Smart fracture plate for quantifying fracture healing: Preliminary efficacy in a biomechanical model.

The diagnosis of fracture nonunion following plate osteosynthesis is subjective and frequently ambiguous. Initially following osteosynthesis, loads applied to the bone are primarily transmitted through the plate. However, as callus stiffness increases, the callus is able to bear load proportional to its stiffness while forces through the plate decrease. The purpose of this study was to use a "smart" fracture plate to distinguish between phases of fracture healing by measuring forces transmitted through the plate. A wireless force sensor and small adapter were placed on the outside of a distal

Biomechanics of periprosthetic femur fractures and early weightbearing.

PURPOSE: The incidence of periprosthetic fractures is expected to rise increase by 4.6% every 10 years between 2015 and 2060. There are few large series examining optimal fixation constructs or the influence of early ambulation on outcome. The purpose of this narrative review is to investigate the published biomechanical considerations for periprosthetic fracture fixation, with specific consideration of early postoperative weightbearing. METHODS: A literature review was performed to identify fracture incidences, etiology, and current trends in weightbearing after fixation. Benefits of early w

Contemporary Management of Osteoporotic Fractures.

With aging of the population, osteoporotic fractures are becoming an increasing medical problem world-wide. It has been estimated that 2,700,000 patients experienced a low energy fracture in the Polish population in 2010. On the basis of contemporary world standards and publications in the field of orthopaedics and trau-matic surgery, a summary of the principles of management of osteoporotic fractures is presented. Both general problems of fracture treatment in elderly patients as well as difficulties in surgical and conservative treatment resulting from osteoporotic bone abnormalities are dis

The effect of haemorrhagic shock and resuscitation on fracture healing in a rabbit model: an animal study.

AIMS: Little is known about the effect of haemorrhagic shock and resuscitation on fracture healing. This study used a rabbit model with a femoral osteotomy and fixation to examine this relationship. MATERIALS AND METHODS: A total of 18 male New Zealand white rabbits underwent femoral osteotomy with intramedullary fixation with 'shock' (n = 9) and control (n = 9) groups. Shock was induced in the study group by removal of 35% of the total blood volume 45 minutes before resuscitation with blood and crystalloid. Fracture healing was monitored for eight weeks using serum markers of healing and rad

Challenge to treat hypertrophic nonunion of the femoral shaft: the Poller screw augmentation technique.

The management of a femoral nonunion after intramedullary nailing is challenging. Exchange nailing or plate augmentation has been used to treat hypertrophic nonunions previously. The Poller screw augmentation technique is a simple procedure that can be performed in the outpatient surgery. In this study, we highlight the method of hypertrophic nonunion management according to the specific indication including our Poller screw augmentation technique.

Lower extremity angular malunion: evaluation and surgical correction.

The lower extremity has a mechanical axis with joint orientation that allows joint longevity and efficiency in bipedal gait. When normal alignment is lost because of trauma or other conditions, deviations from this anatomic norm may be deleterious to long-term joint function. In fractures that have healed with angular malunion, all facets of the deformity must be carefully considered, including alteration in length, rotation, alignment, and translation. Once all elements are fully defined, the effects of the malunion on mechanical axis and joint orientation can be understood. Techniques for su

Three-dimensional fracture mapping and analysis of coronal fractures in AO/OTA types 33-B3 and C3.

BACKGROUND: Although the relatively high incidence of coronal fractures in the supracondylar-intercondylar fractures is well established, little is currently known about the morphology of those fractures. Herein, we characterized the coronal fractures in AO/OTA type 33-C3 and assessed their differences with Busch-Hoffa fractures (33-B3). METHODS: We retrospectively collected 61 cases of AO/OTA type 33-B or C fractures with coronal plane fragments and generated three-dimensional fracture maps of those with coronal fractures based on CT imaging and measured angle α (the angle between the corona

The Management of Orthopaedic Emergencies for the Community Surgeon.

Orthopaedic emergencies demand rapid recognition and decisive action, particularly in resource-constrained or nontertiary settings where the community surgeon often serves as the first and only line of defense. It is important to provide a practical, evidence-informed guide for the management of musculoskeletal injuries, including irreducible joint dislocations, ultra-low-energy knee dislocations, open fractures with vascular compromise, compartment syndrome, and femoral neck fractures in young patients. Emphasis is placed on early diagnosis, stabilization strategies, soft-tissue management, a

Healing fragile bones: a case report on hyperbaric oxygen therapy in pycnodysostosis.

