Ultrasound in developmental dysplasia of the hip: A screening study in Sardinian newborns.

INTRODUCTION: Developmental dysplasia of the hip (DDH) is the most frequent inborn deformity of the locomotor apparatus. Ultrasound screening is frequently used to identify DDH in view of the brevity of the preclinical period during which diagnosis is possible. Appropriate therapeutic intervention during this period can positively affects the evolution of the disorder. METHODS: An unselected population of 1158 Sardinian neonates underwent ultrasound examinations of both hips, and findings were classified according to the method described by Graf. Patients were assessed by means of retrospecti…

Prolonged treatment with the Pavlik harness in infants with developmental dysplasia of the hip.

We prospectively studied the benefits and risks of prolonged treatment with the Pavlik harness in infants with idiopathic developmental dysplasia of the hip. Bracing was continued as long as abduction improved. It was started at a mean age of four months (1 to 6.9). Outcome measures were the number of successful reductions, the time to reduction, the acetabular index and evidence of avascular necrosis at follow-up at one year. In 50 infants with 62 subluxated and dislocated hips (42 Graf type III and 20 type IV), 37 were reduced successfully with a mean time to reduction of 13.4 weeks (sd 6.8)…

Surgical advances in periacetabular osteotomy for treatment of hip dysplasia in adults.

INTRODUCTION: Hip dysplasia is characterized by an excessively oblique and shallow acetabulum with insufficient coverage of the femoral head. It is a known cause of pain and the development of early osteoarthritis in young adults. The periacetabular osteotomy is the joint-preserving treatment of choice in young adults with symptomatic hip dysplasia. The surgical aim of this extensive procedure is to reorient the acetabulum to improve coverage and eliminate the pathological hip joint mechanics. Intraoperative assessment of the achieved acetabular reorientation is therefore crucial. The "classic…

Comparison of morphologic and dynamic US methods in examination of the newborn hip.

PURPOSE: Comparison of morphologic and dynamic methods of hip ultrasonography (US) to differentiate normal from abnormal findings in the diagnosis of developmental dysplasia of the hip (DDH). MATERIALS AND METHODS: A total of 6,800 hips in 3,400 infants were examined with US, using the morphological method of Graf, and the dynamic method of Harcke. RESULTS: According to the Graf classification 81.47% of infants had Type 1 (normal hip), 10% Type 2a (physiologic immaturity), 2.44% Type 2b (acetabular dysplasia), 1.05% Type 2c (critical zone hip), 2.89% Type 3 (mildly dislocated), and 2.10% had…

Anterior dynamic ultrasound of the infant hip: evaluation of investigator dependence.

BACKGROUND: Anterior dynamic ultrasound of the infant hip has been shown to be a valuable tool for detection of developmental dysplasia of the infant hip, but to what extent the results from this method depends on the investigator is unknown. PURPOSE: To study to what extent the proportion of positive findings at anterior dynamic ultrasound investigation of infant hips varies with the observer. MATERIAL AND METHODS: Information on all ultrasound investigations of infant hips at a county hospital between January 2004 and August 2007 was evaluated. Proportion of findings for different investig…

[Clinical anatomical study on high congenital dislocation of hip in adults].

OBJECTIVE: To investigate the morphological anatomical abnormalities of high congenital dislocation of hip in adults and provide anatomical basis for the total hip arthroplasty (THA). METHODS: From May 1997 to July 2008, 49 patients (57 hips) with high congenital dislocation of hip (Hartofilakidis type III) were treated. There were 6 males and 43 females with an average age of 29.4 years old (18-56 years old). The locations were left in 24 hips and right in 33 hips. The morphological parameters (including femoral length, isthmus, height of femoral head center, neck-shaft angle, medial head of…

Dislocation of total hip replacement in patients with fractures of the femoral neck.

