Improved Outcomes and Cost with Palliative Care in the Emergency Department: Case-Control Study.

INTRODUCTION: Palliative care consultation teams provide significant advantages for patients, healthcare professionals, and hospitals, particularly in pain management, family support, and clinician satisfaction. Numerous studies show that inpatient palliative care services yield benefits regardless of the timing of initiation, contributing to shortened hospital stays and cost savings. Recent studies have focused on the timing and setting of palliative care, especially in emergency departments (ED), highlighting improved patient outcomes when initiated early. This study explores the potential o

Blood transfusions in the emergency department: A descriptive analysis and referral patterns at a non-trauma center.

OBJECTIVES: Anemia is a prevalent condition associated with significant healthcare burden, contributing to approximately 800,000 Emergency Department (ED) visits annually in the United States. While immediate management often relies on packed red blood cell (PRBC) transfusions, limited data exist on the populations receiving transfusions in the ED, especially outside traumatic indications. This study aims to characterize patients receiving PRBC transfusions in the ED, with particular focus on the role of outpatient referrals and potential differences among subgroups based on clinical presentat

Triage decisions and health outcomes among oncology patients: a comparative study of medical and surgical cancer cases in emergency departments.

BACKGROUND: Cancer-related emergencies are a significant challenge for healthcare systems globally, including Jordan. Effective triage is critical in ensuring timely and accurate prioritization of care, especially for surgical cancer patients requiring urgent intervention. However, under-triage-misclassification of high-acuity patients into lower urgency categories-can lead to significant delays and worsened outcomes. Despite the recognized importance of accurate triage, limited research has evaluated its impact on cancer patients in Jordan, particularly those requiring surgical care. OBJECTI

Rapid diagnosis of pediatric nonconvulsive status epilepticus using point-of-care EEG.

BACKGROUND: Nonconvulsive status epilepticus (NCSE) is a time-critical diagnosis in children presenting to pediatric emergency departments (PEDs). Diagnostic delays are common, particularly when isolated altered mental status (AMS) is the only symptom. A standard electroencephalogram (EEG) is essential for diagnosis but rarely available outside regular working hours. Point-of-care EEG (pocEEG) is a rapid, bedside alternative that may support earlier recognition and treatment of NCSE in such situations. METHODS: This study describes the use of a rapid, low-cost, two-channel pocEEG device durin

Optimisation of the deployment of automated external defibrillators in public places in England.

BACKGROUND: Ambulance services treat over 32,000 patients sustaining an out-of-hospital cardiac arrest annually, receiving over 90,000 calls. The definitive treatment for out-of-hospital cardiac arrest is defibrillation. Prompt treatment with an automated external defibrillator can improve survival significantly. However, their location in the community limits opportunity for their use. There is a requirement to identify the optimal location for an automated external defibrillator to improve out-of-hospital cardiac arrest coverage, to improve the chances of survival. METHODS: This was a secon

Ketorolac use in the emergency department in children: a systematic review and meta-analysis.

Acute pain is a frequent reason for pediatric patients visiting the emergency department (ED). Non-steroidal anti-inflammatory drugs (NSAIDs) and opioids are often used to manage pain in this setting. This systematic review and meta-analysis evaluates the effectiveness of ketorolac, a NSAIDs, in managing acute pain in children in the ED, comparing its efficacy and potential advantages over other pain management options. The search was conducted in PubMed and Web of Science, for English-language articles published from 1991 to February 2023. Only randomized controlled trials (RCTs) evaluating t

Takotsubo cardiomyopathy induced by opioid withdrawal: a case report.

BACKGROUND: Takotsubo cardiomyopathy is a stress-induced non-ischemic cardiomyopathy that can result from trauma, stimulant use, financial strain, and opioid withdrawal. Opioid withdrawal induced cardiomyopathy is rarely reported in the literature and is thought to result from increased sympathetic activity and catecholamine surge. Early management of opioid withdrawal with opioid agonists, like methadone or buprenorphine, can be crucial. With the opioid epidemic and ongoing rise in opioid-related overdoses, emergency department physicians should maintain a high clinical suspicion for this ser

Clinical and demographic factors associated with increased risk of postpartum readmission among patients presenting to the emergency department by 6 weeks postpartum.

OBJECTIVE: Postpartum emergency department (ED) visits complicate 12% of births and rates of postpartum readmission are on the rise. While there are a wide range of etiologies, prior studies have sought to delineate causes and risk for readmission. Furthering our understanding of risk factors and etiologies for postpartum readmissions may help develop quality metrics and targeted strategies to address the rising rate of readmissions. We aimed to characterize demographic and perinatal characteristics in postpartum ED visits and evaluate risk factors for readmission. METHODS: A retrospective co

The Use of Ultrasound for Diagnosing and Guiding Successful Transcutaneous Electrostimulation in Pseudo Pulseless Electrical Activity in a Prehospital Emergency Setting: A Case Study.

There are limited methods of differentiating between pulseless electrical activity (PEA) and pseudo-PEA in both prehospital and early hospital settings. According to the European Resuscitation Council (ERC) guidelines, point-of-care ultrasound (POCUS) is a rapid and effective method for differentiating between PEA and pseudo PEA. In addition, ERC guidelines recommend the use of POCUS in cases of non-shockable rhythms for the diagnosis of potentially reversible causes of sudden cardiac arrest (SCA). Here, we present the case of a 73-year-old patient who experienced sudden cardiac death due to a

Management of cannabinoid hyperemesis syndrome with an erector spinae plane block in the emergency department.

Cannabinoid hyperemesis syndrome (CHS) is characterized by recurrent nausea, vomiting, and abdominal pain among patients with chronic cannabis use. Emergency department (ED) management is often difficult, as conventional analgesics and antiemetics are frequently ineffective. We describe a case of refractory CHS successfully managed in the ED with a thoracic erector spinae plane block (ESPB). Following the block, the patient had complete symptom relief and was able to be discharged from the ED, avoiding the need for hospital admission for symptom control. Through blockade of sympathetic viscera
