[Open diaphyseal fractures of long tubular bones in traffic accidents].

An analysis of the treatment of 203 patients with open diaphysial fractures of the long tubular bones, resulting from motor-road accidents has been carried out. Late results have been studied in 134 cases (66%) with follow-up ranging from 1 to 10 years. 69% of them have completely restored their working capacity with the possibility to return to their previous work. The results of the treatment were estimated in three-mark grading system terms: 55.7% of all the examined patients showed good results, 30.9% showed fair results and 13.4% showed poor results (pseudoarthrosis, malunion, osteomyelit

Preventing complications during acetabular fracture surgery, a review.

Complications after acetabular fracture surgery include: hemorrhage, nerve injury, infection, malreduction, heterotopic ossification, post-traumatic arthritis, and medical complications may reach an incidence of 30-50 %. Total hip arthroplasty can salvage a poor reduction or post-traumatic arthritis but has higher complication rates then a primary replacement and in many cases may have to last 50 years. Sciatic nerve palsy, which is often caused by the injury, and infection/osteomyelitis, which is often caused by the surgical intervention, result in universally poor outcomes and every effort s

Septic Arthritis: Current Concepts.

Septic arthritis is a major cause of morbidity and mortality worldwide, with survivors often facing long-term functional impairments. Prompt diagnosis is crucial to decrease complications such as cartilage loss, osteomyelitis, and reduced mobility. Imaging plays a central role in the diagnosis, as well as offering guidance for diagnostic aspiration. Because septic arthritis may involve any joint in the body, both native and prosthetic, different imaging modalities may be more applicable in various scenarios. We describe the hallmark imaging findings of septic arthritis in radiography, ultrasou

Minimally Invasive Surgery and Diabetes.

Diabetic foot complications, encompassing diabetic foot ulcers (DFUs), diabetic foot osteomyelitis (DFO), and Charcot foot, are prevalent and debilitating conditions that often lead to infection, deformity, and limb amputation. These conditions substantially impact patient morbidity, health care costs, and quality of life. Integrating minimally invasive techniques offers promising opportunities for improved patient care, reducing the health care burden associated with diabetic foot complications. This review examines the efficacy of minimally invasive metatarsal osteotomies in preventing and t

Piezoelectric Surgery and Oral Cavity Disorders in Small Mammals.

Piezoelectric surgery, leveraging ultrasonic vibrations for selective cutting of mineralized tissues while preserving soft tissue integrity, has revolutionized veterinary dental and maxillofacial procedures. This technique enhances precision, reduces postoperative pain, and accelerates healing by minimizing damage to surrounding structures. Its antibacterial cavitation effects and ability to address fragile bone structures make it suitable for rabbits and small exotic mammals. Applications include dental extractions, treatment of osteomyelitis, otitis media and chronic rhinitis. Ultrasonic wou

Otitis Externa and Malignant Otitis Externa-for the Hospitalist/Internist.

Otitis externa has varying degrees of chronicity and severity. Acute otitis externa refers to a process of less than 6 weeks' duration, while chronic otitis externa lasts more than 3 months. Treatment of both includes ototopical agents and appropriate aural hygiene. Chronic otitis externa often requires routine debridement by an otolaryngologist. Malignant otitis externa is now referred to as osteomyelitis of the temporal bone. Patients will often have excruciating pain on examination. Granulation tissue at the cartilaginous-bony junction is a hallmark of the disease, but not always encountere

Bone SPECT/CT in Advanced Diffuse Tenosynovial Giant Cell Tumor of the Wrist.

Tenosynovial giant cell tumor, previously known as pigmented villonodular synovitis, is a benign low-grade fibrohistiocytic proliferation with hemosiderin deposits in synovial joints. Mostly affecting the knee, it can also manifest in other synovial joints, infrequently also in the wrist. Tenosynovial giant cell tumor typically causes intense radionuclide uptake in all phases in planar bone scintigraphy, making a differentiation from other bone tumors or osteomyelitis difficult, especially in cases associated with extensive bone destruction. We present a case of an unusually advanced and exten

The first reported case of Staphylococcus pettenkoferi prosthetic joint infection.

Staphylococcus aureus and Staphylococcus epidermidis are among the most frequent causes of prosthetic joint infections (PJIs). Staphylococcus pettenkoferi, a coagulase-negative Staphylococcal species first described in 2002, has been detected in fewer than 20 patients with true infection (15 cases of bacteremia and 1 case of osteomyelitis). This organism has never been implicated in a PJI, likely owing in part to the difficulty in identification via biochemical methods. S. pettenkoferi is almost universally reported to be a contaminant when detected. We report the first case of a PJI caused by

Orthopedic infections due to Trichosporon species: Case series and literature review.

Reports of orthopedic fungal infections caused by Trichosporon species are extremely scarce, thus we aimed to describe a case series and review the cases published in the literature. Patients were retrospectively included if a previous culture of bone, joint, or soft tissues had resulted positive for Trichosporon species along with a clinical diagnosis of an orthopedic infection. Eight patients were included with diverse orthopedic conditions, most of them cases of osteomyelitis. The main isolated species was Trichosporon asahii. All patients were treated with antifungals, mainly voriconazole,

Imaging update on musculoskeletal infections.

