Medically uninsured children in the United States: a challenge to public policy.

This article is an examination of the nature and extent of the problem presented by medically uninsured children in the United States. First, the characteristics of the uninsured population are explored with a description of how age, family income, and employment status disproportionately affect families with children. Second, the Medicaid program and its historically inadequate response to this growing problem of uninsured children is examined. Third, the relationship between insurance status and the health and development of children is discussed. Finally, recent public policy initiatives th

A stitch in time: narrative review of interventions to reduce preterm births in Malawi.

BACKGROUND: The rising rate of preterm births (PTBs) is a global concern, and Malawi has a high rate of PTBs (10.5%). The resulting neonatal and under-5 mortality, morbidity and lifelong disability represent a significant loss of human potential affecting individuals, families and society as a whole. This study aims to review the literature to determine the risk factors for PTB in Malawi and to identify effective interventions to prevent PTBs. METHODS: A literature search yielded 22 studies that were categorized according to risk factors implicated for PTBs and health interventions to reduce

Maternal leave policies and vaccination coverage: a global analysis.

Childhood vaccination is a proven and cost-effective way to reduce childhood mortality; however, participation in vaccination programs is not universal even where programs are free or low cost. Studies in diverse countries have reported work conflicts as limiting parents' ability to vaccinate their children. Using policy data for 185 UN member countries, we explore the hypothesis that an increased opportunity for parents to bring children to vaccination sites will translate into higher childhood vaccination rates. To do so, we use OLS regression to examine the relationship between the duration

Nursing implications of the 2006 NIH State of the Science Conference Statement: Cesarean Delivery on Maternal Request.

Will the nation's higher rate of cesarean deliveries (CDs) (over 29%) lead to increased health risks for mothers and their babies? In March 2006, a National Institutes of Health State of the Science Conference was convened to assess available research on the topic. The focus was to identify outcomes from research studies addressing "cesarean delivery on maternal request" (CDMR), defined as CD without a medical indication for either the mother or the fetus. This conference concluded that it is currently unknown whether CDMR is really a trend in the United States, or whether the emergence of thi

Husbands' Plan to Participate in Birth Preparedness and Complication Readiness in Haramaya Health and Demographic Surveillance System Site, Eastern Ethiopia: A Community-Based Cross-Sectional Study.

BACKGROUND: Partner involvement in maternal health services utilization remains a major public challenge in the developing world. Strategies of involving men in maternal health services are a critical and proven intervention for reducing maternal and neonatal mortality by ensuring safe delivery and reducing complications during childbirth. Moreover, the husbands' involvement during pregnancy helps their spouses to make timely decisions and avoid maternal delays, especially first and second delays. Although birth and complication readiness have been studied in developing countries such as Ethio…

An agenda for meeting the special needs of multiple birth families.

Over 80,000 multiple birth babies are born each year in the U.S. Their families must cope with a constellation of complex physical and psychosocial challenges, which jeopardizes their health and functioning. The demands of twin pregnancy and the parenting of twins, triplets, quadruplets, quintuplets or more puts these families at disproportionately high risk for infant mortality, birth defects, child abuse, substance abuse, financial problems and marital problems. Health and social service resources must be developed to alleviate the stresses associated with multiple birth and to empower paren

Global protein profiling of human milk using pre-enriched RNA-sequence libraries.

The complex protein composition of human milk remains challenging to characterize due to technical limitations, yet such insights are crucial as early-life nutrition influences long-term health outcomes and is associated with pathological conditions such as obesity. In this study, we applied APTASHAPE, a high-throughput profiling method based on chemically modified protein-binding RNA molecules, to globally profile protein composition in skimmed human milk and to explore associations with maternal and infant characteristics. A total of 520 human milk samples collected at 3 days, 1 month, 2 mon

Antenatal pertussis vaccine effectiveness in England remains high post COVID-19.

OBJECTIVES: In 2024, pertussis peaked at nearly 15,000 confirmed cases in England leading to the declaration of a national public health incident. Infants aged under 3 months had the highest incidence, with eleven deaths in 2024. The maternal programme effectiveness was reassessed as eligibility had been extended to 16 weeks since earlier estimates. METHODS: 188 laboratory-confirmed cases in under 3-month-olds from January 2020-March 2024, and matched coverage data from 453,740 pregnancies in the Clinical Practice Research Datalink, were used to estimate maternal vaccine effectiveness (VE) us

Maternal BCAA overnutrition programs persistent dysglycemia in lean adult offspring.

Maternal nutrition exerts profound, lasting effects on offspring metabolic health, yet the impact of maternal overconsumption of key nutrients such as branched-chain amino acid (BCAAs) remains poorly understood. Here, we show that intake of a BCAA-enriched isocaloric, protein content-matched diet throughout pregnancy and lactation induces hyperglycemia and altered circulating amino acid profiles in mouse dams, and programs lasting changes in offspring glucose homeostasis. Adult offspring of both sexes on a chow diet exhibited glucose intolerance. Male offspring showed fasting hyperglycemia des

Maternal glycaemia and child educational attainment at age 5 in the Born in Bradford cohort.

AIMS: There is emerging evidence that maternal hyperglycaemia may be associated with adverse offspring neuro-behavioural outcomes, which are foundational to educational success. We hypothesised that higher levels of maternal glucose would be associated with poorer educational attainment in early childhood. METHODS: The sample included 13,627 children from the UK's Born in Bradford cohort. Exposures included maternal fasting glucose, 2-h post-load glucose and a clinical diagnosis of gestational diabetes. The primary outcome was failure to achieve a 'good level of development' on the Early Year

Exposure to Polystyrene Microplastics Disrupts Blood Cell Homeostasis and Metabolic Profiles in Pregnant Mice and Offspring: The Role of Oxidative Stress and Inflammation.