Pycnodysostosis is a rare lysosomal storage disorder characterised by short stature, craniofacial dysmorphisms, dental anomalies, and increased bone fragility due to osteoclast dysfunction caused by cathepsin K gene mutations. This case report describes a 43-year-old female pycnodysostosis patient with recurrent subtrochanteric fractures and delayed bone healing following multiple surgical interventions, including femoral osteotomy and bone grafting. Despite these efforts, bony union was not achieved. The patient underwent 39 sessions of hyperbaric oxygen therapy (HBOT), administered at 243.2

Massive Osteolysis of the Femur (Gorham-Stout Disease) Requiring Total Femur Replacement: A Case Report.

CASE: Gorham-Stout disease (GSD) is a rare disorder marked by massive bone destruction. A 56-year-old woman with GSD presented to our hospital with a proximal femur fracture. She was initially diagnosed with a simple fracture and underwent open reduction and internal fixation using an intramedullary nail. However, the delayed diagnosis of GSD resulted in postponed appropriate treatment. Although the intramedullary nail helped prevent mechanical bone failure, extensive osteolysis continued, ultimately necessitating total femur replacement. CONCLUSION: This case highlights the importance of con

Radiographic Union Assessment in Surgically Treated Distal Femur Fractures: A Systematic Review.

BACKGROUND: Distal femur fractures are known to have challenging nonunion rates. Despite various available treatment methods aimed to improve union, optimal interventions are yet to be determined. Importantly, there remains no standard agreement on what defines radiographic union. Although various proposed criteria of defining radiographic union exist in the literature, there is no clear consensus on which criteria provide the most precise measurement. The use of inconsistent measures of fracture healing between studies can be problematic and limits their generalizability. Therefore, this syst

Is There a "Black Friday" for Geriatric Hip Fracture Surgery?

OBJECTIVES: Reports show an increase in the short-term mortality rates of hip fracture patients admitted on weekends. However, there are few studies on whether there is a similar effect in Friday admissions of geriatric hip fracture patients. The aim of this study was to evaluate the effects of Friday admission on mortality and clinical outcomes in elderly patients with hip fractures. METHODS: A retrospective cohort study was performed at a single orthopaedic trauma centre and included all patients who underwent hip fracture surgery between January 2018 and December 2021. Efficacy and safety of modular versus monoblock stems in revision total hip arthroplasty: a systematic review and meta-analysis.

BACKGROUND: Both modular and monoblock tapered fluted titanium (TFT) stems are increasingly being used for revision total hip arthroplasty (rTHA). However, the differences between the two designs in clinical outcomes and complications are not yet clear. Here, we intend to compare the efficacy and safety of modular versus monoblock TFT stems in rTHA. METHODS: PubMed, Embase, Web of Science, and Cochrane Library databases were searched to include studies comparing modular and monoblock implants in rTHA. Data on the survivorship of stems, postoperative hip function, and complications were extrac

The effects of hydroxychloroquine-induced oxidative stress on fracture healing in an experimental rat model.

OBJECTIVES: The purpose of this study was to investigate whether hydroxychloroquine (HCQ) sulfate causes oxidative stress (OS) and its effect on fracture healing in an experimental rat model. MATERIALS AND METHODS: In this experimental study, open diaphyseal femur fractures were induced in 24 eight-week-old male rats (mean weight: 225±25 g; range, 200 to 250 g) and then fixed with K-wire. The rats were divided into four groups: HCQ-2, control-2 (C-2), HCQ-4, and control-4 (C-4). During the study period, rats in the HCQ groups received an HCQ solution (160 mg/kg/day), whereas rats in the contr

Assessing the Necessity for the "Joint Above and Below" Radiography Approach for Lower-extremity Long Bone Fractures in Children.

OBJECTIVES: A common strategy for evaluation of extremity fractures is the "joint above and below" (JAB) radiograph approach, which includes dedicated imaging of the joint proximal and distal to a fracture independent of clinical suspicion for an injury involving the joint. The incidence of concomitant ipsilateral lower-extremity fractures or dislocations associated with lower-extremity long bone fractures in children has not been commonly reported and represents an evidential gap for determining a radiograph approach. Our purpose was to determine the frequency of and risk factors for concomit
