Comparison of Open Reduction and Internal Fixation with Plate and Titanium Elastic Intramedullary Nail in Treating Pediatric Humeral Fracture.

OBJECTIVE: To investigate the therapeutic efficacy of titanium elastic intramedullary nail (TEN) and open reduction and internal fixation with plate (ORIF) in the treatment of humeral fracture in children. METHODS: A retrospective study was carried out in a total of 69 patients who were admitted to the hospital from January 2013 to December 2018. These patients, including 41 males and 28 females, were aged from 6 to 12 years old with a median of 8 years. These patients were diagnosed with humeral fracture and underwent the surgery of ORIF (n = 22) or TEN (n = 47). The intraoperative bleeding,

Intraarticular Distal Humerus With Anconeus Pedicle and Olecranon Osteotomy.

This review details an open reduction internal fixation technique of a comminuted, intraarticular Y-type bicolumnar distal humerus fracture using supplemental minifragment plating in addition to dual locking plates. Owing to significant articular comminution, an olecranon osteotomy with an anconeus pedicle flap was used to facilitate exposure to achieve an accurate articular reduction to restore elbow function.

Upper Extremity Injuries in Tennis Players: Diagnosis, Treatment, and Management.

Upper extremity tennis injuries are most commonly characterized as overuse injuries to the wrist, elbow, and shoulder. The complex anatomy of these structures and their interaction with biomechanical properties of tennis strokes contributes to the diagnostic challenges. A thorough understanding of tennis kinetics, in combination with the current literature surrounding diagnostic and treatment methods, will improve clinical decision-making.

An unusual complication of ulnar nerve entrapment in a pediatric olecranon fracture: a case report.

The rates of rare complications of acute or late ulnar nerve entrapment after supracondylar fractures, medial condyl fractures, elbow dislocations, forearm fractures, Galeazzi fracture dislocations, and epiphyseal separation of the distal ulna were reported earlier in the literature. Here, we report a late ulnar nerve entrapment after displaced olecranon fracture in a 10-year-old boy.

Subtle Essex-Lopresti lesions: report of 2 cases.

Two patients with high-energy elbow injuries were diagnosed with minimally displaced partial fractures of the radial head that subsequently displaced and were noted to be Essex-Lopresti injuries. The true complexity and instability of the fractures were not immediately obvious. High-energy partial radial head fractures may merit closer observation and evaluation for associated forearm ligament injury.

FDG uptake in a rheumatoid nodule with imaging appearance similar to a malignant soft tissue tumor.

Partial-body F-18 FDG PET/CT was performed in a 40-year-old woman with a soft tissue tumor at the left elbow. She had rheumatoid arthritis for several years. PET/CT showed moderate FDG uptake (SUV max. 4.2) in the known soft tissue lesion. The tumor was resected and histology revealed rheumatoid granulomas with bursitis.

[Results of modern osteosynthesis procedures in fractures of the radial head--are conservative therapy, bone pin osteosynthesis or primary resection still allowed?].

In a retrospective study, we evaluated the outcome of 38 patients who underwent open reduction internal fixation for displaced radial head fractures. We used resorbable material, as well as titanium implants which need no hardware removal. Our results were 84% good, 6% satisfying and 10% unsatisfying. The latter were due to more severe damage to the elbow joint.

Treating joint inflammation in the elderly: an update.

The abrupt onset of monoarthritis in an older patient--especially knee, but also wrist, elbow, ankle, or shoulder--should alert the clinician to the possibility of pseudogout. Joint damage or synovitis may predispose aged patients to sepsis. Rheumatoid synovium, for example, has altered synovial resistance to bacterial seeding; thus, septic arthritis in rheumatoid patients may be polyarticular.

The Challenge of Trans-Ulnar Basal Coronoid Fracture-Dislocations: A Surgical Strategy Based on the Pattern of Coronoid Fracture.

BACKGROUD: The rarity and complexity of trans-ulnar basal coronoid fracture-dislocations pose significant challenges in treatment. This study aimed to categorize these fractures based on coronoid fracture patterns and propose tailored surgical approaches for each type. Additionally, we evaluated the functional and radiological outcomes among the patients managed using our treatment algorithm. METHODS: A total of 19 patients who underwent open reduction and internal fixation for trans-ulnar basal coronoid fracture-dislocations between March 2018 and October 2022 were enrolled in this study. Clinical outcomes and radiographic correlations with fracture complexity in isolated capitellum fractures: A retrospective cohort study.

