Neuro-Ophthalmic Emergencies.

Neuro-ophthalmic emergencies include optic nerve, central visual pathway, and ocular motility disorders that, if not identified and treated promptly, may lead to permanent vision loss, other significant morbidity, or mortality. This article provides a framework for approaching patients with neuro-ophthalmic symptoms and reviews the presentation, evaluation, and treatment of select emergent conditions that can cause them. Emergent causes of blurry vision, transient vision loss, papilledema, and diplopia, including giant cell arteritis, cardioembolic disease, and aggressive infection, are discus

Tympano-Mastoid Cholesterol Granuloma: Case Report and Review of the Literature.

Cholesterol granuloma (CG) is a rare condition histological consisting of a foreign body, giant cell reaction to cholesterol crystals and haemosiderin derived from the ruptured of the erythrocytes. A 25-year-old man came to our Department presenting signs and symptoms of tympano-mastoid cholesterol granuloma. He showed all the specific sign and symptoms of the disease. However, considering the lack of literature regarding TMCG, this study was performed with the aim of presenting the main characteristics of tympano-mastoid CG, describing the case report and reviewing the literature.

Vaping-induced diffuse alveolar hemorrhage.

There are growing reports of adverse health effects from e-cigarette use or vaping. The U.S. Centers for Disease Control and Prevention has reported 2409 cases and 52 deaths associated with e-cigarette use as of December 10, 2019. Vaping has been associated with acute eosinophilic pneumonia, organizing pneumonia, lipoid pneumonia, diffuse alveolar damage, acute respiratory distress syndrome, hypersensitivity pneumonia, and giant cell interstitial pneumonitis. Here we present a case of vaping-associated diffuse alveolar hemorrhage.

Giant mature teratoma in the mediastinum presenting with rapid growth.

Teratomas are primary germ-cell tumours in the mediastinum. Although they are generally slow-growing and asymptomatic, rapid growth causing life-threatening complications can occur. Sebaceous secretion, insulin production, chorionic gonadotropin secretion and pancreatic enzyme secretion are the presumptive causes of tumour progression. Only few cases of rapidly growing teratomas have been reported previously. Here, we present a case of a giant mature teratoma in the mediastinum that presented with rapid growth and compare the characteristics of this case with those of previous cases.

Isolated Ocular Motor Nerve Palsies.

An isolated ocular motor nerve palsy is defined as dysfunction of a single ocular motor nerve (oculomotor, trochlear, or abducens) with no associated or localizing neurologic signs or symptoms. When occurring in patients aged 50 or older, the most common cause is microvascular ischemia, but serious etiologies such as aneurysm, malignancy, and giant cell arteritis should always be considered. In this article, the authors review the clinical approach, anatomy, and differential diagnosis of each isolated ocular motor nerve palsy and discuss the clinical characteristics, pathophysiology, and treat

Tuberous sclerosis presenting with hemorrhagic stroke.

BACKGROUND: Incidence of intracerebral hemorrhage in patients with tuberous sclerosis is rare, and in most of the cases it is associated with either underlying cerebrovascular malformation or hemorrhage into the subependymal giant cell astrocytoma. CASE CHARACTERISTICS: A 2-year-old boy presented with a hemorrhagic stroke, and subsequently diagnosed as a case of tuberous sclerosis. OBSERVATION: Detailed work-up for stroke did not reveal any definite etiology. OUTCOME: Weakness gradually improved. Follow-up neuroimaging showed resolution of hemorrhage. MESSAGE: Clinician must be aware regar

Pigmented epidermal cyst with dense collection of melanin: A rare entity - Report of a case with review of the literature.

Epidermal cyst is a very common benign cystic lesion of the skin. It is usual to find ulceration of the lining epithelium, rupture of the cyst wall with chronic inflammation and foreign body giant cell reaction. But, it is very rare to see an epidermal cyst with marked accumulation of melanin pigment. Only a few cases of pigmented epidermal cyst with dense collection of melanin pigment have been published in the literature. Here, we are reporting a case of ruptured epidermal cyst with keratin granuloma formation and showing dense collection of melanin pigment.

[Transient visual loss: a practical approach].

Transient vision loss results from a variety of disorders including benign dry eye as well as vision- or life-threatening disorders such as giant cell arteritis. The goal of the initial evaluation is to determine the most likely mechanism based on the past medical history, characteristics of visual loss, and ophthalmologic and neurologic examinations. The ophthalmologist is often the first physician consulted and plays an essential role. Indeed, identification of the mechanism of transient visual loss allows the care-taker to properly decide on the work-up, its timing (emergency or otherwise),

Peripheral arterial disease in polymyalgia rheumatica.

Patients with polymyalgia rheumatica have been shown to have an increased risk of peripheral arterial disease on longitudinal follow-up. Possible explanations for this include premature atherosclerosis related to chronic inflammation, as with other inflammatory rheumatological conditions. Alternatively, polymyalgia rheumatica can be associated with vasculitis, even in the absence of clinical giant cell arteritis, and peripheral vascular disease may represent subclinical vasculitis. Further work is required to elucidate the reasons for this increased risk. Currently, it would remain reasonable

Recurrent retroperitoneal hemorrhage in a patient with tuberous sclerosis complex: a case report.

BACKGROUND: Tuberous sclerosis complex (TSC) is an autosomal dominant disorder. It is characterized by seizures, mental retardation and hamartomatous lesions, including facial angiofibroma, subependymal giant cell astrocytoma, cardiac rhabdomyoma and renal angiomyolipoma (AML). AMLs can bleed severely in the retroperitoneal space. CASE PRESENTATION: Herein, we present the case of a TSC patient presenting with recurrent severe episodes of retroperitoneal hemorrhage from AMLs successfully managed by angio-embolization. CONCLUSION: Transarterial embolization is effective in preventing and contr

Bone scintigraphy in common tumors with osteolytic components.

