Bifonazole. A review of its antimicrobial activity and therapeutic use in superficial mycoses.

Bifonazole is a substituted imidazole antifungal agent structurally related to other drugs in this group. It possesses a broad spectrum of activity in vitro against dermatophytes, moulds, yeasts, dimorphic fungi and some Gram-positive bacteria. Both non-comparative and comparative clinical trials have clearly demonstrated the efficacy and safety of various formulations of bifonazole 1% (cream, gel, solution and powder) applied once daily in the treatment of superficial fungal infections of the skin such as dermatophytoses, cutaneous candidiasis and pityriasis versicolor. In comparative studies…

The importance of opportunistic fungal pathogens in causing ocular infections is emphasized. A study was conducted over a period of 4 years … owed by Penicillium citrinum in 8 cases (19.04%) and Aspergillus fumigatus in 5 cases (11.90%). The yeasts were responsible for only 3 cases (7.14%) i.e. one each caused by Candida albicans, C. parapsilosis and C. guilliermondii. Among the remaining 12 cases, one was caused by F. moniliforme, 3 by A. flavus, 2 each by A. niger, Penicillium expansum and P …

Candida parapsilosis in a case of canine otitis media.

Candida parapsilosis, commonly present on the skin and mucous membranes, has been described as an opportunistic pathogen increasingly associated with infections. We present the first documented case of severe otitis media caused by this yeast species in a French Bulldog with a history of atopic dermatitis and seborrhea. The diagnosis was confirmed by culture and molecular methods. Susceptibility testing showed resistance only to fluconazole; however, antifungal therapy could not be initiated, as the patient died in an emergency room. This case highlights the potential of C. parapsilosis to cau

Disseminated histoplasmosis in an immunocompetent infant: an uncommon diagnostic consideration.

A previously asymptomatic male infant presented with persistent fever for 4 weeks with progressive abdominal distension due to hepatosplenomegaly. Investigations were suggestive of bicytopenia. His fever was non-responsive to intravenous antibiotics. Blood and urine cultures were sterile. Bone marrow showed normal haematopoiesis with evidence of yeast forms of Histoplasma capsulatum inside erythroblasts. Urine was positive for histoplasma antigen. The infant responded to intravenous liposomal amphotericin B. Fever resolved, anaemia and thrombocytopenia recovered. Hence, we report this case of

[Pityriasis versicolor].

Pityriasis versicolor is a skin condition caused by Malassezia yeast, leading to pink to reddish hypo- or hyperpigmented, slightly scaly patches usually located at the sebaceous skin areas, i.e. upper trunk and arms. Subjective symptoms are rare, but include itch. In Scandinavia, the prevalence ranges from 0.5% to 4%. Risk factors include genetics, and immune suppression. The diagnosis is usually clinical, but may be supplemented by microscopy showing a "spaghetti meatballs" sign. Treatment involves topical or systemic antifungals. The prognosis is generally good, but recurrence may occur, as

Primary fungal laryngitis mimicking recurrent laryngeal carcinoma.

A 79-year-old smoker with a background history of a treated glottic carcinoma and chronic obstructive pulmonary disease presented with progressive hoarseness, symptoms of aspiration and shortness of breath for 6 months. Examination revealed an ulcero-fungating mass over the posterior commissure of the larynx. A tracheostomy, direct laryngoscopy and biopsy of the mass was performed to secure his airway and to exclude recurrent glottic carcinoma. Reassuringly, a histopathological examination of the mass revealed numerous fungal yeast bodies. He was then treated with itraconazole for 4 weeks and

Candida auris: A Quick Review on Identification, Current Treatments, and Challenges.

Candida auris is a novel and major fungal pathogen that has triggered several outbreaks in the last decade. The few drugs available to treat fungal diseases, the fact that this yeast has a high rate of multidrug resistance and the occurrence of misleading identifications, and the ability of forming biofilms (naturally more resistant to drugs) has made treatments of C. auris infections highly difficult. This review intends to quickly illustrate the main issues in C. auris identification, available treatments and the associated mechanisms of resistance, and the novel and alternative treatment an

Labour-saving construction of a target protein interaction network by selective culture and high-throughput sequencing.

Yeast two-hybrid (Y2H) is a classic method of screening for protein-protein interactions. However, the operation process is labor intensive and time consuming, and there is a high possibility of false positives and false negatives. Combined with wet lab operation and bioinformatics analysis, we developed a novel method of Y2H library screening using Chrysanthemum morifolium CmMPK3 as an example. The protocol can not only greatly simplify the steps of traditional Y2H library screening but also identify as many interacting proteins as possible. Furthermore, this protocol is applicable to any spe

Postpartum histoplasmosis in an HIV-negative woman: a case report and phylogenetic characterization by internal transcribed spacer region analysis.

