Least developed countries (LDCs) are home to over a billion people throughout Africa, Asia-Pacific, and the Caribbean. The people who live in LDCs represent just 13% of the global population but 40% of its growth rate. Characterised by low incomes and low education levels, high proportions of the population practising subsistence living, inadequate infrastructure, and lack of economic diversity and resilience, LDCs face serious health, environmental, social, and economic challenges. Many communities in LDCs have very limited access to adequate sanitation, safe water, and clean cooking fuel. LD

One-seventh of the world's adult population, or approximately one billion people, are estimated to have OSA. Over the past four decades, obesity, the main risk factor for OSA, has risen in striking proportion worldwide. In the past 5 years, the WHO estimates global obesity to affect almost two billion adults. A second major risk factor for OSA is advanced age. As the prevalence of the ageing population and obesity increases, the vulnerability towards having OSA increases. In addition to these traditional OSA risk factors, studies of the global population reveal select contributing features and

The success of combination antiretroviral therapy (cART) has transformed HIV infection into a chronic condition, resulting in an increase in the number of older, cART-treated adults living with HIV. This has increased the incidence of age-related, non-AIDS comorbidities in this population. One of the most common comorbidities is depression, which is also associated with cognitive impairment and a number of neuropathologies. In older people living with HIV, treating these overlapping disorders is complex, often creating pill burden or adverse drug-drug interactions that can exacerbate these neu

We aimed to investigate sarcoidosis mortality in a large, population-based cohort, taking into account disease heterogeneity.Individuals with incident sarcoidosis (n=8207) were identified from the Swedish National Patient Register using International Classification of Disease codes (2003‒2013). In a subset, cases receiving treatment ±3 months from diagnosis were identified from the Prescribed Drug Register. Nonsarcoidosis comparators from the general population were matched to cases 10:1 on birth year, sex and county. Individuals were followed for all-cause death in the Cause of Death Register

Transgender people experience a disproportionate burden of human immunodeficiency virus (HIV) and incarceration. Discrimination, victimization, poverty, and poor mental health drive vulnerability to HIV and related infections, as well as risk of arrest, detention, and incarceration. In this paper, we systematically review published data on HIV, sexually transmitted infections, viral hepatitis, and tuberculosis among incarcerated transgender people; describe potential structural determinants of HIV risk and transmission; identify gaps in the literature; and make recommendations for research and

INTRODUCTION: As the number of patients sustaining hip fractures increases, interventions aimed at improving patient comfort and reducing complication burden acquire increased importance. Frailty, cognitive impairment, and difficulty in assessing pain control characterize this population. In order to inform future care, a review of pain assessment and the use of preoperative intravenous paracetamol (IVP) is presented.
MATERIALS AND METHODS: Systematic review of preoperative IVP administration in patients presenting with a hip fracture.
RESULTS: Intravenous paracetamol is effective in the ear

Transsexualism in the ICD-10 (International Classification of Diseases, Tenth Revision), Gender Dysphoria in adolescents and adults in the DSM-5 (Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition), is characterized by a marked incongruence between one's experienced gender and biological sex. The etiology is complex, but some hypotheses suggest that Gender Dysphoria (GD) arises from discrepant cerebral and biological sexual differentiation. Increasing evidence supports the idea of genetic vulnerability. Henningsson et al, (2004) found significant differences when they examine

OBJECTIVE: To quantify the population at risk of serious adverse reactions to replicating smallpox vaccine.
DESIGN AND SAMPLE: Conditions known or suspected to carry risk were identified via Centers for Disease Control and Prevention planning documents, other federal publications, and peer-reviewed literature. Conditions identified were categorized as historically recognized risks or more recently recognized immunocompromised states that may pose risk. Major historical risk factors were as follows: eczema/atopic dermatitis, pregnancy, HIV, and primary immunodeficiency. More recently identifie