BACKGROUND: Isolated capitellum fractures, though rare, significantly impact elbow function and are often associated with falls on an outstretched hand (FOOSH). This study aims to evaluate the relationship between forearm radiological parameters and fracture complexity, as well as assess clinical outcomes in patients with isolated capitellum fractures. METHODS: A retrospective cohort analysis was conducted on 28 fractures in 27 patients treated surgically at a tertiary center. Fractures were classified using the Bryan-Morrey system, and forearm parameters (maximum radial bow, location of maxi

Comprehensive Classification of the Capitellar Injury Concurrent with Radial Head Fracture.

BACKGROUND: Capitellar injury (CI) includes capitellar cartilage injury (CCI) and capitellar fracture (CF). A comprehensive classification of CI concurrent with radial head fracture (RHF) that can guide surgical strategy is lacking in the literature. Therefore, this study aimed to introduce a comprehensive classification of CI concurrent with RHF and investigate its value. METHODS: A total of 35 patients with CI concurrent with RHF confirmed by surgical exploration were retrospectively analyzed, includingmales in 19 cases and females in 16 cases. RHF was classified according to the Mason clas

Nonunion of distal humeral fractures: Current concepts.

The incidence of complex articular fractures of the distal humeral in adults has increased and will be growing in the future due to the greater incidence of high energy trauma and to the higher percentage of elderly population. Successful treatment is challenging for the needed balance between a stability of often comminuted fracture and early motion. Nonunion is a common complication after distal humerus fractures that is influenced by a variety of factors such as biology, in particular the blood supply of the metaphysis, the non-anatomical reduction of the fracture, the methods of fixation,

Instability of the proximal radioulnar joint in Monteggia fractures-an experimental study.

BACKGROUND: A Monteggia fracture is defined as a fracture of the proximal ulna combined with a luxation of the radial head. The aim of the present work is to evaluate the extent of instability of the radius head in the proximal radioulnar joint (PRUJ) as a function of the severity of elbow fracture and ligamentous injury in an experimental biomechanical approach. METHODS: Eight fresh-frozen cadaver arms were used. All soft tissues were removed except for the ligamentous structures of the PRUJ and forearm. A tensile force of 40 N was exerted laterally, anteriorly or posteriorly onto the proxim

A retrospective cohort study of displaced segmental radial head fractures: is 2 mm of articular displacement an indication for surgery?

HYPOTHESIS: Many investigators agree that 2 mm of articular displacement is a reasonable indication for open reduction and internal fixation of Mason type II fractures of the radial head. However, there is no evidence to support that this degree of articular displacement is predictive of poor outcomes in conservatively treated fractures. We hypothesized there would be no difference between conservatively treated radial head fractures with greater 2 mm of displacement and those with less than 2 mm of displacement in terms of patient-reported or clinical outcomes. MATERIALS AND METHODS: We revi

In vivo three-dimensional motion analysis of chronic radial head dislocations.

BACKGROUND: Forearm kinematics and interosseous membrane function in chronic radial head dislocations sustained in childhood are unknown. Several procedures have been performed to reduce the radial head on the basis of static preoperative assessment in only one forearm position, but clinical results are not always favorable. QUESTIONS/PURPOSES: We investigated the in vivo three-dimensional (3D) kinematics and length changes of interosseous membrane ligaments during forearm rotation in chronic radial head dislocations using 3D CT registration techniques. METHODS: We examined 10 patients with

Distal humerus lateral condyle fracture in children: when is the conservative treatment a valid option?

INTRODUCTION: Distal humerus condyle fracture in children is rare. These fractures often mislead the emergency physician or surgeon. However, treatment adapted to the degree of displacement achieves excellent clinical and radiographic results. The objective of this study was to clarify indications for conservative treatment of lateral humeral condyle fracture in children. MATERIALS AND METHODS: Twenty-two children who had sustained lateral humeral condyle fracture between January 2007 and January 2010 were reviewed in April 2010. At this consultation, the children underwent clinical and radio
