Esophageal achalasia is an infrequent motility disorder characterized by a progressive stasis and dilation of the oesophagus; with subsequent risk of aspiration, weight loss, and malnutrition. Although the treatment of achalasia has been traditionally based on a surgical approach, especially with the introduction of laparoscopic techniques, there is still some space for a medical approach. The present article reviews the non-surgical therapeutic options for achalasia.

Recombinant and native large conductance, Ca(2+)-activated K+ (BK) channels often demonstrate O2 sensitivity in cell-free membrane patches suggesting that a significant component of the O2-sensing machinery must be closely associated with the channel protein complex. Until recently, however, the identity of the O2 sensor itself had remained elusive. Employing functional proteomics we have defined the molecular nature of such an O2 sensor of BK channels. Using immunoprecipitation, 1D and 2D gel electrophoresis, and mass spectroscopy we identified the constitutive form of haem oxygenase, haem ox

Conditions for the fast separation of the enantiomers of a dihydropyridine substituted acid on a 50 x 4.6 mm ID short Chiralpak AD column with 2-propanol modified carbon dioxide as the mobile phase are presented. A high throughout of samples can be accomplished through the continuous sample loading of the loop of the injector. If a continuous data collection was used 10 separations could be performed in about 5 min with a precision of 0.6% RSD for the area ratio (n = 10). The parent drug clevidipine can also be analyzed and its enantiomeric composition determined after alkaline hydrolysis into

Primary pulmonary hypertension (PPH) is a rare disease. The annual incidence in the West is 1-2 cases per million population per year. A recent WHO symposium in 1998 has produced a consensus on classification, methods of screening, risk assessment and treatment. PPH is a diagnosis of exclusion after all other secondary causes of pulmonary hypertension are ruled out. Current treatment strategy involves acute vasodilator drug trial where positive responders are treated with high dose calcium channel blockers and anticoagulation. Those who do not show positive response may be commenced on intrave

Aging in mammals has often been associated with decreased insulin secretion and a subsequent deterioration in the ability to maintain glucose homeostasis. However, recent studies have demonstrated that factors such as disease, obesity, and physical activity more closely reflect diminished insulin secretion rather than aging per se. Thus, the purpose of this article is to review recent studies of how biological aging, i.e. the process independent of disease states such as type II diabetes, may affect insulin secretion. To this end, this review will address the impact of aging on insulin secreti

Angiotensin-converting enzyme (ACE) inhibitors and calcium antagonists are important classes of antihypertensive agents. Within their respective classes, ACE inhibitors and calcium antagonists share common pharmacokinetic properties, but in contrast to ACE inhibitors, some calcium antagonists may cause a significant increase in plasma digoxin concentrations. Clinically, both classes of agents have been shown to be safe and effective in large-scale, long-term clinical trials. ACE inhibitors appear to be very well tolerated and may be associated with fewer adverse effects than some calcium antag

This rare myasthenic disorder is the first neurologic disease found to be caused by antibodies against an ion channel complex. It presents an unusual opportunity to clarify the pathobiology of so-called receptor diseases, as well as to gain better understanding of the normal biology of acetylcholine release and its effect on neuromuscular junctions.

Charybdotoxin (ChTX), a 4.3 kDa polypeptide toxin from the venom of the scorpion Leiurus quinquestriatus, blocks both a Ca-activated K channel (IC50 approximately 15 nM) and a Ca-independent voltage-gated K channel (IC50 approximately 40 nM) in rat brain synaptosomes. These results indicate that in this preparation ChTX is not specific for the Ca-activated K channel and suggest that there may be structural homology among the toxin-binding sites on various types of K channels.

Diabetic cardiomyopathy (DCM) is a major complication of type 2 diabetes mellitus and a leading contributor to heart failure. A central feature of DCM is the disruption of mitochondrial bioenergetics and calcium homeostasis, processes that are tightly regulated through mitochondria-associated endoplasmic reticulum membranes (MAMs). These endoplasmic reticulum (ER)-mitochondria contact sites coordinate mitochondrial ion transport systems, including the mitochondrial calcium uniporter, Na+/Ca2+ exchanger, and potassium channels, which collectively govern mitochondrial metabolism, redox balance,

Calcium signaling is essential for the proper function of immune cells. Recent studies have shown that the scaffold protein, AHNAK1, is important for efficient calcium signaling and NFAT activation in T cells through its ability to properly localize calcium ion (Ca2+) channels at the plasma membrane. Interestingly, both T cells and B cells of systemic lupus erythematosus exhibit activation signaling anomalies with dysregulated Ca2+ response, and enhanced Ca2+ signaling has emerged as a target for treatment with SLE. Therefore, we hypothesized SLE patients may have autoantibodies (Abs) against

Researches have shown that mice lacking the metabotropic glutamate receptor 1 (mGluR) showed albuminuria, remodeling of F-actin, with loss of stress fibers. Selective group I mGluRs agonist (S)-3,5-dihydroxyphenylglycine (DHPG) attenuated albuminuria in several rodent models of nephrotic syndrome. However, the molecular mechanism is obscure. Using a human podocyte cell line, we here investigated the molecular mechanisms of group I mGluRs-induced calcium influx and the formation of stress fibers. Our data showed that group I mGluRs activation by DHPG induced a significant calcium influx, and pr

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disorder associated with intracellular Ca(2+) dysregulation. The P2X receptor family is comprised of ligand-gated ion channels that respond to extracellular adenosine triphosphate (ATP) and increases permeability of calcium into the cell. The underlying mechanisms of purinergic signalling on peripheral blood mononuclear cells (PBMCs) in ALS remain unclear. Herein, we studied the expression of P2X4/P2X7 receptors and calcium homeostasis in blood cells of ALS patients.
METHODS: We used PBMCs from 42 ALS patients and 19 contr

Evodiae fructus (EF), a fruit of Evodia rutaecarpa Bentham, has long been used as an analgesic drug in traditional Chinese and Japanese medicine. However, the underlying molecular mechanism of its pharmacological action is unclear. Here, using calcium imaging, whole-cell patch-clamp recording, and behavioral analysis, we investigated the pharmacological action of EF and its principal compound, evodiamine, on the transient receptor potential (TRP) V1 channels. Dorsal root ganglion (DRG) neurons and TRPV1- or TRPA1-transfected human embryonic kidney-derived (HEK) 293 cells were used for calcium

The carotid body plays an important role in initiating protective responses to hypoxemia. The primary oxygen sensing cells are the glomus or type 1 cells. Hypoxia evokes the secretion of neurotransmitters from these cells which then excite afferent nerves. This response is mediated via membrane depolarization and voltage-gated Ca2+ entry. Studies from this laboratory have revealed that membrane depolarization in response to hypoxia is primarily the result of inhibition of background K+ channels which show strong similarities to the acid sensitive tandem-P-domain K+ channels TASK-1 and TASK-3.
