Clinical Characteristics associated with functional seizures in individuals with psychosis.

BACKGROUND AND HYPOTHESIS: Functional seizures (FS) are episodes characterized by seizure-like events that are not caused by hypersynchronous neuronal activity. Prior studies have suggested an increased prevalence of psychotic disorders among patients with FS, but results have been inconsistent. We hypothesize that FS are associated with psychosis and that among patients with psychosis, the presence of FS may influence patient clinical characteristics, mortality, and medical resource utilization. STUDY DESIGN: The association between FS and psychosis was assessed using electronic health recor

Thoracic Endovascular Aortic Repair for Blunt Thoracic Aortic Injury: Impact of Arch Type on the Rate of Type 1 Endoleak.

BACKGROUND: In recent years, thoracic endovascular aortic repair (TEVAR) has emerged as the standard of care for management of blunt thoracic aortic injury (BTAI), resulting in reduced rates of in-hospital mortality. We identified factors, including arch type, that may impact endoleaks among these patients. METHODS: The Aortic Trauma Foundation registry was used to identify patients with BTAI managed with TEVAR from 2012 to 2023 for whom arch type was documented. Type 1 endoleak rates, graft specifications, lesion characteristics, demographics, and mechanism of injury were compared. RESULTS:

The Thoracoepigastric Flap: An Effective Reconstructive Method for Large Upper Extremity Defects in Low- and Middle-income Countries-A Single-center Study.

BACKGROUND: Injury to the upper limbs, among the most injured anatomical regions of the body, often results in the loss of the skin envelope and the exposure of vital structures, including bone and neurovascular bundles. Reconstructive options in resource-constrained environments are curtailed by both equipment and skilled manpower, necessitating avoidance of resource-intensive techniques. Thoracoepigastric flap (TEF) reconstructions of the forearm, wrist, and hand are simple to perform, provide a sizeable versatile integument cover, and avoid the hygiene and edema complications associated wit

Decoding the Enigma of Gallbladder Perforation: Exploring Patient Profiles, Diagnosis, and Step-Up Management Pathways.

BACKGROUND AND AIM: Gallbladder perforation (GBP) is a serious, life-threatening condition that can result from acute cholecystitis, trauma, or gallstone disease. Diagnosis remains challenging due to its nonspecific symptoms, often mimicking other abdominal pathologies. This study aimed to investigate the clinical features, diagnostic approaches, management strategies, and outcomes of GBP at a tertiary care center. METHODS: A retrospective cohort study was conducted at Vardhman Mahavir Medical College (VMMC) and Safdarjung Hospital from January 2022 to February 2025. Patients aged 18 years an

Stealth Navigation with iFuse-TORQ Implant for Sacroiliac Joint Fusion Technique Guide.

INTRODUCTION: Road traffic accidents (RTA) account for a sizable portion of morbidity and mortality globally, with a particularly high incidence among young and active individuals. Patients presenting with polytrauma require a multidisciplinary approach guided by protocols for advanced trauma life support. CASE REPORT: We report the case of a 31-year-old female, transferred-in to our center following primary care after an RTA on June 17th, 2023. Initial assessment at the other hospital found multiple fractures accompanying her internal injuries - a right haemothorax and pulmonary contusion, w

Identifying patterns of pain, depression, anxiety, interpersonal trauma exposure, and nonmedical prescription opioid use: Latent class analysis among patients with chronic pain.

BACKGROUND: Chronic pain in the context of certain factors may be associated with potential for nonmedical prescription opioid use; however, identifying this risk can be challenging and complex. Several variables alone have been associated with non-prescribed opioid use, including depression, anxiety, pain interference, and trauma exposure. Prior research has often failed to integrate these assessments together, which is important as these factors may cluster together in important and complex ways. The current study aimed to identify classes of patients with chronic pain who have differential

Thoracic Ultrasound for Detection of Pneumothorax Following Thoracostomy Tube Removal in Trauma Patients.

INTRODUCTION: Screening for pneumothorax (PTX) is standard practice after thoracostomy tube removal, with postpull CXR being the gold standard. However, studies have shown that point-of-care thoracic ultrasound (POCTUS) is effective at detecting PTX and may represent a viable alternative. This study aims to evaluate the safety and efficacy of POCTUS for evaluation of clinically significant postpull PTX compared with chest x-ray (CXR). METHODS: We performed a prospective, cohort study at a Level 1 trauma center between April and December 2022 comparing the ability of POCTUS to detect clinicall

Impact of emergency department boarding on patients outcomes in hip fractures.

BACKGROUND: Boarding time in the Emergency Department (ED) is an area of concern for all patients and potentially more problematic for the hip fracture population. Identifying patient outcomes impacted by ED boarding and improving emergent care to reduce surgical delay for this patient population is a recognized opportunity. The objective of this study is to examine the impact of ED boarding in relation to patient outcomes in the surgical hip fracture population. METHODS: This is a retrospective study of hip fracture patients who presented at the ED of a Level 1 trauma center between January

Corticobasal Syndrome: Are There Central or Peripheral Triggers?

BACKGROUND AND OBJECTIVES: Corticobasal syndrome (CBS) is a complex of symptoms and signs comprising limb rigidity, bradykinesia, dystonia, myoclonus, apraxia, cortical sensory loss, and a variety of cognitive and language impairments. CBS is commonly seen in tauopathies. Striking asymmetry in clinical and imaging findings in CBS raises questions about potential triggers initiating neurodegeneration. The objective of this study was to investigate potential central or peripheral triggers preceding CBS symptoms. METHODS: In this retrospective observational study, we reviewed medical records of

More Harm Than Good: It is Time to Reconsider Prophylactic Fasciotomy in Lower-Extremity Vascular Injury.

IntroductionFour-compartment calf fasciotomy (CF) can be limb-saving. Prophylactic fasciotomy (PP) is advised in high-risk situations to prevent limb loss. Calf fasciotomy can cause significant morbidity, particularly if performed unnecessarily. We hypothesized that selective use of fasciotomies (SF) after lower-extremity vascular injury would lead to a lower rate of overall fasciotomies without an increase in limb complications than prophylactic fasciotomies (PFs).MethodsTrauma patients who sustained lower-extremity vascular injury that required operative repair at a high-volume trauma center

Orthodontic treatment of traumatically avulsed maxillary central incisors with bimaxillary dentoalveolar protrusion in an adult female: a case report.

BACKGROUND: Clinicians agree that obtaining and retaining good treatment results for missing maxillary central incisors owing to trauma is not easy. Management of adult patients with permanent maxillary central incisor loss who visit the clinic with high expectations for aesthetics and function pose a significant diagnostic dilemma. Therefore, esthetic and functional outcomes should be taken into consideration when deciding the proper treatment method. The treatment described in this study aimed to reestablish smile esthetics by proposing an effective multidisciplinary clinical approach that i

Comparison of Lateral versus Medial Entry Femoral Traction Pin Complication Rates.

Distal femoral skeletal traction is a common procedure for the stabilization of fractures of the pelvis, acetabulum, and femur following trauma. Femoral traction pins are traditionally inserted via medial-to-lateral (MTL) entry to accurately direct the pin away from the medial neurovascular bundle. Alternatively, cadaveric studies have demonstrated low risk to the neurovascular bundle using a lateral-to-medial (LTM) approach. The purpose of this study was to compare the incidence of complications of LTM and MTL femoral traction pin placement at a single institution. This was a retrospective re
