Growth hormone therapy in children and adults.

Growth hormone (GH) is a polypeptide hormone, secreted by somatotropic cells of the anterior part of the hypophysis. Its application in therapy, first limited to GH deficient children, has now been widened to various other clinical conditions, not necessarily related to short stature. Clinical trials conducted in recent years have proved the safety of its administration in both children and adults. The efficacy of this form of therapy varies, according to different authors, from enthusiastic data to very critical opinions. For many pediatric diseases, such as GH deficiency or Turner syndrome,

Untreated growth hormone deficiency with extremely short stature, bone dysplasia, cleft lip--palate and severe mental retardation in a 26-year-old man with a de novo unbalanced translocation t(1;12)(q24;q24).

We report on a 26-year-old patient presenting with extremely short stature (height 72cm, weight 6.5kg, OFC 42.5cm), facial dysmorphism, cleft lip--palate, severe mental retardation and de novo 1q24.2--q25.2 and 12q24.31 interstitial deletion. He was the only child of non-consanguineous parents and his birth length was 43cm. He had severe feeding difficulties and required enteral nutrition until the age of 3 years. Standard cytogenetic analysis showed an apparently balanced de novo translocation t(1;12)(q24;q24). Endocrine studies at 11 years of age for severe growth retardation revealed multip

Fibroblast growth factor-23 mitigates hyperphosphatemia but accentuates calcitriol deficiency in chronic kidney disease.

Hyperphosphatemia, calcitriol deficiency, and secondary hyperparathyroidism (SHPT) are common complications of chronic kidney disease (CKD). Fibroblast growth factor-23 (FGF-23) is a novel phosphaturic hormone that also inhibits renal 1alpha-hydroxylase activity and thus may be involved in the pathogenesis of SHPT. Several hypotheses were tested: that FGF-23 increases as renal function declines; is linearly associated with serum phosphate levels; is associated with increased phosphaturia independent of parathyroid hormone (PTH); and is associated with decreased calcitriol levels independent of

Increased growth hormone response to apomorphine in Parkinson disease compared with multiple system atrophy.

BACKGROUND: Parkinson disease (PD) is often difficult to distinguish from parkinsonian syndromes of other causes in early stages of the disease. In search of a suitable endocrinologic challenge test, we investigated dopaminergic sensitivity in patients with de novo parkinsonian syndromes. OBJECTIVE: We measured the growth hormone (GH) response to a subthreshold dose of the dopamine 1-dopamine 2 receptor agonist apomorphine hydrochloride to differentiate parkinsonian syndromes from PD. PATIENTS AND METHODS: Seventeen patients with a clinical diagnosis of PD, 16 patients with a clinical diagno

Growth hormone and gonadotropin-releasing hormone analog therapy in haploinsufficiency of SHOX.

We report on GH (0.5 IU or 0.17 mg/kg/week) and GnRH analog (GnRHa, 60 microg/kg, every 4 weeks) therapy in SHOX haploinsufficiency. Case 1 was a 46,XY boy with microdeletion of the Y chromosomal pseudoautosomal region. At 7 years of age, he exhibited short stature (-3.9 SD) with a reduced growth rate (3.8 cm/year), short 4th metacarpals, and mild Madelung deformity. GH therapy resulted in a marked increase in height velocity (10.7 cm/year in the first year). Case 2 was a 46,XX girl with a heterozygous nonsense mutation of SHOX (C674T). At 6 years of age, she presented with short stature (-3.3

The effect of recombinant human growth hormone treatment on bone and mineral metabolism in haemodialysis patients.

BACKGROUND: Uraemia and chronical haemodialysis are associated with an abnormal growth hormone (GH)-insulin-like growth factor (IGF) axis which may contribute to malnutrition and renal bone disease. Short-term studies have shown a beneficial effect of treatment with recombinant human growth hormone (rhGH) on nutritional status in patients on haemodialysis. In the present study, we evaluated the effect of rhGH on bone and mineral metabolism. METHODS: Twenty chronic malnourished patients on haemodialysis took part in a double-blind, placebo controlled trial with subcutaneous injections of rhGH

Genetic diseases with rheumatic manifestations in children.

Many nonrheumatic diseases of childhood present with musculoskeletal abnormalities. A significant proportion of these disorders have a genetic basis, many involving defects in structural proteins of the connective tissue. Chief among these are collagen mutations resulting in spondyloepiphyseal dysplasias and Ehlers-Danlos syndrome, as well as fibrillin defects associated with Marfan's syndrome. A variety of other chromosomal anomalies are associated with musculoskeletal abnormalities, and may result from as yet unidentified connective tissue defects. In addition, metabolic diseases may result

Bioactivity of human growth hormone in serum: validation of an in vitro bioassay.

GH, in clinical practice, is determined by RIA, but RIA estimates may not accurately reflect serum GH bioactivity. The available measures of GH bioactivity lack either sensitivity, specificity, or a physiologically relevant end point. The objective of this research was to develop a physiologically relevant GH bioassay which would not only measure the bioactivity of purified GH preparations, but would also have sufficient sensitivity to measure GH bioactivity in human serum. The method consisted of incubating murine 3T3-F442A adipocytes in serum-free medium containing BSA, 14C-glucose, and incr

Hypothalamic hypopituitarism following external radiotherapy for tumours distant from the adenohypophysis.

