The effect of Disopyramide and Propafenone on intracardiac conduction and refractory periods was tested by means of His bundle electrography and atrial stimulation. Disopyramide (1,7 mg/kg) was administered in 10 patients. There was no significant change of heart rate and conduction time within the atrium and the AV-node. The conduction velocity within the His-Purkinje system was significantly slowed by 13% of the control value. The ERP of the atrium as well as the ERP and FRP of the AV node were prolonged. Propafenone (1,7 mg/kg) was tested in 18 patients. The heart rate was significantly slo

Heterotaxy syndromes, right or left atrial isomerism, result from disruption of left-right axis determination and their manifestations include complicated heart defects. Recent studies in model organisms have revealed complex genetic pathways and several genes involved in this process. In affected humans, however, molecular studies have identified mutations in a small number of individuals, while in most the cause remains unknown. Furthermore, although family data suggest, autosomal recessive inheritance, such genes have not yet been identified. We have studied six members of a family, four ch

Extracorporeal membrane oxygenation (ECMO) is used as circulatory support or bridge to transplantation in patients with severe left ventricular (LV) dysfunction. Left heart decompression is needed to reduce pulmonary edema, prevent pulmonary hemorrhage, and reduce ventricular distention that may aid in recovery of function. We reviewed our experience from November 1993 to December 1997 with 10 patients having severe LV dysfunction (7 myocarditis, 3 dilated cardiomyopathy) who required circulatory support with ECMO and who underwent left heart decompression with blade and balloon atrial septost

Surgical techniques for tachyarrhythmias refractory to medical treatment are used with increasing frequency. Among 211 patients undergoing antiarrhythmic surgery 10 children (2 to 14 years old) were operated by electrophysiologically directed procedures. 7 patients suffered from WPW syndrome, 2 from focal atrial tachycardias and 1 from recurrent ventricular tachycardia following the repair of Fallot's tetralogy. In all cases preoperative electrophysiologic study and intraoperative mapping preceded operative ablation. Surgical treatment consisted of interruption of the bundle of Kent (3 right-s

Transmembrane action potentials (APs) of electrically paced right-atrial tissue obtained from 38 patients of corrective open-heart surgery were analyzed. Two types of APs could be found. The APs of 20 patients (group I) were similar to those of other laboratory mammals. The average resting potential (RP) and the amplitude and maximum rate of rise of phase 0 of APs (Vmax) were -75 mV, 86 mV, and 152 V/sec, respectively, only the repolarization phase being more prolonged than that of other mammalian APs. Epinephrine (5.8 X 10(-6) mole/liter) increased the amplitude of APs and prolonged the plate

BACKGROUND: A patent foramen ovale (PFO) is a common congenital cardiac condition characterized by a persistent communication between the right and left atria, which can facilitate a right-to-left shunt (RLS). This study aimed to determine the diagnostic accuracy, reproducibility, and prognostic value of PFO shunt volume quantified by three-dimensional transesophageal echocardiography (3D-TEE) combined with right-heart contrast echocardiography.
METHODS: In this prospective study, 100 patients with suspected PFO were prospectively enrolled and allocated in a 7:3 ratio to a training cohort (n=

Pulmonary embolism (PE) is a serious and potentially life-threatening medical condition that arises when a blood clot, typically originating in the deep veins of the legs or pelvis, travels through the bloodstream and lodges in the pulmonary arteries. This obstruction can impede blood flow to the lungs, leading to complications. PEs are a critical manifestation of venous thromboembolism, a condition characterized by the formation of blood clots within veins. The consequences of PE can vary, ranging from mild respiratory distress to severe respiratory failure or cardiac arrest, depending on the

OBJECTIVE: Postoperative atrial fibrillation is one of the most frequent complications of cardiac surgery. We developed a novel biodegradable gel pad consisting of biopolymers that directly attach to the myocardium by electrostatic interaction. The present study examines the feasibility and effectiveness of low-energy internal cardioversion using these pads.
METHODS: The hearts of 6 pigs were exposed through a median sternotomy under general anesthesia, and 2 monopolar pacing wires were placed on the left pulmonary veins (chest open group). Two biodegradable cardioversion gel pads were placed

Atrial fibrillation (AF) is the most common cardiac arrhythmia, the prevalence of which is increasing with the aging of the population. Because of its clinical importance and the lack of highly satisfactory management approaches, AF is the subject of active clinical and research efforts. This paper reviews recent and on-going developments in pharmacological and non-drug management of AF. The ideal therapeutic goal for AF is the production and maintenance of sinus rhythm. Comparative studies suggest that available class I and III drugs have comparable and modest efficacy for sinus rhythm mainte
