Reinforcement of thoracolumbar burst fractures with calcium phosphate cement. A biomechanical study.

STUDY DESIGN: A biomechanical study on the stabilization of thoracolumbar burst fractures. OBJECTIVE: To demonstrate that the addition of a calcium phosphate cement into the fractured vertebral body through a transpedicular approach is a feasible technique that improves the stiffness of a transpedicular screw construct. SUMMARY OF BACKGROUND DATA: Short segment pedicle screw instrumentation is a commonly used method for reduction and stabilization of unstable burst fractures. Recent investigators, however, have reported a high rate of instrumentation failure and sagittal collapse when there

Endoscopic repair of a complex midfacial fracture.

Inadequate treatment of complex midfacial fractures involving the orbit and zygomatic arch can result in important functional and aesthetic deformity. Accurate repair of fractures at the zygomatic arch and orbital floor has traditionally necessitated coronal scalp and lower eyelid incisions respectively. Complications such as facial nerve injury, alopecia, external scarring, ectropion, and eyelid edema have been associated with these traditionally open surgical techniques. We report an endoscopic-assisted technique of open reduction and rigid fixation of a complex midfacial fracture. The endos

[Fracture of the lower thoracic spine in ankylosing spondylitis].

Authors report on experiences gained during the treatment of the injury of the lower thoracal spine in a 64 year old male patient and review the relating literature. As a consequence of the basic disease fractures may occur following rather trivial traumas. As all ligaments are calcified a particular unstable state will develop following the fracture.

[The gamma-nail as a resilient alternative to the dynamic hip screw in unstable proximal femoral fractures in the elderly].

In a prospective randomised trial between September 1989 and June 1990 one hundred patients with per- and subtrochanteric fractures were consecutively treated by gamma-nail or DHS. The average age of both groups was about 80 years. The operation time for gamma-nailing was longer than for DHS implantation and also the postoperative blood loss was higher in the gamma-nail group. We found no difference of intraoperative blood loss, of perioperative lethality and in duration of hospital care. 90% of gamma-nail patients and 80% of DHS patients were successfully able to walk four days after operatio

Extra-articular subtalar arthrodesis with cancellous bone graft and internal fixation for children with myelomeningocele.

Extra-articular subtalar arthrodesis with cancellous bone graft and internal fixation was performed on 20 feet of 12 children with myelomeningocele, whose average age at operation was 7.4 years. Indication for surgery was progressive hindfoot valgus deformity causing difficulty in fitting orthoses. At average follow-up of 4.1 years, results were satisfactory in 18 of the 20 feet. Unsatisfactory results were a result of undercorrection at time of surgery. Extra-articular subtalar arthrodesis will correct hindfoot valgus deformity in children with myelomeningocele, and the use of internal fixati

Treatment of femoral neck fractures with a cancellous screw and fibular graft.

Twenty-six patients with femoral neck fractures were treated by open reduction, cancellous screw fixation and free fibular grafting. The patients were between 14 and 50 years of age. There were 16 old and 10 fresh fractures. Four patients had radiological signs of avascular necrosis before the treatment was instituted. Bony union was achieved in all patients except one, where the failure occurred because of a technical error. The patients were followed up for at least two years. No new case of avascular necrosis was detected after treatment, and there was clinical and radiological improvement

[Late septic complications of internal fixations (author's transl)].

The review of the authors' 5 year material (from 1971 to 1975) showed 5 late latent septic complications. The average time in which septic signs became evident, was 7.7 months in their cases. Attention is drawn to certain inadequacies during operations, which may have a role in the aetiology of these infections, and to the importance of the correct assessment of the follow-up X-rays.

[Treatment of fractures of paralyzed and partially paralyzed lower extremities (author's transl)].

The negative effects of fractures of paralyzed and partially paralyzed lower extremities are described. A good result to a certain extent restoring the condition from before the accident can only obtained by early osteosynthesis. Only the AO-method and intramedullary nailing should be applied for osteosynthesis.

Technique of anatomical reduction of lumbar spondylolisthesis and its surgical stabilization.