Half of humanity is now living in cities, according to the United Nations Population Division. The urban population exceeded the rural for the first time in 2008, and by 2050 urbanisation will rise to 70 per cent with increased urban risk. 'Today, 100 cities are in control of 30 per cent of the world's economy.' The need for maintenance and upkeep of these cities makes safety measures for their citizens crucial. In this context, urban risk, city planning and the role of local governments in dealing with risk reduction have been recognised as key factors to build communities resilient to disast

OBJECTIVE: We estimated the prevalence of children with life-threatening conditions (LTC) cared for in the military health system (MHS) in response to a Congressional inquiry and to inform program planning.
METHODS: We developed a case definition of LTC, using the concept ''death trajectory''(1,2) to define our cases. We conducted an unduplicated count of children with LTC in the MHS database during FY 2001/FY 2002 using selected ICD-9 codes based on our case definition. We then surveyed the literature for reported prevalence of LTC among children with similar case definitions. The concept of

BACKGROUND AND OBJECTIVE: Hemodialysis (HD) is associated with restless legs syndrome (RLS) and periodic leg movements in sleep (PLMS), but the mechanisms underlying these relationships remain unclear. African-American HD patients have been reported previously to have a reduced likelihood of RLS. Alterations in iron metabolism, known to be a risk factor for idiopathic forms of RLS, could represent the basis for these racial differences.
PATIENTS AND METHODS: In secondary data analyses from a previously published study, we examined raw and log-transformed values for plasma ferritin and polysom

Frail and vulnerable elderly patients, recognized primarily by the presence of such disabilities as immobility, incontinence, and dementia, are at particularly high risk for the development of infectious diseases, which are the leading cause of hospitalization in this population. The infectious diseases most often observed in the debilitated elderly are pneumonias, urinary tract infections, skin infections, and gastroenteritis, with fever a common manifestation. Some of the factors identified as contributing to their increased susceptibility include diminishing physiologic functioning; comprom

The concept of ocular frailty remains inadequately defined with respect to its clinical manifestations, etiological pathways, and consequential impacts. We delineate the concept of ocular frailty in order to furnish a well-defined and operationalizable conceptual basis for advancing research in the aging population. A total of 15 articles were included for analysis and synthesis. The attributes of ocular frailty were hypofunction, loss of resilience, association with systemic frailty, potential reversibility or modifiability, and multidimensionality. Antecedents of ocular frailty were classifi

INTRODUCTION: Six years later, the impact of the COVID-19 pandemic on children in Latin America remains profound. Even though they were considered less susceptible to the disease, infants have emerged as one of the most affected populations, with the pandemic exposing deep inequities and magnifying vulnerabilities. This paper is presented as a perspective from SLIPE on COVID-19, providing expert insight into the current epidemiological situation in the region.
AREAS COVERED: This review analyzes the effects of the COVID-19 pandemic on children's health and wellbeing, education, and immunizati

Aspirin (acetylsalicylic acid, ASA) is the standard of care for thrombosis prevention in patients with single-ventricle physiology and ductal stents. Although weight-based dosing is widely used, there is no consensus on optimal methods to assess adequate platelet inhibition. While platelet inhibition testing was available at our institution, utilization was infrequent, and a standardized approach was lacking. Clinical events prompted interest in systematic application of testing in this population. We applied quality improvement methodology to implement VerifyNow Aspirin (VN) testing to evalua

To report on the relationship between neuroinflammation and chronic pain, especially in the pediatric population. Pediatric chronic pain is prevalent, affecting about 20% of the population. Neuroinflammation is now recognized as a putative contributing factor to the development and maintenance of chronic pain. Strides have been made in understanding neuroinflammatory processes in the context of pain, as research begins to unravel the involvement of glial cells (e.g., microglia, astrocytes), pro-inflammatory markers (e.g., interleukins, Tumor Necrosis Factor-alpha [TNF-α]), and metabolic pathwa