Micro/nanoplastics (MNPs) are emerging contaminants of concern for maternal and fetal health, yet their effects on the maternal-fetal circulation and serum metabolic homeostasis remain unclear. Here, we investigated the maternal and offspring toxicity of polystyrene microplastics (PS-MPs) and serum metabolomic alterations in dams and offspring. PS-MPs accumulated in multiple tissues, including blood, indicating maternal-to-offspring transfer. Continuous exposure reduced litter size, induced hepatic oxidative stress, and increased IL-6 and TNF-α levels in a dose-dependent manner in both dams an

Liposomal encapsulation of L-arginine and L-citrulline enhances pharmacokinetics and therapeutic effects in a model of preeclampsia and fetal growth restriction.

L-arginine and L-citrulline improve vascular health in preeclampsia (PE) and fetal growth restriction (FGR), but their short half-life limits efficacy. This study evaluated pharmacokinetics, delivery, and therapeutic outcomes of liposomal encapsulation of these amino acids in a rat model of PE and FGR. Pharmacokinetics of liposome-encapsulated L-arginine were compared with free L-arginine (Free L-arg) in normal pregnant (NP) dams. Therapeutic effect on maternal blood pressure and fetal weight were studied in NP rats and the reduced uterine perfusion pressure (RUPP) model. Treatment groups rece

Oral administration of cyantraniliprole to wistar rats during pregnancy and lactation alters maternal behavior and harms the female reproductive system of pubertal and adult offspring.

Over the past years, global pesticide use has increased by 20%. New insecticidal molecules, like cyantraniliprole, aim to reduce side effects due to the high toxicity of pesticides and their harmful effects on health and the environment. Its mechanism involves binding to ryanodine receptors, causing rapid calcium ion release. As a new molecule, studies on its effects on mammals are scarce. This work evaluates oral cyantraniliprole's impacts during pregnancy and lactation on maternal behavior and the reproductive system of female offspring. Approved by CEUA-UEL (OF. CIRC. CEUA n. 20/2020), Wist

Rare variant associations with birth weight identify genes involved in adipose tissue regulation, placental function and insulin-like growth factor signalling.

Investigating the genetic factors influencing human birth weight may lead to biological insights into fetal growth and long-term health. We report analyses of rare variants that impact birth weight when carried by either fetus or mother, using whole exome sequencing data in up to 234,675 participants. Rare protein-truncating and deleterious missense variants are collapsed to perform gene burden tests. We identify 9 genes; 5 with fetal-only effects on birth weight, 1 with maternal-only effects, 3 with both, and observe directionally concordant associations in an independent sample. Four of the

Sex-specific association between maternal childhood adversities and offspring's weight gain in a Brazilian cohort.

Maternal adverse childhood experiences (ACEs) are linked to negative health and developmental outcomes in offspring. However, whether maternal ACEs influence infant weight gain in the first months of life, and if this effect differs by infant sex, remains unclear. This study included 352 full-term newborns from low-risk pregnancies and their mothers in low-income settings in Brazil. Anthropometric data (weight, length, head circumference) and other information (feeding type, offspring sex, family income) were collected at delivery (W0), discharge (W1), and up to 8 weeks postpartum (W2). Potential of Pandan Root and Teak Leaf Extracts in Managing Maternal Hyperglycemia During Pregnancy: Comparative Efficacy and Mechanistic Insights.

Maternal hyperglycemia during pregnancy poses significant health risks to both mother and fetus. Although gestational diabetes mellitus (GDM) is mainly characterized by insulin resistance, severe hyperglycemia may also result from impaired pancreatic function. This study evaluates the therapeutic potential of pandan (Pandanus amaryllifolius) root and teak (Tectona grandis) leaf extracts in managing streptozotocin (STZ)-induced maternal hyperglycemia in pregnant rats, compared to metformin. Methods: Pregnant rats were administered STZ (60 mg/kg) on gestation day 5. Treatments with metformin (30

Effect of α-ketoglutarate on maternal lipid homeostasis and mitochondrial status perturbed by gestational arsenic exposure.

Arsenic is a common environmental toxicant with known hepatotoxic effects, yet its impact on maternal lipid metabolism during pregnancy remains poorly understood. In this study, we established a pregnant mouse model to investigate the effects of gestational arsenic exposure and the potential protective role of α-ketoglutarate (α-KG), a key tricarboxylic acid (TCA) cycle intermediate. In the first experiment, arsenic exposure led to significant disruptions in maternal serum and hepatic lipid profiles. Mechanistically, arsenic reduced hepatic α-KG concentrations, impaired mitochondrial ultrastru

Pregnant and breastfeeding women concerns during a group B Streptococcus phase ll clinical trial: A qualitative study in Kampala, Uganda.

Despite evidence that maternal vaccines can contribute to reduction of neonatal infections, vaccine hesitancy is a challenge in many low- and middle-income countries like Uganda. We conducted in-depth interviews with pregnant women and focus group discussions with breastfeeding women who were part of a Group B Streptococcus (GBS) clinical trial. We explored the women's concerns about vaccination and their reasons for being hesitant to take vaccines before they joined the trial. Women aged 18-39 were randomly selected from follow-up lists during the study period. Data were analysed thematically
