Intertrochanteric fractures: a comparison between fixation with a two-piece nail plate and Ender's nails.

The progress of 182 patients who presented with intertrochanteric fractures was followed over a six month period. Eighty-seven patients were treated using Thornton/McLaughlin nail plates and 95 were treated using Ender's intramedullary nails. Each fracture was classified according to radiological position and mechanical stability. The results show that the more unstable fractures are more likely to develop unsatisfactory results. Post operatively, the different mechanical complications have been recorded at various stages. Those fractures fixed with a nail plate tended to develop varus deformi

[Results of rigid osteosynthesis treatment of diaphyseal fractures of the tibia].

The data for 301 patients (90% of the patients operated on), has been worked out. Those who had received rigid osteosynthesis treatment, were patients who had sustained fractures too critical to be treated with more conservative means. 64.8% of these fractures were of the complicative multifragment type. Of the indirect type, only the open fractures, and those that had become redislocated, were surgically repaired. It is believed that R. O. of open fractures is a routine part of primary surgical treatment. The patients' age ranged from 20-90 years; most of whom (55%) were involved in traffic a

Growth factors in the initial stage of bone formation, analysis of their expression in chondrocytes from epiphyseal cartilage of rat costochondral junction.

INTRODUCTION: In endochondral ossification septoclasts and osteoclasts (also called chondroclasts) release growth factors deposited in non-calcified and calcified zones of the growth plate. They stimulate, within the metaphysis, initial stages of the bone formation. We have recently reported quantitation of several growth factors in calcified cartilage from calf costochondral junction. Data from the analogous human cartilage could possibly help to choose efficient combinations of growth factors for clinical applications, but the amount of the calcified cartilage needed for analysis of numerous

Polydeoxyribonucleotide Promotes Longitudinal Bone Growth Through the Hepatic JAK2-STAT5/IGF-1 Axis Involving A2A Receptor Signaling.

Longitudinal bone growth is primarily regulated by the growth hormone (GH)-insulin-like growth factor-1 (IGF-1) axis. However, non-hormonal and orally administered strategies for promoting longitudinal growth remain limited. Here, we investigated the effects and mechanisms of polydeoxyribonucleotide (PDRN) on longitudinal bone growth in vitro and in vivo. In HepG2 cells, PDRN significantly increased IGF-1 mRNA expression and JAK2 and STAT5 phosphorylation without affecting cell viability. Pretreatment with ZM241385, a selective adenosine A2A receptor (A2AR) antagonist, significantly attenuated

Mandibular bone regeneration after marginal mandibulectomy for advanced MRONJ following denosumab discontinuation: a case report.

BACKGROUND: Medication-related osteonecrosis of the jaw (MRONJ) is a serious complication associated with bone-modifying agents, including denosumab. The clinical course after denosumab discontinuation remains insufficiently understood, and optimal surgical strategies for advanced cases are still under debate. CASE PRESENTATION: We report a 71-year-old man with castration-resistant prostate cancer and multiple bone metastases who developed MRONJ during denosumab therapy for more than 3 years. After diagnosis at our institution, denosumab was discontinued based on the decision of the treating

Comparative analysis of orthopaedic scaffold fixation methods - a finite element study.

Purpose: Critical-size bone defects (CSBD) are a serious challenge in current orthopaedics. Natural healing processes are insufficient, leading to complications such as muscle atrophy, joint stiffness, impaired limb function and reduced quality of life. Currently used autologous or allogeneic bone grafts have disadvantages, such as long surgery time, secondary injuries and immunological reactions. A promising alternative in the treatment of CSBD is the use of a scaffold. The key element of the effective use of scaffolds is their proper anchoring in the bone defect. The article presents numeric

Clinical Outcome of Combined Types 1, 2, and 3 Lateral Hinge Fractures After Open-Wedge High Tibial Osteotomy: A Case Report.

Lateral hinge fractures (LHFs) after open-wedge high tibial osteotomy (OWHTO) pose the risk of delayed bone union and loss of correction, and their management remains a challenge. The following case report describes a rare case of combined type 1, 2, and 3 LHFs following OWHTO. The patient was a 48-year-old female with a history of desmoid tumor resection and radiotherapy on the ipsilateral posterior aspect of the thigh, which was distal to the popliteal area of the knee. Bone atrophy was observed in the proximal tibia. OWHTO was performed for medial knee osteoarthritis (Kellgren-Lawrence grad

Changes in the craniofacial morphology in sclerostin knockout mice.

OBJECTIVES: To investigate the influence of lack of sclerostin on craniofacial morphology in sclerostin gene (Sost) knockout (SostΔ26/Δ26) mice. DESIGN: Micro computed tomography (micro-CT) was performed on both 60-day-old wild-type and SostΔ26/Δ26 mice to analyze changes in craniofacial morphology. Ten anatomical landmarks and eight measurement parameters as well as the total mandibular volume were used to assess craniofacial features. In addition, the femora of both wild-type and SostΔ26/Δ26 mice were examined using micro-CT and histological analysis. Cancellous bone volume, cortical bone v

Terminology of the growing bone: A historical study.