Three cases of panhypopituitarism and five of isolated growth hormone deficiency which occurred following previous external irradiation of tumours distant from the adenohypophysis are described. The hypothalamic pituitary region received between 2800 and 12 000 rads in each case, 1-9 years before endocrine deficiency was recognized. Evidence is presented that the site of damage is in the region of the hypothalamus rather than the pituitary gland itself. Individuals treated with X-ray therapy in whom the hypothalamic-pituitary region is exposed to irradiation would appear to be at risk of devel

Urinary total hydroxyproline: creatinine ratio. Range of normal, and clinical application in British children.

The total hydroxyproline: creatinine ratio has been determined in random samples of urine collected from 1577 normal Bristol children and from children with hypothyroidism, growth hormone deficiency, coeliac disease, and rickets. The results in normal children had a profile similar to that of length velocity. When specific therapy was given to the children with growth failure there was a prompt increase in the total hydroxyproline: creatinine ratio, which occurred well before a growth spurt could be detected by anthropometry. The ratio could be a useful chemical adjunct to anthropometry and ra

The FGF-FGFR axis as an immune-metabolic rheostat in gastrointestinal inflammation and cancer.

Fibroblast growth factor (FGF)-fibroblast growth factor receptor (FGFR) signaling constitutes a fundamental regulatory network governing epithelial turnover, metabolic homeostasis, and immune modulation across the gastrointestinal tract. Although discrete FGF pathways have been intensively investigated in inflammatory bowel disease, hepatobiliary disorders, and gastrointestinal malignancies, how these signaling programs are coordinated across pathological contexts remains insufficiently resolved. In this review, we integrate evidence from human cohorts, experimental systems, and clinical studi

Distinct alterations of adiponectin, fibroblast growth factor 21 (FGF-21), and insulin-like growth factor binding protein 2 (IGFBP-2) link dysmetabolism with cognitive decline across the Alzheimer's disease spectrum.

INTRODUCTION: Metabolic disorders are risk factors for Alzheimer's disease (AD), although underlying mechanisms remain unclear. We investigated the relationship between peripheral metabolic markers - adiponectin, fibroblast growth factor 21 (FGF-21), and insulin-like growth factor binding protein 2 (IGFBP-2) - and AD. METHODS: Participants with subjective cognitive decline (SCD), mild cognitive impairment (MCI), AD, and cognitively healthy (CH) controls were from the Consortium for the Early Identification of Alzheimer's Disease-Quebec cohort (n = 287). Serum adiponectin, FGF-21, and IGFBP-2

Rsk4 Mediates Brain FGF19 Signaling to Constrain Diet-Induced Obesity in Mice.

UNLABELLED: Central administration of fibroblast growth factor-19 (FGF19) has been shown to reduce body weight in obese mice, but the underlying mechanisms remain largely unknown. Here, we show that ribosomal S6 kinase 4 (Rsk4) is required for FGF19 to counteract obesity. Rsk4 is predominantly expressed in hypothalamic neurons. While loss of Rsk4 has negligible effects on energy homeostasis under basal conditions, it exacerbates diet-induced obesity (DIO) in mice due to hyperphagia. These mice also exhibit glucose intolerance and insulin resistance. Reexpression of Rsk4 in mediobasal hypothala

Structurally-defined non-cationic docosahexaenoic acid based siRNA-micelles for safe and effective combinatorial glioblastoma therapy.

RNA interference (RNAi) has emerged as a highly promising therapeutic strategy for glioblastoma (GBM), yet its clinical efficacy heavily relies on the success of delivery systems. Traditional vehicles, including cationic polymers, lipids, and inorganic nanoparticles, frequently encounter critical challenges such as cationic toxicity, poor blood-brain barrier (BBB) penetration, or poorly-defined structures that hinder their clinical translation. To overcome these challenges, we developed a novel, structurally-defined non-cationic siRNA-micelle system based on siRNA-docosahexaenoic acid (DHA) co

Well-Child Care: Newborns and Infants.

Well-child care for newborns and infants (birth to 12 months) allows clinicians to identify any abnormalities in growth and development, administer vaccinations, and provide anticipatory guidance. History should focus on feeding, stooling, and sleeping. Trends in infant growth over time should be observed. Infants require a comprehensive physical examination to assess for normal development. Caregivers should be counseled on vaccination practices and their importance for disease prevention with adherence to standard schedules. Vaccine hesitancy should be addressed. Clinicians should review or

From Doubt to Development: Professional Journeys of Novice CBT Therapists.

Novice therapists often experience a complex interplay of self-doubt, emotional strain, and professional uncertainty as they transition from training to independent clinical practice. This study explored the lived experiences of novice cognitive behavioral therapy (CBT) therapists, focusing on the challenges of early practice and the strategies employed to support regulation and growth. Seven early-career CBT therapists participated in semi-structured interviews, and data were analyzed using Interpretative Phenomenological Analysis (IPA). Two overarching themes were identified: professional id
