Systemic Infection of Pediatric Osteomyelitis: A Clinical Case in a Low-Income Country.

Osteomyelitis is a severe bone infection that requires early diagnosis and targeted treatment, mainly in pediatric populations where clinical presentations can be complex and nonspecific. This case report presents a seven-year-old male from Los Ríos, Santo Domingo, Dominican Republic, diagnosed with methicillin-resistant Staphylococcus aureus (MRSA)-associated osteomyelitis. The child presented with systemic signs, including fever, vomiting, and abdominal pain, along with localized trauma, limb pain, and functional limitations. Clinical examination revealed hemodynamic instability and gastroin

Erythema Nodosum Leprosum Triggered by Herpes Simplex Infection: A Case Report.

Leprosy, caused by Mycobacterium leprae, is a chronic granulomatous disease primarily affecting the skin and peripheral nerves. Erythema nodosum leprosum (ENL), or type 2 lepra reaction, is an immunologic complication of lepromatous leprosy, often presenting with erythematous nodules, fever, and systemic inflammation. Early recognition is crucial to prevent chronic complications. We report a 44-year-old man who developed facial erythematous nodules and fever following a sore throat, initially suggestive of a drug eruption or viral infection. Laboratory tests revealed neutrophilia, elevated inf

Potential Compounds as Inhibitors of Staphylococcal Virulence Factors Involved in the Development of Thrombosis.

For many years, staphylococci have been detected mainly in infections of the skin and soft tissues, organs, bone inflammations, and generalized infections. Thromboembolic diseases have also become a serious plague of our times, which, as it turns out, are closely related to the toxic effects of staphylococci. Staphylococcus aureus, because of the presence of many different kinds of virulence factors, is capable of manipulating the host's innate and adaptive immune responses. These include toxins and cofactors that activate host zymogens and exoenzymes, as well as superantigens, which are highl

Clinical, molecular, and histological characteristics of severely necrotic and fatal mpox in HIV-infected patients.

BACKGROUND: This case series of 5 patients with severely necrotic mpox highlights the predominantly necrotic nature of lesions seen in cases of severe mpox as shown by skin and lung biopsy, as well as the extensive dissemination of the infection, as shown by polymerase chain reaction (PCR) assessment in different body sites. CASE PRESENTATIONS: Patients were male, the median age was 37, all lived with HIV (2 previously undiagnosed), the median CD4+ cell count was 106 cells/mm3, and 2/5 were not receiving antiretroviral treatment. The most common complication was soft tissue infection. Skin an

New Concepts in the Management of Charcot Neuroarthropathy in Diabetes.

Charcot Neuroarthropathy (CN) is an uncommon, debilitating and often underdiagnosed complication of chronic diabetes mellitus though, it can also occur in other medical conditions resulting from nerve injury. Till date, the etiology of CN remains unknown, but enhanced osteoclastogenesis is believed to play a central role in the pathogenesis of CN, in the presence of neuropathy. CN compromises the overall health and quality of life. Delayed diagnosis can result in a severe deformity that can act as a gateway to ulceration, infection and in the worst case, can lead to limb loss. In an early stag

Percutaneous screw fixation for painful non-union of lateral malleolus ankle fractures.

INTRODUCTION: Lateral malleolus non-union can cause pain and loss of function. Standard treatment involves open approach with debridement, bone-grafting and plate stabilisation, with published surgical site infection rates to 17%. To minimise the risk of soft tissue complications and allow early mobilisation, we describe a technique for percutaneous cannulated screw stabilisation. MATERIALS AND METHODS: Retrospective case review for all percutaneous lateral malleolus non-union stabilisation procedures undertaken in our hospital between 2011 and 2017 was performed. Fracture union was diagnosed

Effect of underlying immune compromise on the manifestations and outcomes of group A streptococcal bacteremia.

BACKGROUND: Group A streptococcal bloodstream infection is the most common presentation of invasive group A streptococcal disease. We sought to determine the impact of immunosuppression on severity of disease and clinical outcomes. METHODS: This retrospective review of 148 patients with at least one positive blood culture for Streptococcus pyogenes from 1/2003 to 3/2013 compared immunocompromised patients with those with no immunocompromise in regards to development of severe complications and mortality. RESULTS: Twenty-five patients (17%) were immunocompromised; 123 were not. Skin and soft

Cutaneous Pseudallescheria boydii/Scedosporium apiospermum Complex (Molecular type: Scedosporium apiospermum [Clade 4]) Infection: A Case Report and Literature Review of Cases from Japan.

We report a case of subcutaneous Pseudallescheria boydii/Scedosporium apiospermum complex infection occurring in a 77-year-old Japanese female farmer suffering from interstitial pneumonia. Seven months prior to the current presentation, she noticed nodes on her right forearm after pulling up weeds, and the nodes grew larger. Two soft dome-shaped, protruded nodes ( 15 mm and 30 mm in size ) had fused together on the extensor surface of the right forearm. Yellowish-white, rice-grain-sized pustules clustered on the surface. Histopathological examination of the skin specimen showed large and small

Arthroscopic tibiotalocalcaneal arthrodesis with intramedullary nail with fins: a case series.

