The roles of pre-season immunity, age, viral shedding, and community exposures in shaping influenza household transmission dynamics.

Protective immunity against influenza transmission remains poorly understood due to the high prevalence of undetected asymptomatic infections. To address this, we analyze 1518 individuals and 623 infection episodes from a three-year, rural and urban, South African household cohort study that combines pre-season serum collection with twice-weekly virological testing regardless of symptoms. By developing a subtype/lineage-specific household transmission model and incorporating time-resolved viral shedding kinetics for all observed infections, we disentangle household and community exposure, pre-

Navigating viral load information flow and provider decision-making for adolescent clients at HIV clinics in Kenya.

BACKGROUND: Achieving viral suppression goals for adolescents with HIV requires multifaceted strategies, including efficient and effective health systems to routinely monitor HIV viral load (VL) and support adolescents when adherence falters. OBJECTIVE: This study aimed to identify where gaps in information flow occur in return of HIV VL results to clinics and adolescents and to explore how providers determine 'good adherence' prior to confirmation of virological failure. METHODS: Four focus group discussions and ten in-depth interviews were conducted with purposively selected healthcare wor

Diamonds and viruses: Reconstructing the epidemiological origins of HIV-1 in Angola's mining frontier.

In 1988, the remote mining town of Dundo, in Lunda-Norte, Angola, displayed an anomalously high HIV-1 prevalence of 14.2%, despite the absence of recognised epidemic drivers. Archival and epidemiological evidence suggests that colonial medical interventions, rather than sexual transmission alone, were central to the early amplification of the virus. Under the Companhia de Diamantes de Angola (Diamang), a vast corporate health system promoted extensive use of injections and blood transfusions, relying on reusable equipment and limited sterilization. Quantitative data on transfusion ratios and i

Retrospective Analysis of 50 Postnatal BVDV Outbreaks in Cattle from Central Argentina: Clinical, Pathological, and Epidemiological Insights.

Bovine viral diarrhea virus (BVDV) is an important pathogen in cattle and causes considerable economic losses worldwide. In Argentina, where there is no national control program, BVDV remains endemic. In this retrospective study, the epidemiological, clinical and pathological features of postnatal BVDV-associated diseases in 50 outbreaks in central Argentina (1995-2024) were analyzed. Data were obtained from field reports, necropsies, and virological results (virus isolation, RT-nPCR, immunochromatography). No seasonal pattern was found. Acute infections (AIs) and mucosal disease (MD) occurred

Cognitive impairment in persons with HIV: changing aetiologies and management strategies.

PURPOSE OF REVIEW: Cognitive disorders have been described in persons with HIV since the advent of virologically suppressive antiretroviral therapy. As persons with HIV age, their demographics and risk factors for cognitive health have changed. Here, we review these changing demographics and the recent literature regarding risk factors for cognitive decline and best management strategies. RECENT FINDINGS: Rates on noninfectious comorbidities are higher in persons with HIV compared to those without HIV and specifically include high rates of mental health conditions and cardiovascular complicat

Effects of Sofosbuvir/Velpatasvir Therapy on Extrahepatic Manifestations in Patients With Type 2 Diabetes and Chronic Hepatitis C: Insights From a Nationwide Hepatitis C Virus Registry in Taiwan.

This study examines the impact of hepatitis C virus (HCV) eradication through sofosbuvir/velpatasvir (SOF/VEL) treatment on glycated hemoglobin (HbA1c) levels in patients with chronic hepatitis C and type 2 diabetes mellitus (T2DM). Utilizing data from the Taiwan HCV Registry, a retrospective analysis was conducted on 2180 patients who met the inclusion criteria, 695 of whom had T2DM. HbA1c levels significantly decreased in the diabetes group from 7.32% ± 1.72% at baseline to 6.87% ± 1.34% after achieving sustained virological response (SVR12). Patients with higher baseline HbA1c levels and ci

Bacterial Co-Infection Delays Hepatitis B Virus Clearance in a Chronic Replication Mouse Model by Inducing T Cell Exhaustion.

Hepatitis B virus (HBV) and bacterial infections are major global health concerns. However, the impact of bacterial co-infection on HBV clearance remains unclear. To investigate how bacterial co-infection affects HBV clearance, we used a mouse model with chronic HBV replication and Klebsiella pneumoniae co-infection. We evaluated HBV virological markers, innate immune responses, inflammation, and intrahepatic HBV-specific T cell responses, with and without antibiotic treatment or MDSCs depletion. Our findings demonstrate that bacterial co-infection activates innate immunity and inflammation in

Hepatitis C virus and cardiovascular disease: Current knowledge and unmet needs.

The relationship between hepatitis C virus (HCV) infection and cardiovascular disease (CVD) is increasingly recognized, with studies indicating elevated CVD prevalence and mortality among individuals with HCV. Chronic HCV patients exhibit a higher CVD risk, especially in the population of end-stage renal disease on hemodialysis. Pathogenic mechanisms may include oxidative stress, endothelial damage, metabolic dysregulation, and chronic inflammation. These factors enhance vascular damage, promoting plaque formation and instability. Direct-acting antivirals (DAAs), which have revolutionized HCV

Australian National Enterovirus Reference Laboratory annual report, 2023.

Australia monitors its polio-free status by conducting surveillance for cases of acute flaccid paralysis (AFP) in children less than 15 years of age, as recommended by the World Health Organization (WHO). Cases of AFP in children are notified to the Australian Paediatric Surveillance Unit or the Paediatric Active Enhanced Disease Surveillance System and faecal specimens are referred for virological investigation to the National Enterovirus Reference Laboratory. In 2023, no cases of poliomyelitis were reported from clinical surveillance and Australia reported 1.71 non-polio AFP cases per 100,00

Syphilitic pneumonia: case report and systematic review.

