Early Spontaneous Recanalization of Sigmoid Sinus Thrombosis Following a Closed Head Injury in a Pediatric Patient : A Case Report and Review of Literature.

Cerebral venous sinus thrombosis (CVST) following a closed head injury in pediatric patients is a rare condition, and an early spontaneous recanalization of this condition is extremely rare. A 10-year-old boy was admitted with a mild, intermittent headache and nausea five days after a bicycle accident. The brain computed tomography showed an epidural hematoma at the right occipital area with pneumocephalus due to a fracture of the occipital skull bone. The brain magnetic resonance imaging and the magnetic resonance venography demonstrated a flow signal loss from the right sigmoid sinus to the

Anesthetic management of a pediatric patient with Leigh syndrome.

BACKGROUND AND OBJECTIVES: Leigh syndrome (LS) is a rare disease caused by abnormalities of mitochondrial energy generation. The central nervous system is most frequently affected, with psychomotor underdevelopment, seizures, nystagmus, ophthalmoparesis, optic atrophy, ataxia, dystonia, or respiratory failure. Surgical and anesthetic procedures stimulate the tracheal irritability, and could exacerbate risks of aspiration, wheezing, breathing difficulties, gasping, hypoventilation, and apnea. CASE REPORT: We present the anesthetic management for a six-year-old boy with severe form of LS, invol

[A case of tracheoesophageal fistula cured by surgical therapy after blunt trauma received 38 years ago].

We experienced a case of tracheoesophageal fistula successfully cured by surgical therapy after blunt trauma received 38 years ago. A 71-year-old man was injured blunt trauma at right chest by traffic accident in 1960, and was treated for pneumothorax and ribs fracture. In April, 1998, the patient came to the hospital for hemoptysis. Tracheoesophageal fistula at membranous wall 3 cm upper from the carina was diagnosed and operation was performed. Severe adhesion at 3 cm upper from carina was thought to be fistula. Incision was made at lateral esophageal wall and the fistula was confirmed at th

Oblique Lateral Closing-Wedge Osteotomy for Cubitus Varus in Skeletally Immature Children: A Case Series.

INTRODUCTION: Cubitus varus is the most common late complication following displaced supracondylar fractures of the humerus in children and is primarily caused by malunion. Although often considered cosmetic, it may lead to functional impairment and long-term complications. Corrective osteotomy remains the standard treatment, with multiple techniques described. This study evaluates the clinical and radiological outcomes of oblique lateral closing wedge osteotomy in pediatric patients. CASE SERIES: This case series includes five pediatric patients (2 males, 3 females; mean age 7.6 years) with

Closed reduction of a dislocated constrained total hip arthroplasty using a novel technique with a PEG board.

This article describes a case of closed reduction of a dislocated total hip arthroplasty (THA) out of a constrained acetabular liner using a novel technique with a peg board secured to the operating table.A 79-year-old woman sustained multiple dislocations after a primary THA. She was converted to a constrained liner and sustained no further dislocations for 5 years. She presented after dislocating her hip while getting out of a chair. The patient was taken to the operating room, placed under general anesthesia, and laid supine on the operating table, to which a peg board had been previously s

Abusive head trauma: differentiation between impact and non-impact cases based on neuroimaging findings and skeletal surveys.

OBJECTIVES: To determine whether imaging findings can be used to differentiate between impact and non-impact head trauma in a group of fatal and non-fatal abusive head trauma (AHT) victims. METHODS: We included all AHT cases in the Netherlands in the period 2005-2012 for which a forensic report was written for a court of law, and for which imaging was available for reassessment. Neuroradiological and musculoskeletal findings were scored by an experienced paediatric radiologist. RESULTS: We identified 124 AHT cases; data for 104 cases (84%) were available for radiological reassessment. The AH

[Injuries of carotid arteries].

Carotid artery injuries reveal as a specific and relatively small group of vascular trauma. Hemorrhage from vessels of this diameter and flow (10 % of cardiac output) has predominantly a fatal end or shows severe neurological sequelae. Cut and stab wounds represent the majority of carotid injuries, often associated with venous damage. Blunt trauma and fractures of the skull basis are uncommon causes. We present 43 cases of carotid injuries operated upon as emergency service (in the majority outside of our hospital). The number of postoperative surgical and neurological complications was modera

[Traumatic subarachnoid-pleural fistula causing tension pneumocephalus].

Uncommon subarachnoid-pleural fistula can be caused by a blunt or penetrating trauma and can also occur after a thoracal or neurosurgical operation, ending to a progredient pneumocephalus. Clinical findings might be an enlarging tension pneumocephalus without a cranial fracture, but in most times they will be overlayed by other accompanied injuries, neurologic symptoms can dominate, also rapidly filling clear pleural effusions with obscure cause. Oppurtune identification of the reason of the pneumocephalus and mostly early surgical intervention prevent neurological deficit, CNS infection and o

Outcome of traumatic optic neuropathy. Comparison between surgical and nonsurgical treatment.

