Thin patient-specific clavicle fracture fixation plates can mechanically outperform commercial plates: An in silico approach.

Current fixation plates used to operatively stabilize clavicular fractures are suboptimal, leading to reoperation rates up to 53%. Plate irritation, which can be caused by a poor geometric fit and plate thickness, has been found to be an important factor for reoperation. Moreover, muscle attachment sites (MAS) have to be detached occasionally. To improve current surgical clavicle fracture treatment with plate osteosynthesis, a change in plate design is required. The goal of this study was to design a patient-specific clavicle fracture fixation plate that includes geometrical optimization and s

Hook Plate Fixation of Medial Malleolar Fractures: A Comparative Study of Clinical Outcomes.

Medial malleolar fractures, occurring in isolation or in bi- or trimalleolar fractures, require surgical fixation if there is any displacement. Several techniques have been described in the literature for open reduction and internal fixation of medial malleolar fractures, but no data exist on the functional outcomes after fixation with a hook plate. The objective of this review study was to compare complication and union rates in patients who underwent hook plate fixation of medial malleolar fractures to those who underwent fixation with 2 lag screws. A total of 31 patients were included in th

Intramedullary Nail for a Distal Tibia Fracture.

Extraarticular fractures of the distal tibia have historically been treated with open reduction and fixation with plates and screws. This technique requires a more extensive dissection and comes with a higher risk of wound complications than intramedullary nail fixation. This article and the accompanying video demonstrate the use of closed reduction and suprapatellar nail fixation appropriate for the treatment of most extraarticular distal tibial fractures. A variety of treatment decisions are discussed, including the nail insertion method, fixation of associated fibular fractures, and postope

The impact of odontoid screw fixation techniques on screw-related complications and fusion rates: a systematic review and meta-analysis.

PURPOSE: The primary goal of this study was to conduct a systematic review and meta-analysis of articles focused on odontoid screw fixation (OSF) and screw-related complications or non-union rates. METHODS: We conducted a systematic review of the PubMed and Crossref databases between January 1982 and December 2019. Inclusion criteria comprised detailed descriptions of the surgical technique and screw-related complications (screw cut-out, loosening, breakage, malposition) or fusion rates. RESULTS: The initial selection consisted of 683 abstracts. A total of 150 full texts were chosen for deta

[The PHILOS plate-also an indication for osteosynthesis of the distal tibia?].

According to the manufacturer's instructions the application of a PHILOS plate is restricted to humeral fractures. An extension to other anatomical regions of the body is not provided; however, based on the anatomical design of the plate it was observed that the application of this plate also appears to be possible for the distal tibia. This article reports three different osteosyntheses by a reverse PHILOS plate on the medial malleolus and on the distal tibia posteriorly with a short and a long PHILOS plate design. In summary, the applications have so far resulted in primary wound healing wit

Novel Treatment Options for the Surgical Management of Young Femoral Neck Fractures.

In physiologically young patients with displaced femoral neck fractures, surgical treatment is aimed at achieving fracture union while preserving native hip anatomy and biomechanics. The intracapsular environment, tenuous vascular supply, and unfavorable hip biomechanics contribute to the high complication rates seen after osteosynthesis of these fractures. Conventional fixation methods for osteosynthesis of femoral neck fractures include multiple cancellous screws, fixed-angle dynamic implants, and fixed-angle length-stable constructs. Despite several biomechanical and clinical studies evalua

Percutaneous vertebral augmentation in fragility fractures-indications and limitations.

INTRODUCTION: There is still no general consensus about the management of osteoporotic vertebral fractures. Recommendations depend on type of fracture, grade of instability, bone quality, and general conditions of the patient. Spontaneous fractures may be considered to be treated different compared to cases with high-velocity trauma. METHODS: According to the DVO, patients without trauma should first be treated conservatively. However, there is no more strict time protocol of 3 or 6 week conservative treatment before operations may be indicated. Surgical criteria are not yet distinctly define

Internal fixation of proximal humeral fractures.

Fractures of the proximal humerus are common, especially in elderly individuals, and are usually classified according to Neer into fractures having one, two, three, or four parts with or without an associated dislocation or an injury splitting the humeral head. This article is intended to assist the surgeon in selecting and using techniques for internal fixation of these fractures. The use of open reduction and internal fixation using plates and screws has become less popular because of the extensive dissection required and the tendency for the plate to impinge. Use of multiple percutaneous pi

[Corrective interventions in malunited fractures of the distal radius].

