Simultaneous bilateral shoulder and bilateral central acetabular fracture dislocation: What to do?

Musculoskeletal injuries following seizures have a high morbidity and mortality. These injuries are often missed and the diagnosis is delayed due to a lack of clinical suspicion and appropriate investigations. We report a case of 72 years old male with simultaneous bilateral central acetabular fracture dislocation and bilateral posterior shoulder fracture dislocation secondary to an epileptic seizure. Present study high- lights the significance of clinical suspicion and clinico-radiological evaluation for diagnosis of a rare injury following episode of seizures. Simultaneous fracture dislocatio

T-condylar fracture delayed for 10 days in a 5-year-old boy: a case report and review of the literature.

T-condylar fracture is rare in paediatric age group, especially in skeletally immature children less than 9 years old, with very few cases reported in available literature. We present such a case in a 5 year old child that was initially managed as a supracondylar fracture at another centre before referral to us, 10 days after the injury. The child was diagnosed as having a displaced T-condylar fracture on plain radiograph. Open reduction and internal fixation with K-wires was performed. At 2 years follow-up, the child had good range of motion at elbow with 5°of cubitus varus. With this backgro

Does the term scaphocapitate syndrome need to be revised? A report of 6 cases.

This is a retrospective study of six patients with displaced fractures of the neck of the capitate. In four of the six patients the diagnosis was delayed by at least 1 week. After open reduction and internal fixation the results were assessed as excellent (three cases), fair (two cases) or poor (one case). In all cases there was a concomitant injury to the radial side of the wrist: in four cases this was a fracture of the waist of the scaphoid. In five of the six cases there was also an injury to the ulnar side of the wrist. As a scaphoid fracture does not always occur with the fracture of the

Fractures of the humerus in arm wrestlers.

In 7 cases of fracture of the humerus occurring in arm wrestlers, each patient sustained a simple spiral fracture of the distal third of the humerus. No patient had any neural or vascular compromise. Three patients underwent open reduction and internal fixation with 2 screws, at which time, muscle was found to be interposed between the fracture fragments. The other 4 patients were treated in hanging arm casts. All of the fractures united. Of the 41 cases of this injury reported in English medical literature there were 5 patients with radial nerve palsy and 16 patients who had had operative tre

Fracture of the shaft of the femur in children.

Sixty-five cases of fracture of the femur shaft caused by trauma in children under age 12 were studied. All were treated by various conservative methods. Two patients later underwent open reduction and internal fixation. Children below 5 years of age were treated by Bryant's suspension traction method. Older children were treated by balanced skin traction. In a few cases where the position was not acceptable, the fracture was reduced under general anesthesia and fixed traction was applied. Patients were discharged with a hip spica when the fracture site was nontender (average three weeks). Und

Pertrochanteric Femur Fracture in a Post-Polio Patient treated with a Pediatric Hip Plate: A Case Report.

INTRODUCTION: Fracture fixation in post-polio residual paralysis is technically demanding due to dysplastic proximal femoral anatomy, osteopenia, and distorted trochanteric morphology. Standard cephalomedullary nailing may be impossible when the medullary canal is narrow, or the entry point is malpositioned. CASE REPORT: A 45-year-old man with childhood poliomyelitis and complete paralysis of the right leg sustained an AO 31-A1.2 pertrochanteric fracture. Radiographs revealed coxa valga and a markedly narrow canal, precluding safe cephalomedullary nailing. Closed reduction on a traction table

Catastrophic complications from inadequate early soft tissue surveillance in a closed pilon fracture: a case report.

We present the case of a 55-year-old man with an AO/OTA 43-C3 pilon fracture in whom initial uniplanar external fixation failed to relieve persistent medial skin tenting, resulting in focal ischemic necrosis. Within 72 hours, the patient developed bullae and violaceous discoloration, which progressed to full-thickness skin breakdown. Despite staged open reduction and internal fixation and fasciocutaneous flap coverage, the patient developed chronic osteomyelitis, ultimately requiring segmental bone resection to control the infection. The patient underwent serial debridement procedures and plac

The role of a small posterior malleolar fragment in trimalleolar fractures: a biomechanical study.