34 patients with indirect traumatic optic neuropathy were studied to identify factors affecting outcome and surgical indications. 12 cases (13 eyes = group A) underwent surgery and 24 patients (24 eyes = group B) were managed without surgery. Age, optic canal fracture, visual acuity before treatment (initial visual acuity) and days until surgery (only group A) were employed as variables. Visual acuity improved significantly more in patients with initial visual acuity, hand movement (HM) or better than in those with initial visual acuity for light perception (LP) only or worse. When initial vis

Narrative Review on the Management of Neck of Femur Fractures in People Living with HIV: Challenges, Complications, and Long-Term Outcomes.

Neck of femur (NOF) fractures are a critical orthopaedic emergency with a high morbidity and mortality prevalence, particularly in people living with Human Immunodeficiency Virus (PLWHIV). A combination of HIV infection, combined antiretroviral therapy (cART), and compromised bone health further increases the risk of fragility fractures. Additionally, HIV-related immune dysfunction, cART-induced osteoporosis, and perioperative infection risks further pose challenges in ongoing surgical management. Despite the rising global prevalence of PLWHIV, no specific guidelines exist for the perioperativ

Opioid-free Postoperative Pain Management in Pediatric Upper Limb Fracture Surgery: A Prospective Observational Study.

BACKGROUND: Postoperative pain management in pediatric fracture surgery is challenging. Opioid analgesics, conventionally used for moderate-to-severe pain, are associated with significant adverse effects in children, including respiratory depression, nausea, and sedation. Growing emphasis on opioid stewardship has increased interest in opioid-free strategies; however, real-world data in pediatric orthopedic trauma remain limited. OBJECTIVES: The objective is to evaluate postoperative pain outcomes and analgesic requirements in children undergoing upper limb fracture surgery managed with a sta

Critical Analysis of Pediatric Orthopaedic Surgery Medical Malpractice Litigation: A Nationwide Retrospective Review.

PURPOSE: The primary aim of this study was to identify and characterize medical malpractice lawsuits pertaining to pediatric orthopaedic surgery. METHODS: The Westlaw research database was queried for all jury verdicts and settlements pertaining to pediatric orthopaedic medical malpractice from 1980 to 2024. Cases were only included if the primary basis of litigation rested on a malpractice claim related to pediatric orthopaedic surgery and were levied against an orthopaedic surgeon. Data collected included date and state of case filing, patient (plaintiff) and surgeon (defendant) demographic

Humeral Fractures in Metabolically Compromised Bone: Outcomes of Structural Strut Onlay Allograft with Open Reduction and Internal Fixation - A Case Series of Three Patients.

INTRODUCTION: The "terrible tetrad" of the elbow - simultaneous posterior ulno-humeral dislocation, comminuted radial head fracture with dislocation, coronoid process fracture, and proximal ulnar fracture - is an injury pattern that simultaneously fulfills criteria for both the terrible triad and a Monteggia-type fracture-dislocation. Fewer than fifteen cases have been documented worldwide. We present two cases managed by different surgeons at different institutions and propose the formal adoption of the term "terrible tetrad." CASE REPORT: Case 1: A 50-year-old male painter sustained a right

Outcomes after initial nonoperative management of undisplaced intracapsular femoral neck fractures: a five-year single-center cohort study with structured literature review.

BACKGROUND: Early surgical fixation is the standard approach for intracapsular femoral neck (neck of femur; NOF) fractures. However, a small subgroup of patients is managed conservatively because operative risks outweigh benefits, or due to patient preference. We evaluated outcomes of initial nonoperative management in older adults and contextualised our findings through a structured literature review. METHODS: We conducted a retrospective observational cohort study at a UK tertiary orthopaedic trauma unit (January 2018-August 2023). Patients with undisplaced or valgus-impacted NOF fractures

Enhancing Functional Outcomes in Distal End Radius Fracture Rehabilitation.

INTRODUCTION: From a physiotherapist's point of view, helping patients recover from a distal end radius (DER) fracture is a big part of getting them back to their daily lives. This article highlights how important a structured, step-by-step rehabilitation program is for adults, whether their fracture was treated with a cast or with surgery. As physical therapists, our role is crucial in guiding this recovery, working closely with orthopedic surgeons. MATERIALS AND METHODS: We understand that there is not one perfect exercise plan for every patient, but starting exercises early is always bette

Beyond the pelvic ring: benchmarking multi-bone segmentation for ROI-Guided fracture detection.

Pelvic fractures are critical injuries associated with high mortality rates, yet accurate diagnosis using conventional radiography remains challenging due to complex anatomical overlaps. This study evaluates the impact of segmentation-guided preprocessing on fracture detection from pelvic X-rays by introducing a Multi-Bone Segmentation Method. Unlike traditional approaches that restrict analysis to the pelvic ring or treat the pelvis as a single region, our framework utilizes a transformer-enhanced U-Net to distinctly isolate nine individual pelvic bones, creating an anatomically precise input

Avascular Necrosis of the Hip: A Post COVID-19 Sequela.

A 60-year-old African American male presented to the hospital with seven months of progressively worsening left anterior hip pain with no known trauma. Two months after the pain onset, he underwent an x-ray of the pelvis with the lateral left hip, revealing dystrophic soft tissue calcification adjacent to the superolateral left acetabulum. Pain at this time was attributed to presumed sciatica vs arthritis. The patient underwent multimodal treatment for his pain without relief. In the month prior to the presentation, the patient also developed right hip pain. He then underwent a bilateral hip x