The current indications and technique of wrist osteotomies for ununited fractures of the distal radius are presented. A three-dimensional correction at the metaphyseal level is technically feasible. This not only restores the normal orientation of the articular surface thus reestablishing normal load distribution but also the radial length up to 10 mm which is a prerequisite to prevent ulno-carpal impingement. Painful limitation of forearm rotation due to degenerative changes in the distal radioulnar joint may be treated with Bowers hemiresection arthroplasty. If the radioulnar index should be

The acutely injured wrist and its residuals.

Carpal injuries are confusing and treacherous. Understanding the mechanisms are important in diagnosis and treatment. The mechanisms of extension, intercarpal supination and ulnar deviation can lead to most of the fractures and dislocations of the carpus and distal radius. Internal fixation of some avulsion type fractures will stabilize the wrist. Most carpal fractures and dislocations occur in the vulnerable perilunar zone consisting of the trapezium scaphoid, the head of the capitate and the tail of the hamate and the lunar border of the triquetrum. Carpal injuries occur in 3 stages. They st

Microstructure of the anterior iliac Spine: Identification of trends and relation to fracture tolerance.

Seatbelt-induced pelvic iliac wing injuries have been observed since the 1970s, but only recently has there been quantification of fracture tolerance and injury risk of the iliac wing. Previous studies have shown a wide variation in iliac wing fracture tolerance with no significant relationships to pelvis size, sex, or other factors. A weighted average bone density (BD) calculation of the entire iliac wing produced the best predictive performance of fracture tolerance in parametric (Weibull) survival models. As a result, we endeavored to evaluate local bone microstructural properties at the si

The therapeutic effect of Astaxanthin on drill-induced closed femoral fracture model in male Wistar rats.

PURPOSE: Bone fracture is one of the most significant injuries of the body, which is usually caused by trauma. Astaxanthin (AST) showed beneficial effects on inflammation and oxidative stress, indicating protective effects on bone tissue. This study was performed with the aim of evaluating the effect of AST on the improvement of behavioral changes and the ossification process in the defect created in the femur of the rats. METHODS: Animals were randomly divided into 3 groups, sham (healthy), control (bone fracture), and treatment. The sham group received water and normal food. In the control

Management of Long Bones Metastatic Disease: Concepts That We All Know but Not Always Remember.

Bones are the third most common site of metastatic disease. Treatment is rarely curative; rather, it seeks to control disease progression and palliate symptoms. Imaging evaluation of a patient with symptoms of metastatic bone disease should begin with plain X-rays. Further imaging consists of a combination of (PET)-CT scan and bone scintigraphy. We recommend performing a biopsy after imaging workup has been conducted. Metastatic bone disease is managed with a combination of systemic treatment, radiotherapy (RT), and surgery. External beam RT (EBRT) is used for pain control and postoperatively

[Osteoporosis in pneumological diseases : Joint guideline of the Austrian Society for Bone and Mineral Research (ÖGKM) and the Austrian Society for Pneumology (ÖGP)].

Chronic inflammation induces proinflammatory cytokine cascades. In addition to systemic inflammation, hypoxemia, hypercapnia, a catabolic metabolism, gonadal or thyroid dysfunction, musculoskeletal dysfunction and inactivity as well as vitamin D deficiency contribute to an increased risk of fragility fractures. Iatrogenic causes of osteoporosis are long-term use of inhaled or systemic glucocorticoids (GC). Inhalative GC application in asthma is often indicated in childhood and adolescence, but interstitial lung diseases such as chronic organizing pneumonia, COPD, sarcoid or rheumatic diseases

Anabolic Agents for Postmenopausal Osteoporosis: How Do You Choose?

PURPOSE OF REVIEW: There are now three anabolic agents available for the treatment of postmenopausal women at high risk for fracture. The purpose of this review is to supply a rationale to aid in determining which agent should be used in which clinical settings. RECENT FINDINGS: Studies over the last decade have shown that anabolic agents produce faster and larger effects against fracture than antiresorptive agents. Furthermore, trials evaluating anabolic antiresorptive treatment sequences have shown that anabolic first treatment strategies produce the greatest benefits to bone density, parti

Combined effect of teriparatide and low-intensity pulsed ultrasound for nonunion: a case report.

BACKGROUND: Low-intensity pulsed ultrasound is a pain-free therapy performed daily at home by the patient and has been shown to promote fracture healing. Teriparatide is a parathyroid hormone preparation that activates osteoblastic bone formation and is also reported to be effective in promoting bony union. CASE PRESENTATION: We report the case of a 56-year-old Japanese male with a femoral shaft fracture who underwent intramedullary osteosynthesis nailing initially. He had no radiologic or clinical sign of healing 3 months later and low-intensity pulsed ultrasound was initiated at that time.
