Cytomegalovirus in Bronchoalveolar Lavage Fluid: A Predictor of Outcome in Immunocompromised Patients.

Cytomegalovirus (CMV) is a prevalent herpesvirus that remains typically asymptomatic in immunocompetent individuals but can cause severe, potentially life-threatening disease in immunocompromised patients. This study assessed the impact of CMV-associated lower respiratory tract infections (LRTIs), diagnosed by polymerase chain reaction (PCR) testing of bronchoalveolar lavage fluid (BALF), focusing on coinfections and clinical outcomes. Between 2009 and 2017, a total of 2666 visits involving 1301 immunocompromised patients with suspected LRTI who underwent diagnostic bronchoalveolar lavage (BAL

Evaluation of acellular dermal matrix efficacy in immunocompromised patients' wounds: a retrospective matched-cohort study.

OBJECTIVE: The efficacy of acellular dermal matrix (ADM) in immunocompromised patients' wounds has been little studied. The present study aimed to determine the wound healing effects of ADM in immunocompromised patients compared with patients with normal immune function. METHOD: Between June 2015 and July 2020, medical records of patients treated with ADM for wounds were scrutinised in this retrospective cohort study. Patients exposed to immunosuppression were age- and wound area-matched to patients without immunosuppression. All wounds were treated with ADM without further surgical treatment

Zika Virus Induces Progressive Morphological and Fibrotic Alterations in the Submandibular-Sublingual Salivary Complex of Immunosuppressed Mice.

To perform a morphological, histochemical, morphometric, and fractal analysis of the submandibular-sublingual salivary complex of immunosuppressed male Balb/c mice experimentally infected with Zika virus (ZIKV), aiming to characterize infection-related structural alterations. A total of 110 male Balb/c mice were immunosuppressed with dexamethasone and inoculated intraperitoneally with ZIKV. Animals were allocated into control and infected groups and euthanized at 14, 21, 28, 35, and 42 days post-infection (dpi). Body weight, salivary gland weight, and organosomatic index were assessed. The gla

Coronavirus disease 2019-associated mucormycosis and cytomegalovirus infection: a case report.

BACKGROUND: Mucormycosis is a severe opportunistic fungal infection that has become a notable complication in patients recovering from coronavirus disease 2019, particularly in individuals with preexisting diabetes mellitus or a history of corticosteroid administration. Coinfection with cytomegalovirus in immunocompromised patients with coronavirus disease 2019 is increasingly documented yet remains insufficiently acknowledged. This case is significant owing to the uncommon simultaneous occurrence of cutaneous mucormycosis and cytomegalovirus infection in a post-coronavirus disease patient, un

Infection in VEXAS Patients Undergoing Allogeneic Stem Cell Transplantation.

BACKGROUND: VEXAS is a relatively newly described immunocompromising condition with hematopoietic stem cell transplant increasingly being used. Little is known regarding infections in this high-risk population. METHODS: We conducted a retrospective review of ten adult patients diagnosed with VEXAS who underwent HSCT in the Mayo Clinic Rochester and the Arizona medical record system. RESULTS: Nine out of ten individuals developed bacterial infection with bacteremia found in eight individuals despite being on prophylaxis at the time of infection. Six of the ten individuals were found to have v

Contrasting outcomes of visceral leishmaniasis in two patients with membranous nephropathy receiving immunosuppressive therapy: a case report.

BACKGROUND: Visceral leishmaniasis (VL) often presents with an atypical and fulminant course in immunocompromised hosts, and treatment faces multiple challenges including drug toxicity, immunosuppressive status, and complications. CASE PRESENTATION: Here we report two cases of VL in patients with membranous nephropathy(MN) who developed the infection after long-term immunosuppressive therapy. Both patients presented with fever, pancytopenia, and markedly elevated inflammatory markers. Case 1 had relapsed VL complicated by pre-existing renal insufficiency, experienced disease relapse after ant…

Investigation of Vaccine-Induced Immune Response After Vaccination Against Respiratory Viruses in Immunocompromised Patients With or Without Hemato-Oncological Diseases (RESPONSE): Protocol for a Prospective Cohort Study.

BACKGROUND: Infections with respiratory viruses such as SARS-CoV-2 and influenza are significant international public health concerns. While patients with cancer remain the most vulnerable group, they show poor vaccine response in general. Immunological data in this population are limited and mainly focus on serological parameters. However, in these patients, cellular, and especially T-cell, responses often seem to be induced more reliably than humoral responses. OBJECTIVE: To gain further insights into vaccine-induced immunity, the RESPONSE study will analyze the effect of early and late boo

The curious case of aplastic anemia: a fatal triple co-infection with Aspergillus, Mucorales, and Nocardia.