Division of the growing long bone into individual basic parts, that is, diaphysis, metaphysis, physes and epiphyses, has become generally accepted and used. However, the origin of these terms is almost unknown. Therefore, we have analyzed the literature in order to identify their sources. The terms epiphysis and apophysis have been used since the time of Hippokrates, although with different meanings. During the time of Galen, the term apophysis was used to describe all types of bone processes, and epiphyses denoted articular ends. The term diaphysis denoting the middle cylindrical part of the

Tri-Lineage Differentiation Potential of Osteosarcoma Cell Lines and Human Bone Marrow Stromal Cells from Different Anatomical Locations.

The bone cancer osteosarcoma, found mainly in adolescents, routinely forms around the growth plate/metaphysis of long bones. Bone marrow composition changes with age, shifting from a more hematopoietic to an adipocyte-rich tissue. This conversion occurs in the metaphysis during adolescence, implicating a link between bone marrow conversion and osteosarcoma initiation. To assess this, the tri-lineage differentiation potential of human bone marrow stromal cells (HBMSCs) isolated from the femoral diaphysis/metaphysis (FD) and epiphysis (FE) was characterized and compared to two osteosarcoma cell

Fibula pro tibia and cancellous allograft vitalised with autologous bone for non-union of the distal tibia diaphysis: Surgical technique.

BACKGROUND: The management of medium-sized osseous defects and pseudoarthrosis of the tibia is challenging. This surgical technique aims to bridge medium-sized tibial defects using the fibula as a lead structure. The fibula and cancellous bone graft fuse with the tibia leading to synostosis. METHODS: Cortico-cancellous bone is harvested from the anterior iliac crest. The bone is shredded into small pieces of about 5 mm and mixed with stem cell-rich blood from the situs. Additionally, cancellous allografts can be used for expansion. The approach is extended along the interosseous membrane to r

Fibrillin-1 deficiency in the outer perichondrium causes longitudinal bone overgrowth in mice with Marfan syndrome.

A disproportionate tall stature is the most evident manifestation in Marfan syndrome (MFS), a multisystem condition caused by mutations in the extracellular protein and TGFβ modulator, fibrillin-1. Unlike cardiovascular manifestations, there has been little effort devoted to unravel the molecular mechanism responsible for long bone overgrowth in MFS. By combining the Cre-LoxP recombination system with metatarsal bone cultures, here we identify the outer layer of the perichondrium as the tissue responsible for long bone overgrowth in MFS mice. Analyses of differentially expressed genes in the f

Silk-Based Bioengineered Diaphyseal Cortical Bone Unit Enclosing an Implantable Bone Marrow toward Atrophic Nonunion Grafting.

Postnatal fracture healing of atrophic long bone diaphyseal nonunions remains a challenge for orthopedic surgeons. Paucity of autologous spongiosa has potentiated the use of tissue engineered bone grafts to improve success rates of bone marrow engraftment used in plate reosteosynthesis. Herein, the development and in vitro validation of a "sandwich-type" biofabricated diaphyseal cross-sectional unit, with an outer mechanically robust bioprinted cortical bone shell, encompassing an engineered bone marrow, are reported. Channelized silk fibroin blend sponges derived from Bombyx mori and Antherae

Novel Radiographic Assessment of the Cribriform Plate.

Background The cribriform plate (CP) is a common site of spontaneous cerebrospinal fluid (SCSF) leaks. Radiographic assessment of the anterior and lateral skull base has shown thinner bone in patients with SCSFs; however, prior assessment of the CP has required postmortem cadaver dissection. Objective To develop novel radiographic techniques to assess the anatomy of the CP. Methods Computed tomography (CT) scans were performed on cadaveric specimens. Bone density and anatomy of a predefined volume of interest of the posterior CP were assessed by two independent reviewers. CT assessment of olfa

Arthrodesis of the shoulder.

Shoulder arthrodesis is an end-stage salvage option for the failing, painful joint that cannot undergo or has failed reconstruction. It is indicated for irreversible and nonreconstructible massive rotator cuff tears and deltoid muscle denervation as well as for detachment of the deltoid from its origin. Rarely, arthrodesis is done to stabilize the glenohumeral joint after many failed attempts at shoulder reconstruction. Arthrodesis for failed prosthetic arthroplasty or tumor resection presents additional challenges because of the associated bone loss on the humeral and/or glenoid side of the j

Bisphosphonate action. Alendronate localization in rat bone and effects on osteoclast ultrastructure.

Studies of the mode of action of the bisphosphonate alendronate showed that 1 d after the injection of 0.4 mg/kg [3H]alendronate to newborn rats, 72% of the osteoclastic surface, 2% of the bone forming, and 13% of all other surfaces were densely labeled. Silver grains were seen above the osteoclasts and no other cells. 6 d later the label was 600-1,000 microns away from the epiphyseal plate and buried inside the bone, indicating normal growth and matrix deposition on top of alendronate-containing bone. Biomechanical Comparison of Three Fixation Constructs for Tile Type C1.2 Pelvic Ring Fractures: A Finite Element Analysis.

Fractures of the pelvic ring are among the most severe injuries in orthopaedic practice and Tile type C lesions are characterized by complete disruption of the posterior arch with both vertical and rotational instability. The optimal construct for posterior ring fixation remains a matter of debate. The aim of this study was to compare, by means of finite element analysis, the biomechanical performance of three different methods of osteosynthesis for Tile type C1.2 pelvic ring fractures: a transiliac plate, one iliosacral screw and two anterior reconstruction plates on the sacroiliac joint.