Syphilitic pneumonia is a rare secondary form of Treponema pallidum infection. In this article, we present a case of syphilitic pneumonia in a patient living with Human Immunodeficiency Virus (HIV) with good immune-virological status. We also performed a systematic review of literature and we found 43 cases of syphilitic pneumonia described since 1886. We added our case report and performed a statistical analysis. Our analysis showed that 40/44 (91%) were males, with a median age of 46 years old (IQR 37-56), 9/28 (32%) were people living with HIV (data not available for 16/44 patients), 17/27

Post Mortem Findings of Cetaceans Stranded Along the Campania Coast from 2016 to 2022.

The presence of cetaceans along the Campania coast has always been documented. Between 2016 and 2022, out of 65 cetaceans stranded along the Campania coast, 46 were studied for bacteriological, virological, parasitological, and histopathological investigations. The results highlighted that for 59% (n = 27) of the specimens, the cause of death was of natural origin, while for only 2% (n = 1) of animals, the origin of death was ascribed to anthropic causes. Unfortunately, for 39% (n = 18) of the cetaceans, it was impossible to determine the cause of death. All the cetaceans that died of natural

Autoimmune hepatitis and primary sclerosing cholangitis after direct-acting antiviral treatment for hepatitis C virus: A case report.

BACKGROUND: Chronic hepatitis C virus (HCV) infection is a major global health concern that leads to liver fibrosis, cirrhosis, and cancer. Regimens containing direct-acting antivirals (DAAs) have become the mainstay of HCV treatment, achieving a high sustained virological response (SVR) with minimal adverse events. CASE SUMMARY: A 74-year-old woman with chronic HCV infection was treated with the DAAs ledipasvir, and sofosbuvir for 12 wk and achieved SVR. Twenty-four weeks after treatment completion, the liver enzyme and serum IgG levels increased, and antinuclear antibody became positive wit

Hepatitis C virus-induced differential transcriptional traits in host cells after persistent infection elimination by direct-acting antivirals in cell culture.

Chronic hepatitis C virus infection (HCV) causes liver inflammation and fibrosis, leading to the development of severe liver disease, such as cirrhosis or hepatocellular carcinoma (HCC). Approval of direct-acting antiviral drug combinations has revolutionized chronic HCV therapy, with virus eradication in >98% of the treated patients. The efficacy of these treatments is such that it is formally possible for cured patients to carry formerly infected cells that display irreversible transcriptional alterations directly caused by chronic HCV Infection. Combining differential transcriptomes from tw

Mimics of Guillain-Barré Syndrome in Pediatric Patients Admitted to the Neuro-Intensive Care Unit of a Tertiary Care Hospital-A Case Series.

Guillain-Barré syndrome is the most common cause of acute flaccid paralysis in children, but several diseases mimic GBS. We aimed to identify and report the clinical pointers and battery of tests required to differentiate Guillain-Barré syndrome from its observed mimics in the pediatric population admitted to our neuro-critical care unit. We conducted a retrospective record analysis of all pediatric patients admitted over ten years from 2008-2018, whose initial presentation was compatible with a clinical diagnosis of GBS. Eighty-three patients were at first treated as GBS, of which seven (8.4%

Influenza hospitalisations in Spain between the last influenza and COVID-19 pandemic (2009-2019).

Knowing the burden of severe disease caused by influenza is essential for disease risk communication, to understand the true impact of vaccination programmes and to guide public health and disease control measures. We estimated the number of influenza-attributable hospitalisations in Spain during the 2010-2011 to 2019-2020 seasons - based on the hospitalisations due to severe acute respiratory infection (SARI) in Spain using the hospital discharge database and virological influenza information from the Spanish Influenza Sentinel Surveillance System (SISSS). The weekly numbers of influenza-attr

Lenacapavir and the novel HIV-1 capsid inhibitors: an emerging therapy in the management of multidrug-resistant HIV-1 virus.

PURPOSE OF REVIEW: The aim of this study was to summarize current evidence regarding lenacapavir, a first in class HIV-1 capsid inhibitor, and its role as an emergent therapy for the treatment of HIV-1 infection. RECENT FINDINGS: HIV-1 capsid inhibitors (of which lenacapavir is the first in class) has been postulated to have activity against multidrug resistant HIV-1 viral isolates. Initial results from the phase 3 trial CAPELLA (combining oral and subcutaneous lenacapavir alongside failing drug therapies) suggest that there may be a role for these novel agents in a cohort of patients living

Molecular characterization and phylogenetic analysis of highly pathogenic H5N1 clade 2.3.4.4b virus in Bosnia and Herzegovina.

Influenza A virus continues to represent a growing problem affecting mainly birds but with an increasing number of mammal transmission events reported each year. Nevertheless, molecular characterization and phylogenetic analysis of influenza A viruses originating from all confirmed cases have not been systematically performed in all parts of the world. In this study, we investigated a sample originating from a mute swan that died in November 2021 in the northern part of Bosnia and Herzegovina with RT-qPCR and whole genome sequencing using ONT MinION. It was diagnosed as a highly pathogenic Inf

Lamivudine induced pure red cell aplasia and HIV-1 drug resistance-associated mutations: a case report.

Adverse effects linked to antiretroviral therapy (ART) may contribute to poor adherence on the patient's side. Consequently, human immunodeficiency virus (HIV) drug resistance mutations could emerge, negatively impacting the body's immune system. Meanwhile, severe immunosuppression can lead to several conditions, including anemia. The cause of anemia in HIV infection is multifactorial, and can be mainly explained by deleterious direct effects of the virus on the bone marrow, and opportunistic infections such as Parvovirus B19. Other causes include blood loss resulting from neoplasms and gastro
