[Reconstruction of large foot sole areas with microsurgical tissue transfers].

In 14 patients with microsurgically reconstructed areas of the sole videografic gait analysis was performed. The best results are reached after reconstruction of the forefoot. Loss of more than two thirds of the sole leads to functional impairment. For all reconstructed areas and all types of flaps gait is improved significantly by shoe adaptation: elevation of the shoe under the reconstructed part of the sole directs the main load to the opposite side of the foot and by this relieves the reconstructed area. Even bone corrections can reduce load on a circumscribed area.

Acute rupture of the tibialis anterior tendon: a case report.

Acute rupture of tendons on the dorsum of the foot is rare and the diagnosis can be difficult. We present the case of a 51-year-old man who sustained an acute rupture of the tibialis anterior tendon. Pain about the medial aspect of the midfoot and ambulation with a steppage gait were the keys to formulating a correct diagnosis. The tendon was repaired primarily 10 days after injury. At his final follow-up examination 6 months after surgery, the patient was asymptomatic and ambulated with a normal gait.

The regulation of rhythmic locomotion by motor cortical and dopaminergic inputs in the mouse striatum.

The striatum is a critical component of the basal ganglia and plays a central role in regulating motor initiation and action selection. How cortical and subcortical inputs converging at the striatum regulate locomotion remains unclear. By examining gait changes in head-fixed mice running on a treadmill, we found that mice were capable of performing forward, but not backward, rhythmic locomotion using their forelimbs when the striatum and motor cortex were inactivated. The striatal activity is critical for adjusting initially disorganized gait to efficient rhythmic locomotion during forward run…

Palinopsia in the Setting of Normal Pressure Hydrocephalus.

Normal pressure hydrocephalus (NPH) is characterized by pathologic ventriculomegaly with normal opening pressures on lumbar puncture. It commonly presents with a triad of gait disturbance, cognitive impairment, and urinary bladder detrusor dysfunction. Its pathogenesis is complex but is thought to arise in the setting of imbalanced cerebrospinal fluid (CSF) secretion and absorption. Given that intracranial pressure often remains normal in the setting of NPH, visual symptoms are quite uncommon. Here we present a case of a 70-year-old female with a subacute history of visual aberration described…

Non-traumatic Limping in the Child: A Pediatric Rheumatologist Perspective on Etiology, Clinical Evaluation, Laboratory Diagnosis, and Diagnostic Algorithms using Musculoskeletal Ultrasound (MSUS).

Limping refers to an asymmetrical gait that deviates from the typical gait pattern expected for a child of a certain age. In most children, limping is caused by a mild, self-limiting event, such as a contusion, strain, or sprain. However, a child's limping is always a pathological finding that poses a particular diagnostic challenge and necessitates a thorough assessment. The pediatrician must weigh a wide range of acute and chronic potential causes of a non-traumatic limp, including infection, neoplasia, and chronic inflammatory disorders. A thorough history and clinical examination will help…

Cerebellar Atrophy Associated with Primary Sjögren's Syndrome: Diagnosis, Therapy, and Virtual Reality Rehabilitation: A Case Report.

Cerebellar involvement in primary Sjögren's syndrome (pSS) is an uncommon condition, with only a limited number of cases described worldwide. A 43-year-old woman affected by cerebellar atrophy associated with pSS was referred to our center to undergo a cycle of physical rehabilitation therapy. Although motor symptoms started when the patient was 23 years of age, the underlying disease remained undiagnosed for several years. Neurological examination before rehabilitation revealed ataxic gait, dysmetria, nystagmus, and hypermetric saccades; the patients complained about unsteadiness while standi…

Motion characteristics of human roller skating.

In order to achieve a high speed of skating robot based on the leg structure - with passive wheels on even ground - the motion characteristics of human roller skating are studied in this paper. Utilizing a three dimensional motion capture system, the linear and turning gait are analyzed to express the motion characteristics of human roller skating. According to the observation and analysis, the normal linear gait can be divided into four phases and normal turning can be divided into three phases. The spin angle between the sagittal plane and the roller skate of the supporting leg is mainly gen…

Comparing normal walking and compensated walking: their stability and perturbation resistance. A simulation study.

People usually develop different kinds of compensated gait in response to local function deficits, such as muscle weakness, spasticity in specific muscle groups, or joint stiffness, in order to overcome the falling risk factors. Compensated walking has been analysed empirically in the impaired gait analysis area. However, the compensation could be identified spatially and temporally. The stability and perturbation resistance of compensated walking have not been analysed quantitatively. In this research, a biomimetic human walking simulator was employed to model one individual paraplegic subjec…

Early detection of Parkinson's disease using a multi area graph convolutional network.

Parkinson's disease is a neurological disorder, and early diagnosis is crucial for the treatment and quality of life of patients. Gait movement disorder is a significant manifestation of PD, and automated gait assessment is key to achieving automated detection of PD patients. With the development of deep learning, in order to improve the accuracy of early Parkinson's disease detection and enhance the robustness of motion recognition models, this study introduces an innovative deep learning approach, namely Multi-area Attention Spatiotemporal Directed Graph Convolutional Network (Ma-ST-DGN). Th…

Motor symptoms in autopsy-confirmed Alzheimer's disease increase the risk of progression to severe cognitive impairment.

INTRODUCTION: Motor symptoms may present throughout the Alzheimer's disease (AD) course. We evaluated the impact of motor symptoms on the ri … ression to severe cognitive impairment but not progression to severe NPSs or death. Presenting with gait disturbance and slowness but not falls or tremor was predictive of progression to severe cognitive impairment. Patients with motor symptoms had higher rates of Lewy body disease pathology on autopsy. DISCUSSION: Our findings suggest that the presen …

Association Between Disease Severity and Frailty in Patients with Aortic Stenosis.

Frailty is highly prevalent among patients with aortic stenosis (AS). Nonetheless, the impact of AS severity on frailty remains unclear. This study aimed to clarify the association between AS severity and frailty in this population. This retrospective study included consecutive patients with AS who were hospitalized as candidates for transcatheter aortic valve implantation (TAVI). The prevalence of frailty, N-terminal pro B-type natriuretic peptide (NT-proBNP) level, gait speed, and geriatric nutritional risk index (GNRI) were compared between patients with severe and those with very severe AS.…
