A 41-year-old female nurse's aide presented with a recent history of viral hepatitis B. Approximately 1 month after her liver functions had normalized she developed symptoms of optic neuritis with painful loss of vision and optic disc edema. Her laboratory studies revealed evidence of systemic vasculitis and glomerulonephritis. A discussion is presented of the patient's clinical course and a brief history of the systemic effects of acute hepatitis B infection (HBV); the pathophysiology for HBV-induced autoimmune optic neuritis (ON), management guidelines, and differential diagnoses are discuss

Perinatally acquired bacterial neonatal sepsis is a low incidence, high risk disease with a relatively benign treatment. Accurate diagnosis is difficult because there is no definitive diagnostic test; even blood cultures have an unacceptably low sensitivity. Therefore, the clinician must accept that a number of neonates who do not have the disease will have treatment initiated for sepsis. In order to treat rapidly all infants with sepsis and to minimize therapy for those without infection, historical, clinical, and laboratory data can be used together in a management approach to achieve optima

Parvovirus infection was confirmed by fluorescent antibody staining or viral culturing in 137 (22%) of 615 necropsy accessions from dogs at the Missouri Veterinary Medical Diagnostic Laboratory from Jan 1, 1987 through Sept 30, 1988. Septicemic colibacillosis was diagnosed in 88 (90%) of the 98 canine parvovirus-positive accessions in which liver or lung was cultured bacteriologically. Pulmonary edema or alveolitis similar to that seen in the human adult respiratory distress syndrome was observed in 63 (69%) of the 91 canine parvovirus-positive accessions in which the lungs were examined histo

The purpose of this study was to determine if women who self-reported symptoms of chronic vaginal infection might be at risk for microscopic condyloma and, therefore, in need of colposcopic examinations. The hypothesis tested was that women who report yeast-type symptom chronicity and who have yeast-negative laboratory studies will have a significant incidence of microscopic vulvar condyloma. The hypothesis was supported by the results. Findings show a need to broaden the use of the colposcope to detect condyloma in women with a chronicity of unexplained yeast-type symptoms.

Clinical and laboratory examinations of 250 patients with chronic dermatoses (eczema, neurodermatitis, psoriasis, pemphigus, etc.) have revealed secondary candidal involvement of the skin and mucosa in 121 (46.6%). C. albicans, often associated with bacillary (33.3%) or bacterial (7.3%) microflora, predominated among the yeast-like fungi isolated from the patients. These data point to the necessity of prescribing, besides the antimycotic agents, a complex of pathogenetic therapy including immunomodulators, in order to cut down the terms of treatment and prevent secondary candidal infection in

Leptospirosis, an acute generalized infection transmitted to humans from wild or domestic mammals, is rarely found in urban areas. We report here our experience with five cases of confirmed leptospirosis in the urban area of Mexico City. All five patients had epidemiologic risk factors, fever and a multisystemic illness, and two underwent laparotomy for acute abdominal syndrome. One patient died and four recovered completely. Problems in clinical and laboratory diagnosis are discussed. Leptospirosis should be suspected in patients with fever, jaundice and multiorgan involvement, and a proper e

One hundred and sixteen cases of adult patients hospitalized for shigellosis have been reviewed. The most common presentation included diarrhea (most frequently watery), fever, and abdominal pain, while dysentery has been less frequent. S. flexneri infection, as compared to S. sonnei, was more common in elderly patients and in males, and was characterized by a more prolonged clinical course. Abnormalities of serum electrolytes and hepatic enzymes were the most common laboratory complications, most often seen in elderly patients. Most isolates exhibited susceptibility to the common antimicrobia

The numerous viral skin diseases that affect children present a diagnostic challenge to the clinician. Most of these diseases may be conveniently grouped according to the clinical appearance of the exanthem as maculopapular, petechial, papular, or vesicular. In some situations, viral infection may be difficult to differentiate clinically from nonviral disease; thus, extensive laboratory evaluation is sometimes necessary to pinpoint the virus involved. Nonviral disease should be considered in the differential diagnosis of a patient with skin eruptions, especially of the maculopapular type. Comm

A case of human proliferative sparganosis, the first in the southern hemisphere, is reported. The patient had multiple nodular, papular, acne-like lesions, gynecomastia, and large subcutaneous abscesses. Laboratory findings showed that he also had severe anemia of the type associated with infection and chronic diseases, leukocytosis, eosinophilia, and severe hypoalbuminemia and hypergammaglobulinemia. His cellular and humoral immune responses were unaltered. New aspects on the functional repercussion of the parasitism of the human host by Sparganum proliferum are discussed. Treatment with mebe

This report concerns two patients, a 43-year-old woman and a 53-year-old man, who developed clinical as well as laboratory signs of permanent gonodal and thyroid failure following an acute intracranial infection--in the woman a meningoencephalitis of unknown origin, and in the man an encephalitis caused by Coxsackie B5. Endocrine investigations were compatible with hypothalamic-pituitary dysfunction, with some of the results favoring a hypothalamic lesion. Perhaps hormone deficiency of hypothalamic and/or pituitary origin is a more common sequel of acute meningoencephalitis than has hitherto b

