OBJECTIVES: Hypothalamic hamartomas (HHs) are rare, non-neoplastic malformations associated with gelastic seizures and, less frequently, precocius puberty. Recent classifications have linked HH subtypes with clinical features, and emerging evidence suggests that HH may co-occur with other brain abnormalities. This study investigates the imaging characteristics of HH, the prevalence of concurrent structural abnormalities, and metabolic alterations using magnetic resonance spectroscopy (MRS).
METHODS: We retrospectively analyzed MRI and MRS findings in 18 patients with HH (mean age, 9.24 ± 8 ye

Air pollution (AirP) exposure is associated with increased risk of Alzheimer's disease (AD), yet AirP is chemically heterogeneous, complicating identification of shared pathogenic drivers. We compared acute cortical responses to two chemically distinct but metal-rich AirP sources, diesel exhaust particles (DEP) and World Trade Center (WTC) dust, and contrasted them with woodsmoke (WS), a particulate exposure containing substantially lower metal content. Despite major differences in particle composition and size, DEP and WTC elicited highly convergent transcriptomic responses, sharing more than

BACKGROUND: Tubulin-folding cofactor E (TBCE) plays a central role in tubulin heterodimer formation and disaggregation. Both TBCE biallelic and monoallelic pathogenic variants have been associated with human diseases involving endocrine and/or neurologic system. This study aimed to expand current knowledge on the neurodegenerative phenotype associated with TBCE variants, and to explore possible genotype-phenotype correlations.
METHODS: Subjects with a neurodegenerative syndrome caused by biallelic TBCE variants were recruited from three centers. Clinical, genetic, neuroimaging and neurophysio

INTRODUCTION: ReNU syndrome is a neurodevelopmental disorder (NDD) caused by pathogenic variants in RNU4-2, a non-coding gene encoding the U4 small nuclear RNA (snRNA). As a critical component of the major spliceosome, U4 snRNA is essential for pre-mRNA splicing; however, RNU4-2 is not captured by conventional exome sequencing (ES), often leaving affected individuals undiagnosed.
METHODS: To evaluate the prevalence and clinical impact of RNU4-2 variants, we performed genome sequencing in 95 patients with NDDs in whom prior ES had failed to identify a causative variant.
RESULTS: We identified

ASTN1 encodes astrotactin 1, a neuronal-glial ligand in the developing brain that promotes neuronal migration along radial glia in brain structures with laminar organization, such as the cerebral cortex, hippocampus, and cerebellum. In mouse models, disruption of Astn1 results in neuronal migration deficits, a mild reduction in cerebellar volume, and balance and coordination deficits. In humans, bi-allelic ASTN1 variants have been identified in nine individuals with neurodevelopmental disorders (NDDs) with or without brain malformations. ASTN1 additionally interacts with astrotactin 2 (ASTN2)

Chronic stress is a major risk factor for depression and disrupts myelin integrity in brain regions involved in emotional regulation. Although intermittent fasting (IF) improves metabolic and inflammatory states, its effects on stress-induced depression and demyelination remain unclear. Here, we investigated whether IF alleviates depression-like behaviors and myelin deficits in mice exposed to chronic restraint stress (CRS) and whether these effects involve modulation of the gut microbiota. Adult male C57BL/6 J mice underwent 14 days of CRS while maintained on either an ad libitum (AL) diet or

BACKGROUND: Inflammatory factors interact with brain imaging-derived phenotypes (IDPs); however, the specific causal relationships between inflammatory factors and brain white matter (WM)-related IDPs remain unclear. Thus, we sought to analyze the potential causal effects of inflammatory factors on brain WM-related IDPs using Mendelian randomization (MR).
METHODS: We obtained the data of 41 inflammatory factors from a genome-wide association study (GWAS) from the Cardiovascular Risk in Young Finns Study and the FINRISK surveys, as well as the data of 144 neurite orientation dispersion and den

Pediatric-onset multiple sclerosis (POMS) is an inflammatory demyelinating disorder of the central nervous system that can present with brainstem or cerebellar symptoms and may be initially misattributed to peripheral vestibular etiologies. We report a 15-year-old girl with five days of continuous vertigo described as a sensation that "everything around me is moving," accompanied by unsteady gait and transient unilateral sensory-motor symptoms. Brain magnetic resonance imaging (MRI) with and without contrast demonstrated extensive supratentorial and infratentorial T2-weighted fluid-attenuated

The cuprizone (CPZ) model is widely used for modeling demyelination in multiple sclerosis (MS) and for testing potential remyelination therapies. To better understand the underlying pathology of the CPZ model and evaluate its translatability, we integrated single-cell and spatial transcriptomics (ST) to investigate spatial cellular and molecular interactions during de- and remyelination in multiple brain regions. ST revealed global demyelination and neuroinflammation in the brain beyond the corpus callosum (CC), with region-specific differences. We identified oligodendroglia and microglia as t

BACKGROUND: Obsessive-compulsive disorder (OCD) is commonly treated with cognitive-behavioral therapy (CBT), yet many patients fail to achieve remission. Neuroimaging markers, such as pre-treatment functional and structural connectivity, may help elucidate OCD pathology and CBT mechanisms, and predict treatment outcomes. This study investigates the relationship between pre-treatment functional and structural connectivity and remission status in OCD patients following CBT.
METHODS: Thirty-three OCD patients underwent multimodal MRI, including resting-state fMRI to assess pre-treatment function

Rapid urbanization leads to increased exposure to air pollution, limited greenness, and denser built environments. However, evidence on how these urban factors influence brain development remains limited. We investigated associations between urban characteristics during pregnancy and childhood and brain morphology in preadolescence. The study included 2895 children from the Dutch Generation R Study, with replication in 92 children from the French PELAGIE cohort. Twelve built environment and four urban natural space indicators were estimated at residential addresses during pregnancy and childho
