Clinical implications of white-coat effect among patients attending at a lipid clinic.

We evaluated clinical implications of the white-coat effect (WCE) in cardiovascular (CV) risk stratification in the primary prevention setting of a Lipid Clinic. We compared home self blood pressure measurement (SBPM) with office blood pressure (BP) readings and BP measured by a nurse before and after the visit on consecutive subjects, free of previous CV diseases, attending at a Lipid Clinic for a first visit. Additionally, we evaluated whether and to what extent the difference between these measurements affect the 10-year cardiovascular risk calculated according to current guidelines. Mean h

[Abuse and neglect of older care recipients in domestic settings - a survey among nurses of in-home care services].

Although some anecdotal evidence for the phenomenon of abuse and neglect of community-dwelling older care recipients by in-home care services exists, there is an almost complete lack of data on this topic. In order to determine extent and risk factors of abuse and neglect of older care recipients by nurses, we conducted a self-report study among in-home care workers in the German city of Hanover. A total of 503 nurses took part in the study; the response rate was 43.3%. Nearly 40% of all respondents reported having abused or neglected at least one patient within the last 12 months. Psychologic

Caregiver bodywork: family members' experiences of caring for a person with motor neurone disease.

AIM: This paper reports a study of how family members caring for people living with motor neurone disease managed the deteriorating body, their own bodywork and the associated emotional labour. BACKGROUND: People living with the neurodegenerative condition of motor neurone disease face the prospect of dying in 3-5 years from progressive loss of voluntary muscle mass and function, culminating in respiratory failure. Theories concerning the body in illness have been used to illustrate patient perspectives; however, family caregivers' experiences of the body have been neglected. METHOD: An ethn

Aspirated bezoar in a pediatric patient: a case report.

This case report describes an 11-year-old boy with a known esophageal bezoar admitted to surgery for esophagogas-troduodenoscopy with biopsy and possible removal of the bezoar. On induction, a copious amount of fluid and solid food particulate was noted in the posterior oropharynx, interfering with immediate intubation. After clearing of the oropharynx, the patient was intubated. A bronchoscopy revealed aspirated food materials. A stat intraoperative consultation was initiated for rigid bronchoscope with bronchial suctioning to clear aspirated undigested food particles and an esophageal scope

Factors related to nonadherence to low sodium diet recommendations in heart failure patients.

BACKGROUND: A low sodium diet is a cornerstone of nonpharmacologic therapy for heart failure patients. Although nonadherence is common, little is known about why heart failure patients fail to adhere to this diet. AIMS: The purpose of this study was to explore the experience of heart failure patients in following a low sodium diet. METHODS AND RESULTS: We conducted a qualitative descriptive study with a convenience sample of 20 participants. Interviews were conducted and analyzed for themes. The data reflected three primary themes about nonadherence to the low sodium diet: lack of knowledge,

The impact of homelessness on the health of families.

Qualitative research using the symbolic interactionism framework and grounded theory methodology was employed to discover the perceived health problems and dangers that homeless families with children endure. Data were collected using semistructured interviews from 34 homeless volunteer participants with 87 children. An in-depth analysis of the data using the constant comparative method led to recurrent descriptors and patterns, which were synthesized under four themes: external locus of control, deterrents to health, economic barriers, and lack of support. The findings indicate that there is

Identifying at risk nursing home residents using a polydipsia screening tool.

Persons diagnosed with schizophrenia are considered at risk for polydipsia, a potentially life-threatening condition characterized by excessive consumption of fluids. This study examined the demographic and health-related characteristics of nursing home residents with psychiatric diagnoses (N = 70) who reside in a 92-bed facility. The prevalence of polydipsia and behavioral characteristics and symptoms as measured by a 17-item polydipsia screening tool also were described. Patients who screened positive for polydipsia (36%) exhibited behaviors that included incontinence, smoking, frequent void

Faulty assumptions associated with the use of restraints with children.