BACKGROUND: Concurrent opportunistic infections are rare but can lead to life-threatening complications in patients with hematologic disorders. Immunosuppression, particularly severe neutropenia, significantly increases the risk of invasive fungal and bacterial infections. We report a 34-year-old female with chronic hepatitis B initially presented with pancytopenia. Subsequent bone marrow evaluation revealed aplastic anemia, and she subsequently received immunosuppressive therapy. During hospitalization, she developed multiple infectious episodes, including fever, pulmonary nodular lesions, an

Possible role of hyperimmunoglobulin in reducing the risk of maternal-fetal transmission of cytomegalovirus in the valacyclovir era: a case series.

INTRODUCTION: Valacyclovir is the only treatment option during pregnancy which has been demonstrated to be effective within a randomized clinical trial for prevention of transplacental cytomegalovirus (CMV) transmission. However, the use of high dose intravenous hyperimmunoglobulin (HIG) could reduce the rate of vertical transmission according to some observational studies. CASES PRESENTATION: We report three peculiar cases in which high dose HIG was administered in substitution to or in addition to valacyclovir to reduce the risk of transplacental transmission of CMV. Two were immunocompromi

Identifying HHV-6 Induced Cytopathic Effects in Bronchoalveolar Lavage Fluid Preparations to Enhance Care in Immunocompromised Patients.

Human Herpesvirus 6 (HHV-6) is a ubiquitous virus that remains latent after primary infection but can reactivate in immunocompromised patients, leading to severe complications. Early detection of HHV-6 in respiratory samples is critical for timely intervention. We describe a 61-year-old immunocompromised woman who presented with pneumonitis and encephalopathy. Initial testing confirmed COVID-19, and worsening respiratory symptoms prompted bronchoalveolar lavage (BAL). Cytologic analysis of the BAL revealed epithelial cells with viral cytopathic effects, including multinucleation, chromatin mar

Diagnostic performance of Pneumonia multiplex PCR in critically ill immunocompromised patients.

BACKGROUND: Admissions of immunocompromised patients to intensive care units (ICUs) are on the increase. The main reason for admission is acute respiratory failure, predominantly of infectious origin. In such circumstances, early and appropriate antibiotic therapy guarantees a better prognosis. Rapid diagnostic techniques such as multiplex polymerase chain reaction (PCR) have shown their value in both diagnosis and treatment in immunocompetent patients. To date, little data are available on immunocompromised patients. METHODS: In this retrospective, single-center study, we analyzed data from

Regional analgesia catheter-related infections and the effectiveness of antibiotic prophylaxis in immunocompromised patients: A retrospective multicenter registry analysis.

BACKGROUND: The risk of regional analgesia catheter-related infections in immunocompromised patients remains uncertain. We therefore tested the hypotheses that catheter-related infections appear earlier and are more severe, and that antibiotic prophylaxis is more effective in immunocompromised than immunocompetent patients. METHODS: Data were extracted from the Network for Safety in Regional Anesthesia and Acute Pain Therapy (net-ra) registry from 2007 to 2022. We used multivariable cox and ordinal regression to assess the effect of immune function and antibiotic prophylaxis indicated by surg

Successive SARS-CoV-2 mutations in a highly susceptible host: A case report of persistent viremia and fatality.

A 71-year-old female with post-treatment follicular lymphoma developed COVID-19 with persistent viremia. Despite the initiation of remdesivir on day 23, she remained SARS-CoV-2 PCR-positive and ultimately succumbed to respiratory failure on day 79. Whole-genome analysis of RNA extracted from a post-mortem nasopharyngeal swab identified multiple mutations, including P323L/G671S, which enhances viral replication, and V792I, associated with remdesivir resistance. This case highlights the challenges of managing persistent SARS-CoV-2 infection in immunocompromised patients and underscores the impac

Azole drugs have differential efficacy in varied models of immunosuppression in larval zebrafish hosts.

AIMS: Fungal infections are an increasing cause of mortality in immunosuppressed human patients, and antifungal drug treatments are still ineffective in a significant percentage of cases. A gap in our understanding of antifungal drug treatment is whether different drugs have differential efficacy in hosts that are experiencing different forms of immunosuppression. MATERIALS & METHODS: Here, we used a larval zebrafish vertebrate host model of Aspergillus fumigatus infection to compare the efficacy of four different triazole drugs in five different immunosuppressed or immunodeficient conditions

Complex Differential Diagnosis Between Cerebral Toxoplasmosis and Primary CNS Lymphoma in Immunocompromised Patients: The Value of Brain Biopsy.

We report the case of a 24-year-old woman who received immunosuppressive treatment for systemic lupus erythematosus. In her past medical history, she had been treated for disseminated toxoplasmosis (ocular and cardiac). Four months later, she was hospitalized and treated for a presentation suggestive of cerebral toxoplasmosis. The diagnosis of primary central nervous system lymphoma (PCNSL) was finally made using stereotactic brain biopsy; however, the evolution was rapidly fatal within 6 days after admission. We discuss the challenge of differential diagnosis between cerebral toxoplasmosis an
