Beyond the bowel: novel comorbidity patterns in inflammatory bowel disease from the all of us research program.

BACKGROUND: Inflammatory bowel disease (IBD) manifests systemically, yet most comorbidity studies rely on predominantly European populations. The All of Us Research Program enables investigation across demographically diverse groups. METHODS: We matched 5,094 IBD patients 1:4 with controls by age, gender, and race, analyzing comorbidities using logistic regression with Mantel–Haenszel adjustment. Multiple testing correction used false discovery rate (FDR) with significance thresholds of OR > 1.5 or < 0.5 and FDR < 0.05. RESULTS: Our cohort included 29.2% non-White participants versus 10–15% in

Managing comorbidities in psoriatic arthritis: A state-of-the-art approach to integrated care (2025).

Psoriatic arthritis (PsA) is a chronic immune mediated musculoskeletal condition that is associated with psoriasis. PsA is recognized not as an isolated articular disorder, but as a systemic inflammatory condition with interrelated involvement of the skin, joints, entheses, spine, and gut. The systemic nature of PsA extends beyond musculoskeletal and cutaneous involvement to include a broad spectrum of comorbid and related inflammatory conditions that contribute to disease burden, treatment complexity, and long-term outcomes. PsA is associated with significant comorbidities. This includes card

Therapeutic Potential Targeting Gut Microbiota Modulation With Emphasis on Lactobacillus spp. in Common Metabolic Disorders: A Systematic Review.

Metabolic disorders are complex conditions that arise from abnormal biochemical reactions, disrupting normal metabolic processes. The most prevalent metabolic disorders include obesity, Type 2 diabetes mellitus (T2DM), cardiovascular disease (CVD), nonalcoholic fatty liver disease (NAFLD), and inflammatory bowel disease (IBD). Despite extensive research, no definitive therapeutic strategy has been established for a complete cure. Emerging evidence suggests that gut microbiome dysbiosis plays a critical role in the pathogenesis of these disorders, as maintaining microbial homeostasis is essenti

Cardiac-specific GCN5L1 deficiency promotes MASLD in HFpEF.

The prevalence of cardiometabolic heart failure with preserved ejection fraction (HFpEF) continues to grow, representing over half of heart failure cases in the United States. As no specific medication for HFpEF exists, treatment guidelines focus on the management of comorbidities related to metabolic syndrome (e.g. obesity, diabetes, hypertension) that promote the disease1. These same comorbidities also drive pathology in non-cardiac tissues, and the links between cardiometabolic disease presentations in different organs are increasingly being recognized. Preclinical studies on the potential

Sleep apnea at the frontier- an integrated multidisciplinary functional framework.

In this review, we propose a systems-motivated approach to sleep apnea, as a well characterized disorder illustrative of the sleep-wake process. We advance a framework with mechanistic underpinning of the insulin-resistant cardiometabolic effects of obesity and its putative bi-directional relationship with sleep apnea. Such a physiological construct affords an integrated perspective where pathophysiological outcomes in the brain and heart can be objectively characterized by imaging methods that index functional derangements, including those related to clinical measures of metabolism and ischem

Late Diagnosis of Prader-Willi Syndrome in an Adolescent With Significant Complications of Type 2 Diabetes.

Youth-onset type 2 diabetes (YO-T2D) is on the rise and disproportionately affects those from low socioeconomic status and specific ethnic and racial minority groups. YO-T2D has a more aggressive phenotype than adult-onset T2D. We present a case of a 14-year-old patient who presented after a fall and was found to be acutely unwell with significant peripheral edema. Initial investigations indicated hyperglycemic hyperosmolar state, acute kidney injury, and nephrotic range proteinuria, leading to a new diagnosis of T2D. Subsequent evaluation revealed mild retinopathy and diabetic nephropathy. Si

An Online Prescription for Hospitalisation: Euglycaemic Ketoacidosis Caused by an Online Tirzepatide Prescription.

Obesity affects a significant proportion of the UK population, and tirzepatide, a dual glucagon-like peptide (GLP-1) and gastric inhibitory polypeptide (GIP) receptor agonist, is increasingly being prescribed by online services as well as primary care for weight loss in both diabetic and non-diabetic patients. We present the case of a 61-year-old woman with no past medical history, who developed euglycaemic ketoacidosis following six weeks of tirzepatide (Mounjaro®) therapy, which was initiated via an online consultation for weight loss. Despite normal blood glucose levels, she exhibited signi

Antidepressants to Manage Osteoarthritic Pain: The Value of Pain Phenotyping.

Osteoarthritis (OA) is a chronic condition in which pain significantly affects quality of life, often leading to reduced physical activity and disability. Globally, an estimated 595 million people are affected, with the numbers likely to increase owing to an aging population and rising obesity rates. Effective pain management is crucial, yet current treatments, such as nonsteroidal anti-inflammatory drugs (NSAIDs) and opioids, often provide limited relief and come with risks. One reason for this limited success is the insufficient recognition of the importance of psychosocial factors and heter

[Multiple spontaneous coronary artery dissections: a multidisciplinary challenge in diagnosis and treatment].