BACKGROUND: Total hip replacement is increasingly used in active, relatively healthy elderly patients with fractures of the femoral neck. Dislocation of the prosthesis is a severe complication, and there is still controversy regarding the optimal surgical approach and its influence on stability. We analyzed factors influencing the stability of the total hip replacement, paying special attention to the surgical approach. PATIENTS AND METHODS: We included 713 consecutive hips in a series of 698 patients (573 females) who had undergone a primary total hip replacement (n = 311) for a non-patholog…

Quantifying degree of difficulty in hip - resurfacing of pistol-grip deformity.

This study used computer simulation to endeavor to quantify the relative degree of difficulty of resurfacing femora with pistol-grip deformities compared to relatively normal femora. Computer models of five pistol-grip femora and one relatively normal femur were computed from computed tomography (CT) scans of patients who had undergone computer-assisted hip resurfacing. A computer simulation of positioning the femoral resurfacing component on the femur was performed to count the number of acceptable configurations of the component on the femur. A high number of acceptable configurations implie…

[The role of the ossific nucleus in the treatment of developmental dislocation of the hip joint].

The aim of the study was to compare long-term radiological results and appearance of avascular necrosis in hips with and without ossific nucleus of femoral head at the beginning of treatment. We retrospectively reviewed radiographs of 155 hips in 107 children treated for developmental dislocation of the hip. The age of children at the onset of treatment was 14.2 months. Minimal age at last follow-up was 14 years. Statistical analysis showed no correlation between presence or size of ossific nucleus and avascular necrosis of femoral head or final radiological outcomes. We do not recommend delay…

Necrotic and inflammatory changes in metal-on-metal resurfacing hip arthroplasties.

BACKGROUND: Necrosis and inflammation in peri-implant soft tissues have been described in failed second-generation metal-on-metal (MoM) resurfacing hip arthroplasties and in the pseudotumors associated with these implants. The precise frequency and significance of these tissue changes is unknown. METHOD: We analyzed morphological and immunophenotypic changes in the periprosthetic soft tissues and femoral heads of 52 revised MoM arthroplasties (fracture in 21, pseudotumor in 13, component loosening in 9, and other causes in 9 cases). RESULTS: Substantial necrosis was observed in the periprost…

Black hip, fracture neck of femur and scoliosis: a case of ochronosis.

BACKGROUND: Ochronosis is a very rare disorder with an incidence of 2 to 5 per million live births. It is a genetic disorder with autosomal recessive inheritance. It is characterized by dark brown pigmentation of cartilage, capsules of joints and other soft tissues. Ochronosis usually presents as ochronotic arthropathy and spondylosis. PURPOSE: This article describes a very rare presentation of scoliosis and fracture of the neck of the femur in a patient with ochronosis. METHODS: A 65-year-old woman with left-sided scoliosis presented to us following a fall with fracture of the neck of the f

A painful metal-on-metal total hip arthroplasty: a diagnostic dilemma.

Infection, loosening, osteolysis, or other causes can lead to the development of pain about a previously well-functioning total hip arthroplasty. An inflammatory reaction unique to metal on metal arthroplasty can lead to a painful total hip. A synovial biopsy is needed to make this specific diagnosis, and included in the differential diagnosis is infection. The workup of infection includes obtaining a C-reactive protein and erythrocyte sedimentation rate. Elevations of both the C-reactive protein and erythrocyte sedimentation rate are felt to indicate possible infection. This case report descr…

Differences in innervation and innervated neurons between hip and inguinal skin.

Pain originating from the hip may be referred to the groin and anterior thigh. We investigated sensory dorsal root ganglion neurons innervating the hip and the inguinal skin in rats using retrograde neurotransport and immunohistochemistry. A retrograde neurotracer Fluoro-Gold was injected into the left hip or inguinal skin of rats. Seven days later, we harvested bilateral dorsal root ganglions and counted the number of Fluoro-Gold-labeled neurons positive for calcitonin gene-related peptide, a marker of nerve growth factor-dependent neurons, or isolectin B4, a marker of glial cell line-derived…