This is a report of a method for treatment of spondylolisthesis based upon: (1) preoperative reduction of the olisthesis by plaster casts; (2) fixation of the reduction by posterior arthrodesis, accompanied by a system of instrumentation which has been developed by the authors. The treatment of severe spondyloptosis in six children and of spondylolisthesis in 14 adults is considered separately, because of the somewhat different problems in the two different age groups.

The Bone Core Technique for the Augmentation of Limited Bony Defects: Five-Year Prospective Study with a New Minimally Invasive Technique.

The aim of this study was to evaluate a new minimally invasive surgical technique for the reconstruction of critical-size bony defect with local harvested bone core with simultaneous implant placement. In a prospective study, 186 consecutively treated patients were included and controlled clinically and radiologically for at least 5 years postoperative. Every patient presented a bony defect affecting the buccal, lingual, or palatal wall. In all cases, the alveolar crest was wide enough to allow implant placement inside the bony contours. During implant bed preparation, a trephine bur (3.5 mm e

Biomechanical Comparison of Ulnar Collateral Ligament Reconstruction With a Modified Docking Technique With and Without Suture Augmentation.

BACKGROUND: Throwing athletes with ulnar collateral ligament (UCL) injury and symptomatic valgus instability can expect to return to the same or higher level of play. Reconstruction with tendon graft is the dominant method of surgical treatment. Recent evidence suggests that spanning the joint with a suture anchored on both sides is biomechanically equivalent to reconstruction, with faster time to return to play. The authors developed a hybrid UCL reconstruction technique augmented with a suture brace to improve joint stability. PURPOSE/HYPOTHESIS: The purpose of this study was to biomechanic

[Limits of palmar locking-plate osteosynthesis of unstable distal radius fractures].

PURPOSE: The use of locking plate systems in the treatment of distal radius fractures has increased during the last years. In the presented study our experience and results after palmar plating of dorsal dislocated distal radius fractures are analysed and presented. Besides the functional and radiological results, the potential surgical risks and the observed complications are discussed. PATIENTS AND METHODS: Over a mean 15-month period, 112 consecutive patients (24 men, 88 women) with an average age of 57 years who were treated for an unstable dorsal dislocated distal radius fracture using t

Cochlear implant positioning and fixation using 3D-printed patient specific surgical guides; a cadaveric study.

HYPOTHESIS: To develop and validate the optimal design and evaluate accuracy of individualized 3D- printed surgical guides for cochlear implantation. BACKGROUND: Positioning and fixation of the cochlear implant (CI) are commonly performed free hand. Applications of 3-dimensional (3D) technology now allow us to make patient specific, bone supported surgical guides, to aid CI surgeons with precise placement and drilling out the bony well which accommodates the receiver/stimulator device of the CI. METHODS: Cone beam CT (CBCT) scans were acquired from temporal bones in 9 cadaveric heads (18 ear

The role of cross-link augmentation on fusion rate and patient satisfaction among patients with traumatic thoracolumbar spinal fracture: A randomized clinical trial.

INTRODUCTION AND OBJECTIVES: Most of the studies evaluating the effect of cross links on spinal stability are performed in vitro on porcine or human spine segments and there is limited data regarding clinical benefits of cross link augmentation in traumatic injuries. In this study we aimed to evaluate the effects of cross-links insertion between rods on the fusion rates and post-surgical patients' satisfaction among patients with traumatic thoracolumbar fractures who underwent posterior spinal fixation with pedicle screws. MATERIALS AND METHODS: This study was conducted as a randomized clinic

Neglected Neck of Femur Fracture Managed by Surgical Hip Dislocation: A Case Report and Literature Review.

INTRODUCTION: Talus fracture is an uncommon fracture that can be encountered on day- to- day basis. However, it is the 2nd most common tarsal bone to get fractured after calcaneum and accounts for approximately 1% of all fractures around foot and ankle. The anastomotic ring around the talar neck is highly likely to get damaged at the time of the fracture, which, in turn, hampers the blood supply to the body of talus. As a result, the bone healing is delayed and the integrity of the healed fracture is poor which leads to poor functional outcome. Almost 39% cases are missed during the initial ev
