Leksell Gamma Knife treatment of tic douloureux.

Stereotactic radiosurgery with the Leksell Gamma Knife under local anesthesia can effectively treat patients with recurrent tic douloureux after unsuccessful medical/surgical procedures. Eight of 12 patients have shown complete relief or improvement of their trigeminal neuralgia after 3 to 4 year follow-up. No complications have been observed, with the exception of 1 patient who developed a small area of radionecrosis in the medial temporal lobe.

Spatiotemporal characteristics of the neural representation of event concepts.

Events are a fundamentally important part of our understanding of the world. How lexical concepts denoting events are represented in the brain remains controversial. We conducted two experiments using event and object nouns matched on a range of psycholinguistic variables, including concreteness, to examine spatial and temporal characteristics of event concepts. Both experiments used magnitude and valence tasks on event and object nouns. The fMRI experiment revealed a distributed set of regions for events, including the angular gyrus, anterior temporal lobe, and posterior cingulate across task

Zero-crossing patterns reveal subtle epileptiform discharges in the scalp EEG.

Clinical diagnosis of epilepsy depends heavily on the detection of interictal epileptiform discharges (IEDs) from scalp electroencephalographic (EEG) signals, which by purely visual means is far from straightforward. Here, we introduce a simple signal analysis procedure based on scalp EEG zero-crossing patterns which can extract the spatiotemporal structure of scalp voltage fluctuations. We analyzed simultaneous scalp and intracranial EEG recordings from patients with pharmacoresistant temporal lobe epilepsy. Our data show that a large proportion of intracranial IEDs manifest only as subtle, l

Remote photonic sensing of cerebral hemodynamic changes via temporal spatial analysis of acoustic vibrations.

A novel photonic method for remote monitoring of task-related hemodynamic changes in human brain activation is presented. Physiological processes associated with neural activity, such as nano-vibrations due to blood flow and tissue oxygenation in the brain, are detected by remote sensing of nano-acoustic vibrations using temporal spatial analysis of defocused self-interference random patterns. Temporal nanometric changes of the speckle pattern due to visual task-induced hemodynamic responses were tracked by this method. Reversing visual checkerboard stimulation alternated with rest epochs, and

The Landau-Kleffner syndrome of acquired epileptic aphasia: unusual clinical outcome, surgical experience, and absence of encephalitis.

The syndrome of acquired verbal auditory agnosia in childhood with mutism and epileptic discharges has been described in over 100 cases. An encephalitic etiology has often been postulated but never proved. We report two patients with this syndrome who were treated surgically. Despite careful search, no pathologic evidence of encephalitis was found. One patient, with the typical course, had no seizures but striking positive correlation between epileptic discharge and language disorder; the second, after classic onset, developed intractable temporal lobe epilepsy, a previously unreported outcome

Correlation of findings in advanced MR techniques with global severity scales in patients with some grade of cognitive impairment.

INTRODUCTION: Some previous studies in patients with mild cognitive impairment and Alzheimer's disease have probed changes in the results of (1)H magnetic resonance spectroscopy and perfusion- and diffusion-weighted imaging. The purpose of this work was to correlate the results of perfusion- and diffusion-weighted imaging and magnetic resonance spectroscopy with the results of two global severity scales in cognitive impairment: the clinical dementia rating (CDR) and the global deterioration scale (GDS). PATIENTS AND METHODS: We evaluated 87 patients with cognitive impairment of diverse grade

Executive Functions in Healthy Older Adults Are Differentially Related to Macro- and Microstructural White Matter Characteristics of the Cerebral Lobes.

Aging is associated with microstructural white matter (WM) changes. WM microstructural characteristics, measured with diffusion tensor imaging (DTI), are different in normal appearing white matter (NAWM) and WM hyperintensities (WMH). It is largely unknown how the microstructural properties of WMH are associated with cognition and if there are regional effects for specific cognitive domains. We therefore examined within 200 healthy older participants (a) differences in microstructural characteristics of NAWM and WMH per cerebral lobe; and (b) the association of macrostructural (WMH volume) and

Stereotactic biopsy for lesions in brainstem and deep brain: a single-center experience of 72 cases.

Stereotactic biopsies for lesions in the brainstem and deep brain are rare. This study aimed to summarize our 6-year experience in the accurate diagnosis of lesions in the brain stem and deep brain and to discuss the technical note and strategies. From December 2011 to January 2018, 72 cases of intracranial lesions in the brainstem or deep in the lobes undergoing stereotactic biopsy were retrospectively reviewed. An individualized puncture path was designed based on the lesion's location and the image characteristics. The most common biopsy targets were deep in the lobes (43 cases, 59.7%), inc

Distribution and histopathological changes induced by cysts of Taenia solium in the brain of pigs from Tanzania.

Neurocysticercosis (NCC) caused by Taenia solium cysts is a frequent but neglected parasitic disease of the central nervous system (CNS) worldwide. The aim of this study was to describe anatomical locations of cysts in the CNS and the corresponding inflammation. A total of 17 naturally infected pigs were used to evaluate the distribution of cysts and, of these, seven were used to evaluate the corresponding inflammation further, through histopathology. Clinical signs in the pigs included dullness, sluggishness, somnolence, apathy and loss of consciousness. Cysts were distributed in all cerebral

Pediatric frontal lobe epilepsy: white matter abnormalities and cognitive impairment.

