Fungal infections in the recipients of solid organ transplantation.

The advent of effective antibacterial and antiviral prophylatic and therapeutic strategies has led to the emergence of opportunistic mycoses as a principal cause of infection-related mortality in organ transplant recipients. Candida and Aspergillus species have accounted for most invasive fungal infections in organ transplant recipients. Epidemiologic trends within the last decade, however, are notable for the emergence of mycelial fungi other than Aspergillus as increasingly important pathogens in these patients. This article reviews the epidemiology, clinical manifestations, pathogenetic bas

Childhood central nervous system viral infections and adult schizophrenia.

OBJECTIVE: An earlier Finnish cohort study suggested that childhood viral CNS infections are associated with a fivefold increased odds of developing schizophrenia in adulthood. The authors sought to replicate this finding. METHOD: From the archives of the Department of Virology of the National Public Health Institute in Finland, 320 individuals born between 1960 and 1976 who had suffered virologically confirmed CNS infections before their 15th birthdays were identified. Of the infections, 202 had been caused by enteroviruses. The sample was followed up in the 1969-2000 records of the National

[Are domestic Cryptosporidium and Giardia infections in Norway underdiagnosed?].

BACKGROUND: Cryptosporidium and Giardia are recognised as common causes of waterborne disease in several countries. In order to describe investigative practices for these protozoan parasites in Norway, we surveyed medical microbiology laboratories nationwide for faecal screening policies and methods used for detection of Cryptosporidium and Giardia. MATERIAL AND METHODS: All medical microbiology laboratories in Norway received questionnaires on laboratory methods, indications for screening, and numbers of samples investigated over the 1998-2002 period. RESULTS: Of the 22 laboratories that re

Preventing bacterial infections and antimicrobial resistance in dialysis patients.

Antimicrobial use, in concert with patient-to-patient transmission of resistant strains, has caused a rapid increase in the prevalence of antimicrobial resistance in recent years. This increase is a particular threat to dialysis patients, who often have been in the forefront of the epidemic of resistance. In this report, which was written in collaboration between the American Society of Nephrology and the Centers for Disease Control and Prevention and has been endorsed by the Executive Council of the Infectious Diseases Society of America, we review and summarize existing clinical practice gui

A study of orofacial bacterial infections in elderly Nigerians.

This retrospective study was designed to determine the pattern of occurrence, and principles of management of orofacial bacterial infections in elderly Nigerians. Ninety-two patients aged 55-90 years (mean +/- SD 62.4 +/- 7.5 years), were admitted into the study (male:female ratio 1.1:1). The origin of orofacial infection was predominantly odontogenic and the lower face was three times more commonly involved. The percentage of patients with underlying systemic disease was 16.2%. Patients with underlying disease had a 3.5 times greater risk of developing orofacial infections. Similarly, a signi

Immunoregulation of CNS autoimmunity by helminth and mycobacterial infections.

The 'hygiene hypothesis' has been proposed to explain apparent increases in autoimmune disease and allergy in areas of the world with improved health care and sanitation. This hypothesis proposes that the lack of serious childhood infections impairs development of an appropriately educated immune response. Imbalance of Th1 and Th2 responses and lack of regulatory T-cell populations are two of many proposed potential mechanisms for immune failures such as autoimmunity and allergy. We summarize the literature evidence for the influence of infectious organisms on autoimmunity with focus on helmin

Ventriculostomy-related infections: a critical review of the literature.

OBJECTIVE: To provide a critical evaluation of the published literature describing risk factors for ventriculostomy-related infections (VRIs) and the efficacy of prophylactic catheter exchange. METHODS: A MEDLINE literature search was performed, and data were extracted from studies published from 1941 through 2001. RESULTS: Published criteria for diagnosing VRIs are highly variable. Intraventricular hemorrhage, subarachnoid hemorrhage, cranial fracture with cerebrospinal fluid leak, craniotomy, systemic infections, and catheter irrigation all predispose patients to the development of VRIs. Serologic and molecular diagnosis of zoonotic larval cestode infections.

Neurocysticercosis, cystic echinococcosis and alveolar echinococcosis are the three major zoonotic larval cestodiases worldwide and threatening human life. Early therapeutic treatment based on early differential diagnosis at the early stage of infection and epidemiological surveillance with the expectation for control and prevention are the keys for improvement of quality of human life. Recent advances in (1). differential serodiagnosis of these three diseases, (2). molecular identification of human taeniid species and (3). polymorphism of mitochodrial DNA highly informative for future molecul

Antimicrobial resistance of Staphylococcus aureus isolated from skin infections.

The antimicrobial susceptibility of 229 strains of Staphylococcus aureus isolated from various skin infections was determined against 22 antimicrobial agents by the agar dilution method. The clinical isolates were most sensitive to vancomycin, teicoplanin, mupirocin and fusidic acid. No strains were resistant to vancomycin or teicoplanin. Three strains were highly resistant (MIC > or =100 mg/l) to mupirocin and eight strains to fusidic acid. The MIC(50) of all antimicrobials, except for gentamicin, were below 3.13 mg/l. The incidence of resistance to penicillin, cephalosporins and clindamycin

Novel agents for resistant Gram-positive infections--a review.

Gram-positive infections have increased in recent years, particularly those that are of nosocomial origin, leading to a broad use of agents with activity against these pathogens. Concomitantly, antimicrobial resistance of these pathogens also became widespread. Among the most common Gram-positive resistant pathogens are: Streptococcus pneumoniae, resistant to penicillin and macrolides, methicillin-resistant Staphylococcus aureus (MRSA), glycopeptide-intermediately-resistant S. aureus (GISA), methicillin-resistant S. epidermidis, glycopeptide-resistant enterococci and vancomycin-resistant enter

Central venous catheter infections in patients with acute leukemia.

BACKGROUND: Central venous catheters (CVCs) have become an essential tool for an appropriate management of patients with acute leukemia. Infectious complications are a major concern in patients treated for acute leukemia. Although CVC-related infections are considered to be a major source of infections during neutropenia (<500/microl), data regarding the incidence of CVC-related infections are rare in acute leukemia. PATIENTS AND METHODS: We analyzed nontunneled CVCs in 58 patients with acute leukemia (22 men/36 women) in 119 chemotherapy cycles from April 1996 to January 1998 in a prospectiv
