Peroneal tendon dislocation/subluxation - Case series and review of the literature.

BACKGROUND: Dislocating or subluxing peroneal tendons is a relatively infrequent injury. Although infrequent it is very debilitating for the athlete. This retrospective study addresses primarily the surgical technique. METHODS: Twenty-three patients between 2005 and 2014 were operated on for symptomatic dislocating or subluxing peroneal tendons. Five patients presented in the acute phase and 18 patients were late cases. Twenty patients were available for follow-up at a mean of 53.1 months. Three patients were classified as Stage III and 17 as Stage I/II. The procedures varied from pure repair

A preliminary report on intra-sheath peroneal tendon subluxation: a prospective review of 7 patients with ultrasound verification.

UNLABELLED: Peroneal tendon dislocation with recurrent subluxation over the lateral malleolus, both acute and chronic, is well documented in the literature. However, there remains a subset of patients who report symptoms similar to peroneal subluxation that do not actually display active or passive displacement of the tendon over the lateral malleolus. In this article, we describe 7 patients who were followed prospectively for the treatment of ultrasound-confirmed, retrofibular, intrasheath subluxation without the typical lateral subluxation or dislocation of the tendon over the malleolus. Six

Aggressive surgical treatment and early return to sports in athletes with grade III syndesmosis sprains.

BACKGROUND: Grade III syndesmosis sprains are usually treated with internal fixation. Limited information is available on early weightbearing and early return to activity after operative treatment for grade III syndesmosis sprains. HYPOTHESIS: Treatment of grade III syndesmosis sprains in intercollegiate athletes with internal fixation, early range of motion, and early weightbearing can lead to rapid return to sport with minimal complications and no ankle problems in midterm follow-up. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: We evaluated a consecutive series of intercolleg

Musculofascial flaps based on the dorsalis pedis vascular pedicle for coverage of the foot and ankle.

Soft-tissue reconstruction of the foot and ankle has long presented challenging problems for the plastic surgeon. Limitations of available local tissue, the need for specialized tissue, and donor site morbidity restrict the options available to the reconstructive surgeon. In an effort to solve these difficult problems, we have begun to use musculofascial flaps based on the branches of the dorsalis pedis artery. We present our early experience of 5 patients treated with an extensor digitorum brevis muscle flap with fascial extensions often containing the contents of the first web space. Our pat

Steinmann pin retractor: An auxiliary reduction equipment for the minimally invasive treatment of calcaneal fractures.

A minimally invasive approach has been widely used for the treatment of calcaneal fractures, however, its downside in exposing the calcaneal body, affects fracture reduction. We used a Steinmann pin retractor mentioned in our previous case series study to solve this issue. To further evaluate the efficacy of this reduction technique, and elucidate its mechanism, we compared it with the control groups in this study. Between March 2017 and November 2020, 52 patients were included in this retrospective comparative study. 20 patients were included in the S-S group (Steinmann pin retractor with int

Plantar Plating for the Treatment of Proximal Fifth Metatarsal Fractures in Elite Athletes.

Proximal fifth metatarsal fractures, zones II and III, are commonly treated surgically, especially in elite athletes. Intramedullary screw fixation remains the most used construct despite nonunion and refracture. High tensile forces on the plantar-lateral aspect of the fifth metatarsal are difficult to control, and intramedullary screw fixation depends on ideal screw position, length, and width. The authors present a plantar plating technique with cancellous bone autograft for zones II and III proximal fifth metatarsal fractures. Rotational instability and plantar-lateral gapping are resisted

Comparison of diclofenac sodium and aspirin in the treatment of acute sports injuries.

A randomized, double-blind, parallel-group clinical trial compared diclofenac sodium (Voltaren, Ciba-Geigy Summit, NJ) with aspirin for the treatment of acute sprains and/or strains of the knee or ankle. One hundred thirty-nine patients were admitted to the study. Patients received either 150 mg (75 mg twice daily) of diclofenac (N = 69) or 3.6 g (1.2 g three times daily) of aspirin (N = 70) for 3 to 10 days. Forty-seven diclofenac patients and 49 aspirin patients, mean age for both groups 25 years, were evaluated to determine the efficacy of each treatment. Both groups experienced significant

Kinematic chain-related risk factors in the development of lower extremity injuries in women: A prospective study.

