Ultrastructural examination of the antral G cells has been carried out on 11 patients with chronic duodenal ulcer, before and after treatment with a histamine H-2 - receptor antagonist (cimetidine 1 g/day) for 8 weeks. The study demonstrated an increased area of the Golgi complex, rough endoplasmic reticulum and electron-dense granules, indicating increased G cell activity during treatment. The effect of gastrectomy and duodenal reflux on gastric carcinogenesis was studied because gastrectomized patients may be considered at "high risk" for the development of gastric stump cancer. Wistar rats received N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) (83 mg/liter) ad libitum in the drinking water for either four, eight, or twelve weeks. A control group received tap water. The paper is devoted to analysis of results of the investigation of the motor-evacuatory function of the stomach at different terms after truncal vagotomy with antrumectomy in patients with complicated duodenal ulcers. Postvagotomy gastrostasis was shown to be twice more frequent after antral resection with anastomosis by Billroth-I than by billroth-II. Truncal vagotomy with antrumectomy is not recommended for patients with duodenal ulcers complicated by decompensated stenosis. Gastric resection is thought to be the operation of choice in such cases.

Duodenal duplication cysts are rare malformations that characteristically manifest with intestinal obstruction. In a 50-year experience, we found a duodenal duplication cyst in only three patients, one of whom had massive gastrointestinal bleeding as the initial symptom. Six other duplication cysts (two esophageal, one jejunal, and three ileal) were found. Dulcerozine-induced duodenal ulcers in rats were studied in order to establish the optimum conditions for a routine production of ulcers and to elucidate their possible pathogenesis. Single doses of dulcerozine administered either subcutaneously or orally produced a dose-related duodenal ulceration in rats. The ulcerogenic effect of dulcerozine was most potent when given subcutaneously to 24-hr fasted animals at 300 mg/kg. Gastroduodenostomy (Billroth I) is our reconstruction of choice following gastric resection for gastroduodenal ulcer. In order to evaluate whether prepyloric ulcer (PPU) could be classified as an intermediate ulcer type between duodenal ulcer (DU) and gastric ulcer (GU), fasting serum gastrin as well as basal and pentagastrin-stimulated acid secretion were studied. The aim of the study was to compare the use of cimetidine and colloidal bismuth in the initial healing of chronic duodenal ulcer and to note the recurrence rate over 1 year after healing induced by both forms of treatment. The aim of this study was to assess the clinical results after highly selective vagotomy (HSV) when used routinely at a district general hospital. A total of 229 patients with chronic duodenal or prepyloric ulcers was included in a 92 per cent complete follow-up 1-6 years after an elective HSV without drainage. There was no postoperative mortality, and the frequency of postoperative complicatons was low. Transient dysphagia and early fullness each occurred in about a quarter of the patients, whereas persistent dumping was found only in 1.3 per cent and diarrhea in 2.2 per cent of the patients.
