Clinical and genetic study in Chinese patients with Alexander disease.

Alexander disease is a rare progressive leukoencephalopathy inherited in an autosomal dominant manner. The infantile form is the most common, with onset before 2 years of age. The typical clinical signs include psychomotor retardation and regression, seizures, and megalencephaly. Juvenile and adult forms are also recognized. The neuropathology of Alexander disease is characterized by abundant presence of Rosenthal fibers in astrocytes in the brain. GFAP has been identified to be the only gene associated with Alexander disease since 2001. Only 1 patient with Alexander disease confirmed by genet

Relevance of an evidence-based ejaculation time cutoff point for neurobiological research of premature ejaculation.

The Diagnostic and Statistical Manual (DSM)-IV definition of premature ejaculation is not based on evidence-based studies. In particular, the absence of a well-defined quantitative measure of the intravaginal ejaculation latency time (IELT) makes the DSM definition inadequate. Therefore, the DSM-IV definition should be replaced by a medical definition that incorporates both quantitative as qualitative parameters of premature ejaculation. An evidence-based medical definition should include a cutoff point of the IELT at the 0.5 and 2.5 percentiles of the IELT distribution in the general male pop

Schizophrenia and the structure of language: the linguist's view.

Patients with schizophrenia often display unusual language impairments. This is a wide ranging critical review of the literature on language in schizophrenia since the 19th century. We survey schizophrenic language level by level, from phonetics through phonology, morphology, syntax, semantics, and pragmatics. There are at least two kinds of impairment (perhaps not fully distinct): thought disorder, or failure to maintain a discourse plan, and schizophasia, comprising various dysphasia-like impairments such as clanging, neologism, and unintelligible utterances. Thought disorder appears to be p

Emerging pathways for hereditary axonopathies.

Motor neurons are affected in a number of neurological diseases. Their unifying pathological signature is degeneration of extended projecting axons and loss of motor neurons in the prefrontal cortex and/or the spinal cord. Based on clinical criteria, hereditary forms have been traditionally divided into distinct entities, such as familial amyotrophic lateral sclerosis, hereditary motor neuropathy, spinal muscular atrophy, familial spinal paraplegia, and Charcot-Marie-Tooth disease type 2, also known as hereditary motor and sensory neuropathy II. Genetic research of the last decade has revealed

Neuro-ophthalmology of von Hippel-Lindau.

von Hippel-Lindau (VHL) disease is a multisystem inherited cancer syndrome with characteristic tumors and a known genetic basis. Patients with VHL develop, among other tumors, retinal capillary hemangiomas, central nervous system hemangioblastomas, renal clear cell carcinomas, and pheochromocytomas. Nearly all patients can be shown to have a mutation in the VHL gene, which is located on chromosome 3p25. Genetic abnormalities result in abnormal levels of pVHL, which in turn lead to the different tumors. Four distinct phenotypes of VHL disease have been identified based on the relative frequency

BCG--different strains, different vaccines?

After nearly a century of use, BCG vaccines continue to generate controversy and confusion. Their ability to prevent tuberculosis in studies has been inconsistent. When they have been protective, no clear mechanism of action has been established. Furthermore, the existence of different BCG strains has been described since the 1940s. These strains vary according to several laboratory properties, which may or may not translate into a discernible effect on vaccination. With recent genomic comparisons, it is now clear that different BCG vaccine strains have evolved and differ from each other and f

Renal cancer genetics: von Hippel Lindau and other syndromes.

There have been significant advances in our understanding of the genetic basis of renal carcinogenesis. In particular, research in the last five years has demonstrated a central role for the inactivation of the von Hippel-Lindau gene by mutation or hypermethylation in the formation of the conventional type of renal cell carcinoma. The von Hippel-Lindau syndrome is characterised by germ-line inactivating mutation whereas sporadic renal carcinoma is associated with somatic mutations. Tumour formation is accompanied by loss of the remaining wild-type allele. The biology of the von Hippel-Lindau g

Molecular and cellular defects in nephrogenic diabetes insipidus.

Nephrogenic diabetes insipidus is a rare genetic disorder characterized by insensitivity of the distal nephron to the antidiuretic effect of arginine vasopressin. Two different molecular defects underlying this disease have so far been identified. Mutations in the gene encoding the vasopressin type-2 receptor cause the X-chromosomal form of the disease, whereas mutations in the gene encoding the vasopressin-dependent water channel aquaporin-2 are responsible for the autosomal recessive, and (in some cases) an autosomal dominant type of the disease. Functional analysis of naturally occurring mu

Interpretation of constitutional cancer predisposition gene variants in 14 765 individuals in the 100 000 Genomes Project cancer arm: a retrospective cohort analysis.