The link between dynamic range of motion (dROM) parameters within the kinematic chain and the development of lower extremity (LE) injury has gained attention; however, very few longitudinal studies have been conducted. Therefore, we wanted to prospectively determine dROM-related risk factors for the development of LE injury. Within this study, 90 female physical education students aged 19.33±0.87 years were tested. Full-body kinematic and vertical ground reaction force (VGRF) parameters were measured during a triple hop jump (THJ). Cox regression analysis was used to identify potential risk fa

Stress Fractures of the Foot.

Stress fractures of the foot and ankle may be more common among athletes than previously reported. A low threshold for investigation is warranted and further imaging may be appropriate if initial radiographs remain inconclusive. Most of these fractures can be treated conservatively with a period of non-weight-bearing mobilization followed by gradual return to activity. Early surgery augmented by bone graft may allow athletes to return to sports earlier. Risk of delayed union, nonunion, and recurrent fracture is high. Many of the patients may also have risk factors for injury that should be mod

Dislocation of the fibular head in an unusual sports injury: a case report.

INTRODUCTION: One of the primary functions of the proximal tibiofibular joint is slight rotation to accommodate rotational stress at the ankle. Proximal tibiofibular joint dislocation is a rare injury and accounts for less than 1% of all knee injuries. This dislocation has been reported in patients who had been engaged in football, ballet dancing, equestrian jumping, parachuting and snowboarding. CASE PRESENTATION: A 20-year-old man was injured whilst playing football. He felt a pop in the right knee and was subsequently unable to bear weight on it. The range of movement in his knee joint was

Knee dislocations with vascular injuries.

Any knee that is dislocated or that has both anterior and posterior cruciates and one collateral ligament torn should be carefully evaluated for neurologic and vascular injury. Reduction and immobilization should be performed. If the pulses are anything but normal, arrangements should be made to proceed to the operating room immediately. A one-shot arteriogram can be obtained in the operating room, but proceeding directly with popliteal artery exploration is reasonable if the pulses have not returned to normal. If the pulses have returned to normal, an arteriogram should be obtained. After the

Surgical treatment of displaced ankle fractures.

To test the validity of the practice of open reduction and internal fixation for the treatment of displaced ankle fractures, 25 cases were reviewed after a mean follow-up period of 1.5 years. Eighty-eight per cent had satisfactory objective results and 76% satisfactory subjective results. Eight-eight per cent of the patients with good results had anatomic reductions, whereas none of those with poor results was anatomically reduced. No patient with anatomic reduction had early evidence of degenerative arthritis. There were no nonunions of the medial malleolus, and open fractures did not predisp

Injury patterns and safety practices of rock climbers.

BACKGROUND: Rock climbing is gaining popularity. The injury patterns and safety practices of climbers have not been well described. This study seeks to identify the general injury patterns and safety practices associated with rock climbing. METHODS: An anonymous multiple choice, Likert scale, and short-answer Internet survey was posted on several rock climbing Websites. Data were collected autonomously for a 2-month period in 2004. Demographic data were obtained and subjects self-identified their three most significant injuries. Participants were also surveyed regarding safety practices and t

Radiographic evaluation of plantar plate injury: an in vitro biomechanical study.

BACKGROUND: The term turf toe has been generally applied to athletic dorsiflexion injuries of the hallux metatarsophalangeal (MTP) joint. Variations in injury patterns coupled with the complexity of the MTP joint makes the diagnosis of more severe injuries challenging and may lead to a failure in recognition of an unstable joint and a subsequently prolonged recovery. Although the proximal migration of one or both sesamoids has been previously identified with a plantar plate disruption, the current study proposes a quantitative grading system based on dorsiflexion stress radiographs, providing