A review of the records of 79 consecutive patients who had a positive immunodiffusion or complement-fixation test for histoplasmosis showed 28 (35%) with false-positive serologic results in relation to their clinical significance. Twelve patients (15%) had complement-fixation titers of 1:32 or greater, without cultural or histological evidence of active infection. No common clinical or laboratory finding could be correlated with the presence of false-positive tests. The latter finding correlates well with the incidence of false-positive complement-fixation titers (12%) in the normal random pat

In the past, investigators have been able to produce hepatic amebiasis in laboratory animals only by direct introduction of parasites into the liver or its vasculature, or by other artificial manipulations. A natural model of human visceral amebiasis has been lacking. We document an extensive outbreak of amebic dystentery which took place in a colony of spider monkeys; severe hepatic abscesses occurred in many animals. The spider monkey is highly susceptible to infection with Entamoeba histolytica and could provide a valuable model for the study of the pathogenesis of invasive amebiasis.

Actinomycosis of the gallbladder is rare, and the pathogenesis of the infection is poorly understood. The authors report the case of a 77-year-old man admitted to hospital because of abdominal pain. Clinical and laboratory findings suggested the diagnosis of cholecystitis and cholelithiasis. At cholecystectomy the gallbladder was inflamed and contained many calculi. Gram staining of material from the mucosa of the gallbladder demonstrated gram-positive pleomorphic filaments, and Actinomyces israelii grew in pure culture, thus confirming the diagnosis of actinomycosis of the gallbladder. The pa

A patient with endocarditis produced by Listeria monocytogenes is presented, the twelfth such case reported. A review of the available literature shows that the infection has involved only the left side of the heart, has not been associated with debilitating diseases, and carries a significant mortality rate. Otherwise, clinical and laboratory features have been the same as those of usual forms of bacterial endocarditis. It is pointed out that the Listeria organism is commonly mistaken for Erysipelothrix and diphtheroids and that a battery of tests should be employed before one disregards thes

Numerous studies reported an increase of postpartum mood symptoms during the COVID-19 pandemic. Yet, the link between severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and perinatal mental health is less well understood. We investigated the associations between prenatal SARS-CoV-2 infection and postpartum depressive and anxiety symptoms, including examinations of infection timing and pandemic timeline. We included 595 participants from Generation C, a prospective pregnancy cohort in New York City (2020-2022). Prenatal SARS-CoV-2 infection was determined via laboratory or m

BACKGROUND: Group B Streptococcus (GBS) is a major cause of serious neonatal infection but its role in maternal morbidity has received little investigation. The aim of this study was to determine whether GBS colonization at delivery is associated with increased risk of maternal peripartum infection.
METHODS: In this prospective cohort study, 1746 unselected women had a vaginal-rectal culture taken at the onset of labor. Diagnosis of maternal peripartum infection was based on a combination of two or more signs or symptoms including fever, breast pain, severe wound or pelvic pain, purulent disc

Reovirus infections pose a significant threat to waterfowl health and productivity globally. This review provides a comprehensive update on various aspects of waterfowl reoviruses (WRVs) affecting domestic duck and goose species. We outline the genetic diversity and evolution of circulating strains. The paper details the array of clinical signs and pathologies observed in infected birds. Most advanced laboratory diagnostic methods, including molecular techniques, are reviewed for their role in rapid and accurate detection, forming the cornerstone of effective surveillance programs. Acute bacterial skin and soft tissue infections represent a common clinical problem, and accurate diagnosis is crucial for initiating appropriate therapy. However, conditions such as cellulitis and erysipelas can be clinically mimicked by a variety of non-infectious conditions, including eczematous, venous, lymphatic, and autoimmune diseases. This review summarizes the key differences in clinical presentation, patient history, and laboratory findings that help distinguish true infections from their non-infectious mimickers. Special attention is given to conditions such as microbial eczema, her

Individuals who have shown recovery from coronavirus disease (COVID-19) are increasingly getting diagnosed with Mucormycosis or "Black fungus." It is a difficult condition to diagnose as it has symptoms that are common among a variety of diseases. Hence, it is important to identify the presenting signs and understand the underlying pathogenesis of COVID-19 associated Mucormycosis. The incidence of these mycotic infections has shown a substantial increase in current times owing to an increase in the prevalence of immunocompromised subjects, human immunodeficiency virus (HIV) infection, and acqu

Malassezia are lipophilic and commensal yeasts capable of inducing skin disease among susceptible hosts. However, severely immunocompromised patients and preterm infants admitted to intensive care units are particularly at risk of developing Malassezia systemic infections. Patients often have central venous catheters which are usually the portal of entry for colonization and infection. In addition to the clinically non-specific findings, a delay in the laboratorial diagnosis may occur as there is often the need to add lipid supplementation to culture in order to support these organisms' growth