Arthroscopic arthrodesis of the ankle has several advantages compared with open arthrodesis, including a smaller skin incision, less damage to the soft tissue around the joint, a lower risk of skin necrosis and infection, a lower incidence of postoperative infection and swelling, and better preservation of the contour of the surface of the joint, which maintains a larger contact area between the talus and tibia. We successfully performed arthroscopic arthrodesis of the tibiotalocalcaneal joints with intramedullary nails with fins in 9 ankles of 8 patients. Solid fusion was attained in all case

[SIRS and ARDS as a result of drug injection in the shoulder region].

Bacterial infections with local inflammation or hematogenous spreading may occur after joint punctures and intra- or periarticular injections. The risk of severe infections increases in patients with diseases accompanied by low immunity, e.g., gout, alcoholism, rheumatoid arthritis, and diabetes mellitus. Cases of septic omarthritis after intra-articular injection with fatal outcome after delayed onset of therapy are known. In our clinic we treated a female patient who previously received an injection in the shoulder region in a different facility. On admission she was suffering from an absces

Bacillary angiomatosis in an HIV-positive man with multiple risk factors: A clinical and epidemiological puzzle.

A 36-year-old man with advanced AIDS presented with skin lesions, fever and weight loss. Physical examination revealed skin lesions suggestive of bacillary angiomatosis. Diagnostic imaging identified bone lesions, soft tissue masses and a single hepatic lesion. His clinical presentation was most consistent with Bartonella quintana; however, his risk for infection with B quintana was limited to a remote, brief period of homelessness. In contrast, his frequent contact with cats presented substantial potential exposure to Bartonella henselae.Diagnosing a Bartonella infection presents challenges.

High incidence of ischemic necrosis of the gluteal muscle after transcatheter angiographic embolization for severe pelvic fracture.

BACKGROUND: After transcatheter angiographic embolization (TAE), massive gluteal muscle necrosis was found in patients during open reduction and internal fixation for pelvic fracture. METHODS: In our six patients, magnetic resonance imaging (MRI) scans obtained 1 and 4 weeks after TAE demonstrated ischemic damage of the gluteal muscle. RESULTS: Total reopening of the embolized artery was confirmed in only one case on the second angiogram obtained 1 month after TAE. In five patients, massive muscle necrosis, previously confirmed on MRI, was macroscopically found during open reduction and inte

[Diagnosis and therapy of necrotizing fasciitis].

Necrotizing fasciitis is a soft tissue infection with a lethality ranging up to 80%. Infection causes the activation of interleukin, tumor necrosis factor alpha, and gamma-interferon through a triggering mechanism. This results in a capillary thrombosis with necrosis of the fascia, cutis, and subcutis. The patient's history often reveals a triggering event in the form of a recent minimal trauma or operative procedure. In a fulminant necrotizing fasciitis, the development of sepsis with consecutive multiple-organ failure mainly determines the outcome of the disease. Diagnosis is made initially

War injuries of the talus.

Twenty-eight patients with war fractures of the talus were reviewed to determine the incidence of infection and late functional results. The average age of our patients was 37.5 years. Twenty-one injuries were explosive fractures that resulted from shell fragments or mines; seven fractures resulted from gunshot missiles. Seven patients had isolated talus injuries, whereas 21 patients had associated fractures of other bones of the foot or lower leg. All patients were initially treated with debridement, tetanus immunization, intravenous antibiotics, and stabilization, either with cast immobiliza

[Arterial disease of the lower limbs in diabetic patients].

Lower limb arterial disease in diabetics resembles that in non diabetics. However, some important differences include the vessels involved and the extent of the involvement. In the diabetic, the arteries most frequently involved are those below the knee. Arterial occlusions are bilateral, multisegmental, and involve unusual vessels such as the internal iliac artery, the deep femoral artery, the small branches and the collateral circulation. Arterial disease in the diabetic appears at a younger age, advances more rapidly, is more diffuse, and is almost as common in women as in men. Interaction

The 1990-1991 outbreak of melioidosis in the Northern Territory of Australia: clinical aspects.

From November 1990 to June 1991, 33 cases of acute melioidosis were diagnosed in tropical Northern Territory, Australia during an exceptionally wet monsoon. Eighteen (55%) were alcoholic, 16 (48%) diabetic and only 4 (12%, all survivors) had no risk factors. Twenty-seven (82%) were considered recent infection, with an incubation period of 3-21 days (mean 14) documented in eight cases with presumed cutaneous inoculation. Fourteen patients presented with pneumonia (4 septicemic) and of 11 others with septicemia 4 had genitourinary foci. Three of 4 with splenic abscesses required splenectomy. Thr

[Surgical therapy of the acne tetrad].

On the basis of a 3 years experience with surgical treatment of acne tetrad (acne conglobata, apocrine acne) in 22 patients, we can draw the following conclusions: 1. Radical surgical excision yields better long-term results than local incision. Recurrences are mostly the result of non-radical surgery. 2. In cases of limited acne--especially if it is localized either in the axilla or the groin--primary wound closure may be successfully attained. 3. Gentamycin chains may support the primary wound healing in defect closures of infected areas. 4. In the axilla, we suggest wound closure by myocuta

Lymphatic Endothelial Cells Regulate Neutrophil Phenotypes and Function in a Microphysiological Model of Infection.

Early skin inflammation requires coordinated immune regulation, with neutrophils acting as first-line responders.