OBJECTIVES: Cognitive impairment is frequent in children with frontal lobe epilepsy (FLE). Its etiology remains unknown. With diffusion tensor imaging, we have studied cerebral white matter properties and associations with cognitive functioning in children with FLE and healthy controls. METHODS: Thirty children aged 8-13 years with FLE of unknown cause and 39 healthy age-matched controls underwent neuropsychological assessment, structural and diffusion-weighted brain MRI. Patients were grouped as cognitively impaired or unimpaired, and their white matter diffusion properties were compared wit

Corpus callosal connection mapping using cortical gray matter parcellation and DT-MRI.

Population maps of the corpus callosum (CC) and cortical lobe connections were generated by combining cortical gray matter parcellation with the diffusion tensor fiber tractography of individual subjects. This method is based on the fact that the cortical lobes of both hemispheres are interconnected by the corpus callosal fibers. T1-weighted structural MRIs and diffusion tensor MRIs (DT-MRI) of 22 right-handed, healthy subjects were used. Forty-seven cortical parcellations in the dorsal prefrontal cortex, ventral prefrontal cortex, sensory-motor cortex, parietal cortex, temporal cortex, and oc

The neural basis of semantic memory: evidence from semantic dementia.

Semantic dementia (SD) is a syndrome of progressive impairment in semantic memory. Fifty-eight brain regions were measured in seven post mortem SD cases, ten normal controls and two disease controls (diagnosis frontotemporal dementia and motor neuron disease, FTD-MND). Manual segmentation of the whole brain has not previously been undertaken in a series of SD cases, either post mortem or during life. Widespread volume loss relative to controls was found in SD, with anterior temporal lobe regions bearing the brunt (>60% atrophy of temporopolar and perirhinal cortices bilaterally). Comparison of

Differences in regional blood volume during a 28-day period of abstinence in chronic cannabis smokers.

Cerebral blood volume (CBV) studies have provided important insight into the effects of illicit substances such as cannabis. The present study examined changes in regional blood volume in the frontal and temporal lobe, and the cerebellum during 28 days of supervised abstinence from cannabis. Dynamic susceptibility contrast MRI (DSCMRI) data were collected on 15 current, long-term cannabis users between 6 and 36 h after the subjects' last reported cannabis use (Day 0), and again after 7 and 28 days of abstinence. Resting state CBV images were also acquired on 17 healthy comparison subjects. The

P300 in schizophrenia: confirmation and statistical validation of temporal region deficit in P300 topography.

Comparison of normal and medicated schizophrenic groups on the auditory P300 component of the event-related potential confirmed our earlier finding of a left temporal deficit in P300 amplitude in schizophrenia. A difference in P300 topography between groups was evident in both color mapping and in grand-averaged waveforms, which was statistically validated by the presence of a group-by-scalp region interaction (p less than 0.05). The left temporal area in schizophrenics was denoted as the region of greatest deficit and of maximal statistical separation (p less than 0.05) relative to normals by

Evaluation of functional changes in the medial temporal region during intracarotid amobarbital procedure by use of SPECT.

PURPOSE: Because the main blood supply to the medial temporal region is through the posterior cerebral artery, the validity of the intracarotid amobarbital procedure (IAP) is still debated. To verify clinical validity of the IAP, we investigated changes in neuronal function in the medial temporal region during IAP. METHODS: Brain single photon emission computed tomography (SPECT) was performed during IAP (IAP-SPECT) in 22 patients with temporal lobe epilepsy (TLE), and regional cerebral blood flow (rCBF) in the medial and lateral temporal regions of interest (ROIs) was measured quantitatively

Neuroimaging findings in twins discordant for Alzheimer's disease.

Data from computed tomography (CT) scans of 12 twin pairs in which one partner had Azheimer's disease (AD) and the other partner is cognitively intact were analyzed to study structural brain features associated with AD while controlling for familial factors. Visual ratings and analysis of quantified areas and volumes indicated that AD twins showed more dilation of temporal horns, lateral ventricles and third ventricle, and more atrophy of temporal lobes, particularly in the anterior temporal/perisylvian area, than their healthy cotwins. Demented twins did not have smaller intracranial areas or

Auditory agnosia. Analysis of a case with bilateral subcortical lesions.

We report a case of typical auditory agnosia. It is unique in that the clinical picture began with generalized auditory agnosia for verbal and nonverbal sounds and changed, over a period of two months, to a restricted auditory agnosia, confined to the recognition of nonverbal sounds. The lesions, confirmed by CT scanning and MRI, were subcortical bilaterally, without evidence of cortical damage, and have not previously been described. The results of audiological studies on temporal discrimination were similar to those described for bilateral temporal lobe lesions. The relationships between cli

Migrainous infarction with appearance of laminar necrosis on MRI.

Although there is accepted criteria for migrainous infarction, the pathophysiology of this disease process remains poorly defined. Delineation of MRI characteristics of migrainous infarction may improve our understanding of the pathophysiology, diagnosis, and prognosis of this disorder. We present a case of a migrainous infarction that involved apparent laminar necrosis of a "cortical ribbon" of the right temporal, parietal, and occipital lobes.