AIMS: The aim of this study was to investigate the effect of a posterior malleolar fragment (PMF), with < 25% ankle joint surface, on pressure distribution and joint-stability. There is still little scientific evidence available to advise on the size of PMF, which is essential to provide treatment. To date, studies show inconsistent results and recommendations for surgical treatment date from 1940. MATERIALS AND METHODS: A total of 12 cadaveric ankles were assigned to two study groups. A trimalleolar fracture was created, followed by open reduction and internal fixation. PMF was fixed in Grou

Arthroscopic Reduction and Fixation of Transverse Intra-articular Glenoid Fractures With Scapular Extension.

The most common procedure to address transverse glenoid fractures that are characterized by intra-articular step-off or gapping is open reduction and internal fixation. Disadvantages of open surgery are delay in regaining full range of motion, increased approach morbidity, neurovascular complications, and the need for capsulotomy, which delays healing and increases the risk of stiffness. An arthroscopically assisted fracture fixation, as described in this article, is characterized by better visualization of the glenoid articular surface and reduction of the intra-articular fragments under dire

The effect of minimally displaced posterior malleolar fractures on decision making in minimally displaced lateral malleolus fractures.

PURPOSE: Despite the high incidence of cases of minimally displaced lateral or posterior malleolus ankle fractures, treatment guidelines are still an issue of controversy. The purpose of this study was to delineate treatment preferences among orthopaedic surgeons in these fractures with and without concomitant posterior malleolus fractures. We hypothesized that concomitant minimally displaced fractures of the posterior malleolus can shift treatment preference towards operative intervention. METHODS: A questionnaire-based study was conducted among orthopaedic surgeons attending the 2012 Europe

Neglected neck femur fractures in adolescents and young adults: factors predicting the surgical outcome.

BACKGROUND: Neglected femoral neck fracture in young adults is an intriguing problem. This retrospective study tried to solve that challenge through open reduction, cannulated screw internal fixation, autogenous iliac bone and bone marrow grafting. METHODS: Thirty-six cases were studied; they were classified according to Sandhu et al.'s classification. Twenty cases were type I and 16 cases were type II fractures; the mean age was 26.8 years; fracture neglect averaged 44.6 days. Twenty cases had posterior comminution and 16 cases had anterior comminution. All cases had open reduction, cannulat

Evaluation of nonunions of the tibia diaphysis - own experience.

BACKGROUND: Delayed union is not an expected outcome of treatment of a bone fracture. The cause of the delayed union must be established before treatment of the complication can be undertaken. The method of treatment depends mainly on the specific nature of delayed bone union. The study evaluates the causes of delayed bone union during treatment of tibial shaft fractures. MATERIAL AND METHODS: Seventy-two patients with delayed union following a fracture of the tibial diaphysis were treated in the Department of Orthopaedics and Musculoskeletal Traumatology of Wrocław Medical University between

Operative vs Nonoperative Management of Fractures of the Humeral Diaphysis: The Humeral Shaft Fracture Fixation Randomized Clinical Trial.

IMPORTANCE: Humeral shaft fractures are routinely managed nonoperatively, but this approach is potentially associated with higher nonunion rates and inferior functional outcomes when compared with operative fixation. OBJECTIVE: To assess whether there is any difference in outcome between surgery and functional bracing for adults with an isolated, closed humeral shaft fracture. DESIGN, SETTING, AND PARTICIPANTS: This prospective, superiority, parallel-group randomized clinical trial was conducted between September 2018 and October 2023 and took place at an academic major trauma center in the

Pathological fracture of the humeral shaft: Antegrade intramedullary nailing versus plate osteosynthesis with bone defect cementation.

PurposeClinically, metastatic pathological fractures of the humeral shaft remain a challenging issue. Plate osteosynthesis and intramedullary nailing are common fixation methods; however, the optimal strategy remains debated, especially in patients with limited life expectancy where surgical goals include rapid pain relief, functional recovery, and fewer reoperations.MethodsWe retrospectively reviewed 27 patients with metastatic humeral shaft fractures treated surgically during 2009-2019. Patients were grouped by plate osteosynthesis (n = 17) and antegrade intramedullary nailing (n = 10), and

Temporary immobilization methods for closed low-energy ankle fracture-dislocations: comparative analysis of a retrospective cohort.

PURPOSE: Ankle fracture-dislocations (AFD) often necessitate staged management involving temporary external fixation (EF) due to mechanical instability or blistering. However, limited literature exists on the optimal temporary immobilization method for low-energy closed AFD. This study compared baseline patient and fracture characteristics, along with clinical and radiological outcomes between AFD initially immobilized with EF versus splinting. METHODS: A retrospective cohort study was conducted involving patients with AFD temporarily immobilized using EF or splinting, followed by definitive