The present report describes the first case of postpartum disseminated histoplasmosis in a 24-year-old HIV-negative woman. On the tenth day after vaginal delivery, the patient presented with dyspnea, fever, hypotension, tachycardia, and painful hepatomegaly. Yeast-like Histoplasma capsulatum features were isolated in the buffy coat. The phylogenetic analysis demonstrated that the fungal isolate was similar to other H. capsulatum isolates identified in HIV patients from Ceará and Latin America. Thus, histoplasmosis development in individuals with transitory immunosuppression or during the perio

Candida haemulonii: An emerging opportunistic pathogen in the United States?

Candida species are ubiquitous fungal pathogens that exhibit increasing resistance to anti-fungal agents. Candida haemulonii, a rare subtype, is an emerging and virulent yeast pathogen. Species identification is difficult due to phenotypic similarity to other Candida subtypes, such that there is a high risk of inappropriate antimicrobial administration and worsening of emerging resistance patterns. Candida haemulonii has a proclivity for infection of chronic lower extremity wounds particularly in diabetic patients, as exemplified in our case. This case raises awareness about the necessity for

Verrucous Plaque With Unusually Large Candida Blastoconidia: A Unique Clinicopathological Presentation of Systemic Mucocutaneous Candidiasis.

Mucocutaneous candidiasis is a common infection affecting both immunocompetent and immunosuppressed individuals. Diversity in the clinical and histopathological presentation of mucocutaneous candidiasis is well known. However, the occurrence of cutaneous verrucous lesions and giant yeast-like structures has been rarely reported. In this article, we describe a case of disseminated mucocutaneous candidiasis in an immunosuppressed patient who presented as a verrucous plaque on the scrotum with giant Candida blastoconidia. This peculiar presentation expands the clinicopathological spectrum of muco

A pharmacokinetic case study of intravenous posaconazole in a critically ill patient with hypoalbuminaemia receiving continuous venovenous haemodiafiltration.

This article reports a pharmacokinetic case study of intravenous posaconazole in a 49-year-old male patient with intra-abdominal sepsis and hypoalbuminaemia receiving continuous venovenous haemodiafiltration. Concentration-time data following 300 mg intravenous posaconazole was analysed using a population pharmacokinetics approach. The 300-mg intravenous dose appears to be adequate for the treatment of yeast infections; however, for the treatment of invasive aspergillosis in isolated cases of critically ill patients with hypoalbuminaemia, the current dosing may not achieve desired exposure, al

Autophagy and Proteases: Basic Study of the Autophagic Flux by Western Blot.

Autophagy is a catabolic process triggered in the cell by a wide range of stress stimuli, both external (including nutrient deprivation) and internal (like the presence of protein aggregates or damaged organelles). First described in yeast, this pathway has recently gained major importance due to its role in several pathologies, from inflammatory processes to cancer or aging. However, its analysis can be easily misinterpreted if it is not done properly, leading to conflicting results. Here, the classical autophagy flux study by Western blot is described, as a first and basic analysis of the st

Recent synthetic and medicinal perspectives of tryptanthrin.

Tryptanthrin is a natural alkaloidal compound having basic indoloquinazoline moiety. It is obtained from various natural plant sources as well as different cell cultures including yeast etc. Trptanthrin is considered as biogenetic precursor for phaitanthrin A-C, pyrroloindoloquinazoline, (±)-cruciferane. Different synthetic approaches for the synthesis of tryptanthrin have been very well reported. It has broad spectrum of biological activities including anticancer activity, anti-inflammatory, antiprotozoal, antiallergic, antioxidant, and antimicrobial. In this review, our focus will be, on the

Case report of invasive, disseminated candidiasis with peripheral nodular cavitary lesions in the lung.

We report a case of invasive candidiasis presenting as multiple lung nodules and cavitary lesions with minimal pleural effusion. Candida infections of the lung are rare but can occur after hematologic dissemination of the yeast from other body sites, such as the skin and the gastrointestinal and genitourinary tracts. Here, we describe the case of a 56-year-old female with a history of end-stage renal disease (ESRD) who presented with fever, productive cough, and pulmonary nodules and cavitary lesions seen on a chest computed tomography (CT). The patient's blood cultures were positive for Candi

HEPATOPROTECTIVE ACTIVITY OF EXOGENOUS RNA.

Hepatoprotective activity of Nuclex, a pharmaceutical composed of low-molecular yeast RNA, was investigated during acute and chronic thioacetamide-induced hepatotoxicity. It is demonstrated, that Nuclex administration at a dose of 200 mg/kg during acute and chronic liver injury produces hepatoprotective effect, which is associated with decrease in liver parenchyma lesions and in its inflammatory infiltration. Nuclex application attenuates thioacetamide-induced free radical damage of hepatic biopolymers, expressed in the reduction of TBA-reactive products, carbonyl derivatives, and recovery of
