In the light of an analysis of late results of surgical treatment obtained in a group of 2450 patients with chronic gastric or duodenal ulcer subjected to various types of operations the author stresses the importance of choice of a proper surgical method. It is concluded that in treatment of chronic gastric ulcer the most appropriate method is partial gastrectomy including the whole pyloric part of the stomach. Intermittent electrical stimulation of the left cervical vagus increased intragastric pressure and induced a 100% incidence of haemorrhagic ulcers in the glandular mucosa of rat stomachs. Atropine pretreatment of sub-diaphragmatic vagotomy prevented these effects. The findings substantiate the idea that stress-induced glandular ulcers result from vagal-mediated increased gastric contractions.

Corrective surgical procedures for postgastrectomy syndromes are well-established in abdominal surgery. The indications for early corrective intervention are clearly defined on the basis of the patients' postoperative course. Corrections at a later date should only be considered in presence of major symptoms which did not respond to conservative therapy and following careful morphological and functional assessment of the patient. The possibility of healing of a corticosteroid-induced ulcer depsite the continuation of the treatment, is now an established fact. It is thus reasonable to continue corticosteroids in those cases where interupption severely compromises the prognosis. The chances of healing of the ulcer would appear to be great, especially when corticosteroids are being used in low dosage, but the exact frequency of healing remains unknown, just as the frequency of ulcers due to corticosteroid therapy has not been accurately established.

It is the authors' opinion that low values of acid production do not exclude the importance of the acid-peptic factor in formation of duodenal ulcers. Vagotomy and pyloroplasty in most patients showing duodenal ulcer with reduced posthistamine secretion provide for good results.

The clinical status, secretory and motor activity of the stomach were studied in 213 patients showing ulcerous disease prior to surgery and in 153 operated patients. All (151) adult patients in north-east Scotland newly diagnosed in one year (1967) as having a supposedly benign definite gastric ulcer crater were studied after a minimum of four years, with a 98 per cent follow-up.