TOPIC: The use of restraints in nursing and other professions has a long and troubling history, particularly in psychiatric settings. No research evidence has demonstrated restraints to be therapeutically effective. A number of faulty assumptions appear to support the continued practice of this unproven and potentially dangerous intervention. PURPOSE: To present and refute several faulty assumptions using the empirical literature and theory, and to discuss alternatives based on the developmental-ecological theoretical framework for responding to crises in children. SOURCES: Extant empirical

Experiences and expectations of the new genetics in relation to familial risk of breast cancer: a comparison of the views of GPs and practice nurses.

BACKGROUND: Advances in genetics may change the practice of medicine in many ways. Ascertaining practitioners' perceptions about managing the risk of familial breast cancer can give an insight into the current and expected impact on general practice to inform relevant education. Little is known about the practice nurses' (PNs) views of the new genetics in comparison with those of the GP. OBJECTIVES: Our aim was to describe and compare the views of GPs and PNs on their experiences and expectations of the new genetics in relation to managing familial risk of breast cancer. Managing trauma patients with abdominal compartment syndrome.

ACS is due to a rapid increase in intra-abdominal pressure. Although ACS may occur in both surgical and nonsurgical patients, patients who have abdominal or pelvic trauma and/or require massive fluid replacement are at increased risk. Critical care nurses are in a unique position to recognize early signs and symptoms of increased intra-abdominal pressure to ensure timely intervention. Aggressive hemodynamic, pulmonary, and operative management is essential for the optimal outcome of patients with ACS. Without definitive treatment, multisystem organ dysfunction and death ultimately ensue.

Common but curable. Responding to symptoms of testicular cancer.

Testicular cancer is one of the most common cancers in men between the ages of 15 and 34. Symptoms of testicular cancer vary but can include scrotal nodules or swelling and a sensation of fullness or heaviness of the scrotum, which may be interpreted as pain. Four treatment options are available for testicular cancer: surgery to remove the cancer, radiation therapy, chemotherapy and bone marrow transplantation. No matter what a patient's treatment course, follow-up studies and office visits are imperative.

Time and motion: a study of trauma surgeons' work at the bedside during the first 24 hours of blunt trauma care.

BACKGROUND: The current literature defines the costs of trauma care in terms of hospital costs and charges. We sought to define the qualitative and quantitative labor costs of trauma care by measuring the various components of bedside care provided by surgeons at a community hospital. METHODS: We conducted a prospective time-and-motion study during the initial 24 hours of blunt trauma patients' stay in the hospital at a Level II trauma center. The services provided by two surgeons and one nurse practitioner were examined. All patients were resuscitated and seen initially by one of the physici

Use of folic acid-fortified milk in the elderly population.

Folic acid deficiency is common in the elderly population, resulting in anaemia, dementia, many neurological sequelae and an indirect role in atheromatous disease. An increase in natural food folate is relatively ineffective at increasing folate status and the use of folate fortification of foodstuffs is recommended. The aim of our study was to assess the benefits of folic acid-fortified milk to the folate status of an elderly institutionalised population. 49 subjects received fortified milk as part of their daily diet for at least 6 months (active group) and 40 subjects received unfortified m

Programs for families of organ and tissue donors.

Until recently, the families of patients who have given the gift of life have been the invisible group in the transplant circle. They donated the organs and tissues of their loved ones to unknown transplant recipients and then were to grieve alone. As transplantation has matured and become the treatment of choice for end stage organ failure and for other life-enhancing procedures, the importance of the donor and the donor family is being recognised and their needs and expectations identified.

Parents' experience of hospitalization: different strategies for feeling secure.

Twenty parents of boys (ages 2-14 years) hospitalized for hypospadias repair in a pediatric surgery department in Sweden, were interviewed concerning their experience when their child was hospitalized. A qualitative analysis of the interviews indicated that the most important issue to the parents was finding security at the hospital. Parents manifested one of three different strategies that enabled them to feel secure at the hospital; (a) relinquishing the care of their children to the nursing staff; (b) obtaining a measure of control over their children's care; and (c) relying on knowing thei