BACKGROUND: Cancer predisposition due to constitutional (germline) genetic variants in high-risk or moderate-risk cancer predisposition genes presents clinical opportunities for risk mitigation. Focusing genetic testing only on patients who are most likely to be positive for germline variants might enhance the clinical utility of positive results, but this approach could fail to assess the rate and pattern of such variants in patients with cancer overall. We aimed to assess the frequency and nature of constitutional variants in cancer predisposition genes in patients with cancer in the UK heal

Familial Isolated Tetralogy of Fallot Associated With JAG1 Variant: Justifying Phenotype-Agnostic Genetic Workup for Critical Congenital Heart Disease.

Alagille syndrome (ALGS) is a rare, typically multisystem genetic disorder that impacts the liver, heart, eyes, vertebrae, and other areas of development. A clinical diagnosis can be established through defined clinical diagnostic criteria, while a molecular diagnosis requires the presence of a heterozygous pathogenic variant in JAG1 or NOTCH2 detected through molecular genetic testing. Disruption of either gene product results in disrupted Notch signaling pathways, resulting in the broad spectrum of clinical manifestations. The estimated incidence of ALGS is approximately 1 in 30,000-70,000 n

Genomic structural equation modelling of 12 cancers identifies a latent pan-cancer susceptibility factor and shared determinants of carcinogenesis.

BACKGROUND: Common cancers share germline susceptibility, yet the genetic architecture of a truly pan-cancer component of carcinogenesis remains incompletely resolved. METHODS: We applied genomic structural equation modelling (gSEM) to genome-wide association studies (GWAS) summary statistics for 12 cancers (>600,000 European-ancestry cases) to separate a latent pan-cancer genetic factor from cancer-specific components, and then performed a multivariate GWAS of that factor. We conducted comprehensive downstream genetic, predictive, and functional analyses to characterise the resulting pan-can

Compositional brain scores capture Alzheimer's disease-specific structural brain patterns along the disease continuum.

INTRODUCTION: Traditional multivariate methods for neuroimaging studies overlook the interdependent relationship between brain features. This study addresses this gap by analyzing relative brain volumetric patterns to capture how Alzheimer's disease (AD) and genetics influence brain structure along the disease continuum. METHODS: This study analyzed data from participants across the AD continuum from the Alzheimer's and Families (ALFA) and Alzheimer's Disease Neuroimaging Initiative (ADNI) studies. Compositional data analysis (CoDA) was exploited to examine relative brain volumetric variation

Integrating NLP and LLMs to discover biomarkers and mechanisms in Alzheimer's disease.

Alzheimer's disease (AD) is a progressive neurological condition characterized by cognitive decline, memory loss, and aberrant behaviour. It affects millions of people globally and is one of the main causes of dementia. The neurodegenerative condition known as AD has intricate, multifaceted mechanisms that make it difficult to comprehend and identify in its early stages. Conventional diagnostic techniques frequently fail to detect the disease in its early stages. By combining Natural Language Processing (NLP) and Large Language Models (LLMs), this research suggests a novel approach for identif

Genetic Predisposition to Benign Prostatic Hyperplasia: Where Do We Stand?

BACKGROUND AND OBJECTIVE: Genetic predisposition is a factor in 40-70% of cases of benign prostatic hyperplasia (BPH) and voiding symptoms. However, informal reviews summarizing genes and variants imparting genetic disposition to BPH are not yet available. METHODS: We conducted an informal narrative review of genes and variants associated with BPH or voiding symptoms in candidate gene studies, genome-wide association studies (GWAS), and Mendelian randomization studies. A literature search of PubMed was performed using the terms "BPH heritability", "LUTS heritability", "BPH risk variant", "LUT

Microevolution of Bartonella grahamii driven by geographic and host factors.

UNLABELLED: Bartonella grahamii is one of the most prevalent Bartonella species in wild rodents and has been associated with human cases of neuroretinitis. The structure and distribution of genomic diversity in natural B. grahamii is largely unexplored. Here, we have applied a comprehensive population genomic and phylogenomic analysis to 172 strains of B. grahamii to unravel the genetic differences and influencing factors that shape its populations. The findings reveal a remarkable genomic diversity within the species, primarily in the form of single- nucleotide polymorphisms. The open pangeno

The role of a mechanistic host in maintaining arctic rabies variant distributions: Assessment of functional genetic diversity in Alaskan red fox (Vulpes vulpes).

Populations are exposed to different types and strains of pathogens across heterogeneous landscapes, where local interactions between host and pathogen may present reciprocal selective forces leading to correlated patterns of spatial genetic structure. Understanding these coevolutionary patterns provides insight into mechanisms of disease spread and maintenance. Arctic rabies (AR) is a lethal disease with viral variants that occupy distinct geographic distributions across North America and Europe. Red fox (Vulpes vulpes) are a highly susceptible AR host, whose range overlaps both geographicall
