Ionizing Radiation Exposure in NICU.

The authors prospectively studied ionizing radiation exposure in consecutive 107 very low birth weight (VLBW) neonates, admitted to their Level III neonatal intensive care unit (NICU). Number of X-rays, their indications and calculated dose of radiation were documented. Their mean birth weight (+SD) and gestational age (+SD) were 1077 (±219.8) g and 29.7 (+2.57) wk respectively. Extremely low birth weight (ELBW) neonates underwent significantly higher radiographs when compared with VLBW neonates; 7.5(5-13.25) vs. 2(1-6); p < 0.0001. ELBW neonates received 3 times higher dose of radiation, when…

Effects of Extreme Prematurity on Numerical Skills and Executive Function in Kindergarten Children: An Application of Partially Ordered Classification Modeling.

Although mathematics disabilities (MD) are common in extremely preterm/extremely low birth weight (EPT/ELBW) children, little is known about the nature of these problems. In this study partially ordered set (POSET) models were applied to classify 140 EPT/ELBW kindergarten children (gestational age <28 weeks and/or birth weight <1000 g) and 110 normal birth weight (NBW) controls into profiles of numerical and cognitive skills. Models based on five numerical skills and five executive function and processing speed skills provided a good fit to performance data. The EPT/ELBW group had poorer skill…

[Intrauterine growth restriction--diagnosis and treatment].

The prevalence of low birth weight affects approximately 3-10% of live-born newborns in developed countries and in developing countries it affects 15-20% of newborns. The most common cause of low birth weight is considered to intrauterine fetal growth resctriction. Low birth weight is responsible for 69.6% of stillbirths and for 66.4% of neonatal deaths. The purpose of this paper is to review reliable scientific data in order to summarize the current guidelines on intrauterine fetal growth restriction, addressed to obstetricians. The present review is based on guidelines of the American Colleg…

Civil unrest linked to intrauterine growth restriction in western Kenya.

Risk factors associated with intrauterine growth restriction (IUGR) have previously been identified, but few studies have described the relationship between IUGR and maternal stress caused by exposure to civil unrest. Here, we investigate this relationship during the Mount Elgon crisis in western Kenya between 2006 and 2008, following a period of violence. Birth weight data were compared between three hospitals in an exposed area, Mount Elgon (n=570), and one hospital in a control area, Kimilili (n=530). In a sub-analysis, the most stress exposed hospital, Bungoma West (n=211), was compared wi…

Neonatal intensive care unit candidemia: epidemiology, risk factors, outcome, and critical review of published case series.

Evaluation of epidemiological trends, risk factors, and clinical outcome associated with candidemia at a neonatal intensive care unit is reported. From January 2005 to December 2009, forty candidemia cases were identified. C. albicans and C. parapsilosis were the most common species recovered (69 and 24%, respectively). All C. parapsilosis strains were susceptible to antifungals, whereas, C. albicans exhibited higher resistance rates to azoles. Low birth weight, low gestational age, presence of central lines, endotracheal intubation, total parenteral nutrition, previous use of antibiotics, ste…

The prediction of intra-partum fetal compromise in prolonged pregnancy.

This is a prospective study conducted in a dedicated post-dates clinic to investigate the importance of antenatal ultrasound, Doppler and cardiotocographic (CTG) indices in the prediction of adverse intra-partum events in prolonged pregnancy. Operative delivery for abnormal fetal ECG-ST segment analysis and/or an arterial cord pH < 7.15 were regarded as adverse events. There were 462 singleton pregnancies with 87 adverse intra-partum events included in the analysis. Intra-partum adverse events were associated with nulliparity, oligohydramnios and induction of labour. The birth weight of fetuse…

[Video-polygraphic-EEG study in the full-term newborn with low birthweight for their gestational age].

Video-polygraphic-EEG studies were performed in the first 24 life-hours of 26 healthy full-term newborns without perinatal injuries. The neurological examination and cranial ultrasonography were normal. The newborns were divided into two groups: one, with full-term appropriate--birth weight 11 newborns (control group) and the other with full-term low-birth weight 15 newborns. Thirteen newborns of the second group had video-polygraphic-EEG study abnormalities. The most frequent abnormalities were found in 11 cases, as far as sleep architecture is concerned. Also, when compared with the control …

Risk factors associated with retinopathy of prematurity: a study from Oman.

In a prospective study at Sultan Qaboos University Hospital, out of 73 premature infants screened for retinopathy of prematurity (ROP), 25 (34 per cent) developed the disease. Nine significant risk factors were found to be associated with the development of ROP. These factors were lower birth weight, shorter gestational age, apnoea, top-up blood transfusion, mechanical ventilation, receiving sodium bicarbonate for correction of metabolic acidosis, total parenteral nutrition, intraventricular haemorrhage, and sepsis. However, with stepwise logistic regression analysis, only birth weight, gestat…

A randomized placebo-controlled trial of erythromycin for the treatment of Ureaplasma urealyticum to prevent premature delivery. The Vaginal Infections and Prematurity Study Group.

Ureaplasma urealyticum has been associated with low birth weight and histologic chorioamnionitis and it is a frequent isolate from the chorioamnion of patients who are delivered prematurely. In prior clinical trials using antibiotics active against U. urealyticum, antibiotic treatment was associated with reduced prematurity and increased mean birth weight. In this multicenter, randomized, double-blind clinical trial, pregnant women with U. urealyticum were treated with 333 mg of erythromycin base or placebo three times daily, starting between 26 and 30 weeks' gestation and continuing through 3…

Ligation of patent ductus arteriosus in very low birthweight premature neonates.

The results of ligation of a patent ductus arteriosus in 30 premature neonates with a very low birth weight were analysed. The mean gestational age at birth was 27 weeks and the mean weight 811 g. Attempted closure of the ductus with indomethacin in 19 infants failed, though there was temporary closure in nine. Congestive heart failure was present in 29 and respiratory distress in 22 patients. Endotracheal intubation with assisted ventilation was necessary in all cases. The mean age at operation was 13.5 days and ductal ligation was performed in the intensive care unit under local anaesthesia …

[Development and fate of premature infants--then and now].

New scientific results and progress in technology allow even premature infants with very low birth weight to survive today. A vehement discussion arouse about effectiveness and efficiency of intensive care programs for these infants. However, an appreciation of the results at present should not be made without taking note of those achieved in former decades. A. Ylppö was the first pediatrician in Germany at the beginning of the 20th century, who was working systematically for the survival of low birth weight infants. He achieved remarkable results even at that time. After World War II the chan…

Improving the accuracy of fetal growth restriction diagnosis by integrating fetal genetic growth potential: a proof of concept study.

BACKGROUND: Fetal growth restriction (FGR) is a common obstetric complication where a fetus fails to reach its genetically determined growth … (1,806 mother-infant pairs), fetal growth potential was calculated using a polygenic risk score for birth weight derived from fetal genome-wide association studies. A new metric, “FGR_degree,” was developed as the difference between standardized genetic growth potential and actual birth weight Z-score. Fetuses were stratified into risk categories using a dual- …

Early development and reproductive lifespan of rabbit females: implications of growth rate, rearing diet and body condition at first mating.

Factors influencing early development such as birth weight, nest competition, and the diet received during rearing have been proposed as elements conditioning the future reproductive performance of European rabbit (Oryctolagus cuniculus) females. To evaluate their effects, we followed the life of 1513 females from birth to time of death, culling or censoring (animals alive at a fixed date). Between 0 and 63 days of age 353 females died. From the remaining 1160 females, 864 were chosen based on their birth weight to be transferred from the selection to the production farm. At this farm, 431 fem…
