PURPOSE: Amblyopia treatment occasionally is complicated by patch intolerance, resulting in a poor outcome. Therefore, a prospective study was designed to examine the efficacy and safety of using occlusive contact lenses in the treatment of amblyopia.
METHODS: Twenty-five patients, aged 2.5 to 9.5 years, who were using an opaque hydrogel contact lens for the treatment of amblyopia, were followed prospectively.
RESULTS: Overall, 92% of patients improved at least one line of visual acuity. Eight patients improved > or = 2 octaves and 12 other patients improved > or = 1 octave. In three patient

BACKGROUND: The Frisby stereotest commonly is used in clinical practice to estimate stereoacuity. Assessment of the presence or absence of stereopsis is valuable particularly in toddlers because of the difficulties encountered in this age group with assessment of other aspects of visual function, such as monocular visual acuities.
METHODS: The present study describes two modifications to the Frisby stereotest: 1) the introduction of a nonstereo practice plate; and 2) the use of an auditory "reward" for correct identification of the target. These modifications aim to increase the success rate

BACKGROUND: Glaucoma occurring after successful cataract surgery in infancy and childhood requires persistence to establish timely diagnosis and presents a therapeutic challenge. Previous studies have sought to identify risk factors, which would alert the clinician for the likelihood of developing glaucoma after cataract surgery in the young patient. Some risk factors are early age of surgery, microcornea, poor pupillary dilation, retained lens cortex, coexisting ocular anomalies, and cataract type. Other studies did not find these associations. In our experience, the majority of young aphakic

BACKGROUND: Abnormally increased diameter and tortuosity of retinal blood vessels in the posterior pole, or "plus disease," is recognized as a powerful predictor of poor outcome in eyes with retinopathy of prematurity (ROP). Although the diagnosis of plus disease depends upon the examiner's ability to examine retinal blood vessels, the ability of the human observer to identify changes in retinal blood vessel diameter and tortuosity accurately has not been studied.
METHODS: Using computer-aided analysis of fundus photographs from eyes with a wide range of ROP severity, we generated tracings of

OBJECTIVE: A retrospective study was performed to examine the relationship between the severity of vitreous hemorrhages and the severity of neurological injury in infants with shaken baby syndrome, and the result of early vitrectomy in these infants.
METHODS: Five infants, ages 3 to 8 months, with confirmed child abuse underwent ocular examination and neurological testing to identify varying degrees of vitreoretinal changes and neurological dysfunction. Pars plana vitrectomy was performed on one or both eyes in each infant to remove the vitreous hemorrhage.
RESULTS: The three infants with bi

The neurofibromatoses are a set of at least two distinct disorders characterized by the development of nerve sheath tumors and diverse other features. Both are genetically determined as autosomal dominant traits, and both genes have been identified, with resultant major insights into pathogenesis. The neurofibromatoses have prominent effects on the visual system, which are important both for diagnosis and clinical management. This review focuses on clinical aspects of the neurofibromatoses, highlighting ophthalmological issues and recent advances in molecular genetics.

BACKGROUND: Some standards for childhood ophthalmological variables exist. However, for most parameters, the range of variation found in normal children remains poorly defined.
METHODS: We have determined the range of normal function for a number of commonly measured ophthalmological variables in 162 children, classified normal by Gold Standard ophthalmological examination. This group is representative of almost 12,000 normal children aged 4 1/2 to 5 1/2 in Nova Scotia.
RESULTS: In 56.8% of eyes the visual acuity was 6/4.5. The use of a chart with crowding bars was more discriminating betwee

Neonatal dacryocystitis is a rare complication of congenital nasolacrimal duct (NLD) obstruction. Recent reports suggest that the association of neonatal dacryocystitis and NLD cysts may be more common than previously believed. The purpose of this study is to describe the nasal endoscopic findings and treatment of three patients with this disorder. In this prospective study, three consecutive patients who presented at 2 weeks of age or less with dacryocystitis were treated with NLD probing and rigid nasal endoscopy. All patients were found to have congenital NLD cysts at the time of NLD probin

Premature dislocation, a potential disadvantage of conventional, unilateral silicone intubation for lacrimal disease, can be alleviated with a firmly-tied, bilateral, superior canaliculus intubation. Since the looped silicone stent can be removed easily at home by parents, this technique is particularly beneficial for remote patients for whom postoperative, clinic removal is a financial burden.

The visual function of some children with structural defects of the macula or optic nerve has previously been shown to improve with occlusion therapy. The charts of five children, ages 4 to 8 years, who had various types of severe structural abnormalities were reviewed. Two patients had optic nerve hypoplasia; there was one case each of foveal hypoplasia, posterior persistent hyperplastic primary vitreous, and retinopathy of prematurity. All the patients had undergone extensive amblyopia therapy prior to referral. In three children, occlusion therapy had been initiated and continued intensivel

Dilation and tortuosity of the posterior pole vessels "(plus disease)" is a sign of poor prognosis and may be associated with threshold or prethreshold retinopathy of prematurity (ROP). We have found that normal posterior pole vessels are a reliable marker for the absence of stage 3 ROP. One hundred thirty-two consecutive premature infants weighing less than 1600 g at birth underwent ROP examinations between 32 and 40 weeks' postconceptional age. The status of the posterior pole vessels was compared to peripheral retinal pathology. Vascular findings in the posterior pole were graded 0, 1, 2, o

This study evaluates functional vision in visually impaired children with stage IV to V cicatricial retinopathy of prematurity (CROP) by comparing results derived from the Teller acuity cards and a battery of behavioral tasks, the visual function battery (VFB). Vision was evaluated in 26 eyes of 15 children ages 6 months to 14 years, who underwent retinal reattachment procedures for active stage IV and stage V retinopathy of prematurity. Results showed that 10 of 26 eyes (38%) had a recordable grating acuity to Teller acuity card assessment. Twenty-five of 26 eyes (96%) showed a response to th

BACKGROUND: Dilation and tortuosity of the posterior pole vessels ("plus disease") is a poor prognostic sign and may indicate the presence of threshold or pre-threshold retinopathy of prematurity (ROP). Because stage 3 ROP appears rarely in the absence of posterior pole vascular abnormalities, we prospectively evaluated the ability of non-ophthalmologists (4th-year medical students, pediatric residents, and nurse practitioners) to identify abnormalities in the retinal blood vessels of infants undergoing examinations for ROP.
METHODS: Fifty infants weighting less than 1600 g at birth were exam

Measuring the intraocular pressure (IOP) in children can be difficult. We compared the accuracy of IOP measurements in pediatric patients using three tonometers: Perkins, Schiötz, and Tono-pen. Seventy-seven children (132 eyes; average age, 1 year, 7 months; range, 1 month to 60 months), mainly afflicted with retinopathy of prematurity (107 eyes), were evaluated. A good correlation (r = 0.867) was found between the Tono-pen and the Perkins tonometers. No statistical difference was found between the mean difference of IOP values obtained with the Tono-pen and the Perkins (P > .05). The Schiötz
