndrome (OSAS) patients than those in normal subjects. This is believed to be related to central compensatory mechanisms. This study aimed to assess the central motor conductivity of genioglossus (GG) during wakefulness and to evaluate the compensatory site in OSAS patients. METHODS: Twelve OSAS patients and 12 normal subjects were recruited to record motor evoked potential (MEP) of GG to transcranial magnetic stimulation applied at dominant-sided anterolatera

Role of the melanin-concentrating hormone neuropeptide in sleep regulation.

Melanin-concentrating hormone (MCH), a neuropeptide secreted by a limited number of neurons within the tuberal hypothalamus, has been drawn in the field of sleep only fairly recently in 2003. Since then, growing experimental evidence indicates that MCH may play a crucial role in the homeostatic regulation of paradoxical sleep (PS). MCH-expressing neurons fire specifically during PS. When injected icv MCH induces a 200% increase in PS quantities in rats and the lack of MCH induces a decrease in sleep quantities in transgenic mice. Here, we review recent studies suggesting a role for MCH in the

A transdiagnostic approach to treating sleep disturbance in psychiatric disorders.

Sleep disturbance commonly co-occurs with a range of psychiatric disorders. This is of concern given the accruing evidence that sleep is critically important for mood regulation, physical health, cognitive functioning, and quality of life. There is strong evidence that insomnia, even if it is comorbid with another psychiatric disorder, is treatable with cognitive behavioral therapy. There is a need to develop cognitive behavioral approaches to the other types of sleep disturbance often experienced by patients with psychiatric disorders, such as hypersomnia, reduced sleep need, delayed phase, n

The possible role of human milk nucleotides as sleep inducers.

Breast-milk contains a potent mixture of diverse components, such as the non-protein nitrogen fraction which includes nucleotides, whose variation in levels is evident throughout lactation. In addition, these substances play an important role in sleep homeostasis. In the present study, human milk samples were analyzed using a capillary electrophoresis system. The rhythmicity of each nucleotide was studied by cosinor analysis. It was found that the nucleotides 5'AMP, 5'GMP, 5'CMP, and 5'IMP have significant (P < 0.05) circadian rhythms, the acrophases of the first two being during the night, an

Effects of sleep fragmentation on glucose metabolism in normal subjects.

BACKGROUND: Sleep disorders are increasingly associated with insulin resistance, glucose intolerance, and type 2 diabetes mellitus. Whether the metabolic toll imposed by sleep-related disorders is caused by poor-quality sleep or due to other confounding factors is not known. The objective of this study was to examine whether experimental sleep fragmentation across all sleep stages would alter glucose metabolism, adrenocortical function, and sympathovagal balance. METHODS: Sleep was experimentally fragmented across all stages in 11 healthy, normal volunteers for two nights using auditory and m

Levodopa use and sleep in patients with dementia with Lewy bodies.

Sleep disturbance and excessive daytime somnolence (EDS) are features of Parkinson's disease (PD) and dementia with Lewy bodies (DLB) that may be influenced by dopamine replacement therapy. The effect of levodopa on sleep and EDS in DLB is unknown and unclear in PD. The aim of this study is to determine if levodopa treatment alters sleep symptoms and EDS in DLB. Dopamine naïve patients with DLB (n = 15; mean mini mental state examination (MMSE) score 17.7(4.6)) and PD (n = 9; mean MMSE 25.5(2.2)) were assessed using the Epworth sleep scale, Parkinson's disease sleep scale, and the neuropsychia