Osteolytic lesions are frequently encountered in clinical practice. Radionuclide bone scans with technetium-99m-labeled diphosphonates are often performed in the evaluation of both solitary and multiple osteolytic lesions. In this pictorial review, we critically evaluate the current role of bone scan in common osteolytic tumors including aneurysmal bone cyst, simple bone cyst, fibrous dysplasia, nonossifying fibroma, giant cell tumor, eosinophilic granuloma, enchondroma, chondrosarcoma, osteosarcoma, Ewing sarcoma, myeloma, and metastases. The merits and limitations of bone scanning are emphas

Therapy of neurological disorders in systemic vasculitis.

Significant progress has been made in our understanding of the underlying cellular and molecular mechanisms associated with vasculitis. Simultaneous involvement of the nervous system and systemic organs points to shared antigenic targets or immune mechanisms operative at both sites. Although specific immunotherapy is not available, the old and new immunomodulatory therapies have been instrumental in providing effective therapy in most of these chronic autoimmune disorders. The treatment of giant cell arteritis, neurosarcoidosis, Wegener's granulomatosis, systemic lupus erythematosus, polyarter

MRI of aggressive bone lesions of childhood.

MRI can yield valuable information about involvement of bone and soft tissue by aggressive bone lesions of childhood. The 18 lesions evaluated with MRI included osteosarcoma, Ewing's sarcoma, histiocytosis, osteomyelitis, osteoblastoma, giant cell tumor, aneurysmal bone cyst, ossifying fibroma, and fibrous dysplasia. In 12 children, where CT correlation was available, MRI provided equal or more information than CT. Our study shows that MRI should be the modality of choice in the evaluation of aggressive bone lesions of childhood, after conventional radiography.

Illustrated histopathologic classification criteria for selected vasculitis syndromes. American College of Rheumatology Subcommittee on Classification of Vasculitis.

We describe the histopathologic criteria for the diagnosis of 7 selected vasculitis syndromes: polyarteritis nodosa, Churg-Strauss syndrome, Wegener's granulomatosis, hypersensitivity vasculitis, Henoch-Schönlein purpura, giant cell (temporal) arteritis, and Takayasu arteritis. The criteria apply to the stereotypical cases in each category; they were formulated after a review of submitted biopsy material from 278 of the 1,000 patients entered into the American College of Rheumatology Vasculitis Study Registry, and from prior experience with the pathologic diagnosis of vasculitis in 1,079 nonre

[Polymyalgia rheumatica with drug eruption--an important differential diagnosis to dermatomyositis].

With regard to certain clinical features, polymyalgia rheumatica (PR) may closely resemble dermatomyositis. In contrast to dermatomyositis, PR usually does not show any cutaneous manifestations, although there might be seen concomitant giant cell arteritis. Furthermore, we do not find muscle enzymes in the serum with PR, and there is no histologic evidence of myositis. In rare cases, however, PR may be associated with cutaneous drug eruption and/or non-specific increase of muscle enzymes, which might cause considerable difficulties regarding the diagnostic differentiation from dermatomyositis.

[Soft tissue tumors of the hand].

This paper is based on the author's experience and a review of the most recent literature. First, the five most common tumors are discussed: ganglia, giant cell tumor, mucous cyst, hemangioma, and epithelial inclusion cyst. The less-common lesions are then discussed that present special problems in the hand, such as the glomus tumor or recurring digital fibromatosis of childhood among others. Lastly, the general guidelines for treatment of malignant tumors of the soft tissue tumors of the hand are presented, followed by a description of a selection of lesions (epitheloid sarcoma, subungual mal

Calcific tendinitis in unusual sites associated with cortical bone erosion.

Five cases of calcific tendinitis with radiographic evidence of cortical bone erosion are presented. All five cases arose at unusual sites: two at the pectoralis major insertion on the proximal humerus, two at the insertion of the gluteus maximus, and one at the insertion of the adductor magnus. Biopsy was performed in four cases and revealed areas of histiocytic infiltration, amorphous spherules of calcification, and areas of giant cell reaction extending into the cortex. To our knowledge, calcific tendinitis presenting with radiographic evidence of bone erosion has not been reported previous

[Granulomatous angiitis of the central nervous system].

It is reported on 3 observations of isolated granulomatous angiitis of the central nervous system. The case in question was in two 42-year-old men a granulomatous giant cell arteriitis with exclusive affection of the intracranial arteries. In a 33-year-old woman (case 3) there existed a granulomatous angiitis in generalised cerebrospinal granulomatous epitheloid-cellular inflammation (sarcoidosis) of the central nervous system with participation of bifurcal lymph nodes and of the lung. In all the three reports the diagnosis could be made only by autopsy. Clinic, pathomorphology, and diagnostic

Disorders of phagocytic function: ultrastructural aspects.

The electron microscopic changes in genetic and acquired disorders of granulocytes have been reviewed. In rare situations, such as the Chédiak-Higashi syndrome, there are bizarre giant lysosomes. In other conditions associated with abnormal phagocytic function, such as chronic granulomatous disease, there are no qualitative morphologic abnormalities. In chronic infection and in protein calorie malnutrition (kwashiorkor) granulocytes contain frequent Döhle bodies and cytoplasmic vacuolization. The development of new cytochemical methodology and the combination of electron microscopy with in vit