Spontaneous coronary artery dissection (SCAD) is a rare but increasingly recognized cause of acute coronary syndrome, primarily affecting young women without traditional cardiovascular risk factors. We report the case of a 57-year-old woman with obesity and a history of smoking, admitted for acute respiratory failure due to pneumonia, complicated by cardiac arrest. Elevated troponin levels, along with subsequent ECG and echocardiographic findings suggestive of previous myocardial ischemia, led to the indication for coronary angiography. The angiogram revealed severe three-vessel coronary disea

Metabolic challenges of glucose and lipid dysregulation in psoriatic arthritis: a narrative review from pathogenesis to clinical practice.

Emerging evidence highlights a complex interconnection between metabolic dysfunction and chronic inflammation in psoriatic arthritis (PsA), positioning glucose and lipid abnormalities not only as comorbidities but as active contributors to disease pathogenesis. In fact, rather than being incidental findings, conditions such as insulin resistance, atherogenic dyslipidemia, and visceral obesity interact with the immune system, amplifying inflammatory circuits and promoting joint and systemic damage. This review delves into the metabolic dimension of PsA, shedding light on how pro-inflammatory cy

Comorbidities and multimorbidity in asthma.

PURPOSE OF REVIEW: To describe the associations between asthma and relevant comorbidities, and appraise the latest evidence on the management strategies of asthmatics with comorbid conditions. RECENT FINDINGS: Conditions such as allergic rhinitis, chronic rhinosinusitis with and without nasal polyps, gastroesophageal reflux disease, obesity, chronic obstructive pulmonary disease, bronchiectasis, anxiety and depression have been linked to worse outcomes in asthma. Recognition and treatment of these conditions is important in asthma, particularly in those with uncontrolled or severe asthma. Bio

New approaches to triglyceride reduction: Is there any hope left?

Triglycerides play a crucial role in the efficient storage of energy in the body. Mild and moderate hypertriglyceridemia (HTG) is a heterogeneous disorder with significant association with atherosclerotic cardiovascular disease (ASCVD), including myocardial infarction, ischemic stroke, and peripheral artery disease and represents an important component of the residual ASCVD risk in statin treated patients despite optimal low-density lipoprotein cholesterol reduction. Individuals with severe HTG (>1,000 mg/dL) rarely develop atherosclerosis but have an incremental incidence of acute pancreatiti

Characterizations of White Mulberry, Sea-Buckthorn, Garlic, Lily of the Valley, Motherwort, and Hawthorn as Potential Candidates for Managing Cardiovascular Disease-In Vitro and Ex Vivo Animal Studies.

Cardiovascular diseases are a broadly understood concept focusing on vascular and heart dysfunction. Lack of physical exercise, type 2 diabetes, obesity, hypertension, dyslipidemia, thromboembolism, and kidney and lung diseases all contribute to the development of heart and blood vessel dysfunction. Although effective and important, traditional treatment with diuretics, statins, beta blockers, calcium inhibitors, ACE inhibitors, and anti-platelet drugs remains a second-line treatment after dietary interventions and lifestyle changes. Scientists worldwide are still looking for an herbal product

Prognostic Impact of Metabolic Syndrome and Steatotic Liver Disease in Hepatocellular Carcinoma Using Machine Learning Techniques.

Metabolic dysfunction-associated steatotic liver disease (MASLD) currently represents the predominant cause of chronic liver disease and is closely linked to a significant increase in the risk of hepatocellular carcinoma (HCC), even in the absence of liver cirrhosis. In this retrospective multicenter study, machine learning (ML) methods were employed to investigate the relationship between metabolic profile and prognosis at diagnosis in a total of 219 HCC patients. The eXtreme Gradient Boosting (XGB) method demonstrated superiority in identifying mortality predictors in our patients. Etiology

Cardiovascular Implications of Gynecological Disorders: Bridging the Gap Between Gynecology and Cardiology.

Gynecological disorders such as endometriosis, polycystic ovary syndrome, and gynecological cancers are increasingly recognized as potential risk factors for cardiovascular disease (CVD). Endometriosis, a chronic inflammatory condition, exhibits shared pathogenic mechanisms with CVD, including endothelial dysfunction and an atherogenic lipid profile. Emerging evidence suggests a link between endometriosis and an elevated risk of cardiovascular events such as myocardial infarction, ischemic heart disease, and hypertension. Hyperglycemia and Venous Thromboembolism.

Patients with diabetes mellitus (DM) have chronically increased blood glucose and multiple physiologic alterations that place them at elevated risk for vascular disease. Traditionally, this vascular risk has mainly referred to chronic atherosclerosis and embolic arterial disease. Retrospective studies have suggested an increased risk of a pulmonary embolism (PE) and deep vein thrombosis (DVT), collectively termed venous thromboembolism (VTE), in patients with DM, but this association has been difficult to demonstrate with comorbidities such as obesity in meta-analysis. Clinical studies have de
