[Brucella keratoconjunctivitis].

A case is described in which the rubber cap of a vial containing Brucella abortus vaccine came off accidentally and some of the contents spl … in two hours of handling Brucella vaccine. In the right eye a corneal abscess developed, as well as uveitis and cataract. Extracapsular lens extraction was performed. Panophthalmitis followed and the eye was enucleated. Superficial reticular degeneration of Koby.

A 36-year-old white man with congenital ichthyosis and a history of a posterior uveitis beginning at age 8 presented with an unusual corneal opacity. At age 26 bilateral central corneal opacities were noted consisting of fine interlacing white lines forming a reticulum at the level of Bowman's membrane. A faint brown background tint of the basal epithelium was also present. Over the next 10 years the opacity progressed only slightly, with some mild superficial scarring of the underlying corneal stroma. The visual acuity remained good. This corneal pattern appeared virtually identical to the su…

[Clinico-immunological study of 6 cases of Behçet's disease].

6 cases of Behçet's disease are reported. Diagnosis was particularly arduous because the typical triad of symptoms (oral aphthae, genital ulcers, uveitis) was masked by secondary disturbances in other organs and systems. Immunology confirmed the presence in these patients of changes in various in vivo and in vitro tests and particularly interesting was the aspecific cutaneous hypersensitivity seen in 5 cases and enhanced lymphocyte blastic transformation. This was seen spontaneously and following PHA. On the basis of the outcome of the studies carried out and the literature data, the possibili…

Clinical Presentation, Management, and Long-Term Outcome of Pars Planitis, Panuveitis, and Vogt-Koyanagi-Harada Disease in Children and Adolescents.

OBJECTIVE: Chronic uveitis is a common manifestation of pediatric rheumatologic conditions and may result in irreversible blindness and long-term disability. While chronic anterior uveitis is the most commonly encountered ocular manifestation of rheumatic disease, little is known about the clinical presentation, management, and long-term outcome of more complex eye conditions such as pars planitis (PP), panuveitis (PU), and Vogt-Koyanagi-Harada disease (VKH). The present study was undertaken to comprehensively assess the long-term safety and efficacy of disease-modifying antirheumatic drugs (D…

[Uveitic glaucoma in Behçet's disease: When everything gets complicated].

INTRODUCTION: Behçet's disease is a systemic disease of unknown etiology. Its ocular manifestations are multiple and polymorphic, with uveitis at the top of the list. Classically, uveitis in Behçet's disease does not cause elevation of intraocular pressure. However, this may be found in 10 to 20% of cases, secondary to trabeculitis, trabecular obstruction by inflammatory cells, anterior or posterior synechiae, or induced by prolonged use of corticosteroids. Its occurrence is a major complication, the management of which is especially delicate when glaucoma occurs. The purpose of this study was…

The CD4/CD8 ratio in vitreous fluid is of high diagnostic value in sarcoidosis.

PURPOSE: Sarcoidosis is an idiopathic inflammatory disorder involving multiple organs, and ocular manifestation (represented by granulomatous uveitis) is one of the common features. A well-known immunologic feature in sarcoidosis is an increased CD4+ helper T-cell type 1 lymphocyte subset in bronchoalveolar lavage (BAL) fluid. The current study investigated the vitreous lymphocyte subsets of ocular sarcoidosis to elucidate the immunologic features of this disorder in the eye. DESIGN: Case-control study. PARTICIPANTS AND CONTROLS: Fifty-one eyes of 38 patients with ocular sarcoidosis, confirm…

Endocrine aspects of paediatric rheumatic diseases.

Compared to the now numerous studies on the endocrinology of rheumatic diseases in adults, only a small number of studies has been published on children with rheumatic diseases. Prolactin has been most extensively investigated, showing interesting parallels with the findings in adults with rheumatological diseases. Thus, analogous to adult RA most forms of JRA or JCA (with the exception of ANA-positive JRA with uveitis) appear to show, if anything, low to normal levels of prolactin. Since the prolactin levels in adult RA depend on the inflammatory activity, and the physiological prolactin secr…

Unilateral uveitic congenital cataract caused by Spiroplasma ixodetis.

Congenital cataract and uveitis caused by Spiroplasma ixodetis are rare, with only a small number of cases reported worldwide. We report 2 infants with unilateral uveitic congenital cataracts. Both underwent lensectomy and anterior vitrectomy, received intracameral antibiotics, and were postoperatively treated with a topical corticosteroid-antibiotic regimen. Routine microbiological tests were negative, but 16S rRNA PCR detected S. ixodetis in the vitreous and lens. Identification was confirmed by partial sequencing of the Mollicutes 16S-23S rRNA intergenic spacer region and a species-specific…

Headache as an extraocular manifestation of birdshot chorioretinopathy: a case series.

Birdshot chorioretinopathy (BSCR) is a type of chronic posterior uveitis with limited characterization of extraocular manifestations, including headache. We describe two HLA-A29-positive patients with BSCR who developed refractory unilateral periorbital headaches temporally associated with visual symptom onset. Headaches lateralized to the more affected eye and the patients exhibited migrainous and prominent cranial autonomic features resembling trigeminal autonomic cephalalgias (TACs) and variable responses to standard headache therapies. Together, this supports the International Classificati…