The clinical diagnosis of musculoskeletal infections can be challenging due to non-specific signs and symptoms on presentation. These infections include infectious myositis, necrotising fasciitis, septic arthritis, septic bursitis, suppurative tenosynovitis, osteomyelitis, spondylodiscitis and periprosthetic infections. Diagnostic imaging is routinely employed as part of the investigative pathway to characterise the underlying infectious disease pattern, allowing expedited and customised patient management plans to optimise outcomes. This article provides an update on the various imaging modal

Chronic sternal wound fistula after coronary artery bypass surgery: A case series.

Chronic extensive infection of the sternal wound may be a serious problem in patients undergoing sternotomy, especially those who have been operated for coronary artery bypass grafts. We report and evaluate the outcomes of five cases involved in chronic sternal osteomyelitis who were treated with two different strategies as follows: (1) debridement and secondary healing (conventional treatment), and (2) debridement and omental flap transfer for primary wound closure. All of the patients had acceptable results after treatment, but those who were managed by omental flap and primary wound closure

Aspergillus Sternomyelitis Developed from Chronic Pulmonary Aspergillosis as a Late Complication to Lobectomy for Lung Cancer.

Progressive fibrobullous changes in the residual lobes are sometimes observed after lobectomy. Aspergillus osteomyelitis is an uncommon infection that rarely occurs sternally. A 70-year-old man who had undergone lobectomy 12 years earlier was admitted to our hospital for chest pain. He was diagnosed with Aspergillus sternomyelitis based on sternal bone culture after an ultrasound-guided percutaneous needle biopsy. The fibrosis and right residual lung apex volume loss had gradually progressed over 12 years, and therefore, chronic pulmonary aspergillosis (CPA) with direct invasion sternal from t

MR Imaging of the Diabetic Foot.

Abnormalities of the peripheral nervous, vascular, and immune systems contribute to the development of numerous foot and ankle pathologies in the diabetic population. Although radiographs remain the most practical first-line imaging tool, magnetic resonance (MR) is the tertiary imaging modality of choice, allowing for optimal assessment of bone and soft tissue abnormalities. MR allows for the accurate distinction between osteomyelitis/septic arthritis and neuropathic osteoarthropathy. Furthermore, it provides an excellent presurgical anatomic road map of involved tissues and devitalized skin t

Circular External Fixation as a Primary or Adjunctive Therapy for the Podoplastic Approach of the Diabetic Charcot Foot.

Numerous techniques have been described for surgical management of the diabetic Charcot foot. External fixation has become a main surgical tool for the reconstructive foot and ankle surgeon when dealing with the ulcerated diabetic Charcot foot. In the presence of an open wound and/or osteomyelitis, staged reconstruction with circular external fixation becomes ideal for salvage of the diabetic lower extremity. Also, circular external fixation can provide simultaneous compression and stabilization, correct the underlying osseous or soft tissue deformities, and surgically offload the diabetic Cha

Soft Tissue Reconstruction Pyramid for the Diabetic Charcot Foot.

Foot and ankle ulcerations in patients with diabetic Charcot neuroarthropathy (DCN) occur frequently and can be challenging to address surgically when conservative care fails. Patients with acute or chronic diabetic foot ulcers (DFU) are at continued risk for development of osteomyelitis, septic arthritis, gas gangrene, and potential lower extremity amputation. Concurrent vasculopathy and peripheral neuropathy as well as uncontrolled medical comorbidities complicate the treatment approach. In addition, pathomechanical forces left untreated may contribute to DFU recurrence in this patient popul

Empyema thoracis due to intrapleural migration of retained vascular catheter.

Intravascular retention and embolization of fragment of central venous catheters is a rare but well-documented complication of in-dwelling vascular access devices and ports. We describe the case of a 39-year-old male with fracture of the central venous catheter during removal of subcutaneous vascular access port. Over the next 3 years, the catheter fragment embolized through the pulmonary circulation and subsequently migrated into the left pleural space. He presented with empyema thoracis associated with thoracolumbar vertebral osteomyelitis. He required video-assisted thoracoscopy for retriev

Incidental findings of massive heel spurs in a veteran with a variant of psoriatic arthritis.

A middle-aged man presented for left foot diabetic ulcer care. Pedal radiographs were negative for signs of osteomyelitis. However, asymptomatic incidental osseous findings demonstrated significant plantar and posterior calcaneal spurring possibly consistent with diffuse idiopathic skeletal hyperostosis (DISH). A differential of DISH, psoriatic arthritis, Reiter's, and ankylosing spondylitis was developed. Subsequent spinal imaging and laboratory work-up did not satisfy the diagnostic criteria for DISH. This case illustrates radiographic changes characteristic of multiple seronegative arthropa

Extranodal natural killer/T cell lymphoma, nasal type presenting as a palatal perforation and naso-oral fistula.

Extranodal natural killer/T cell lymphoma (ENKL), nasal type, a rare disease presenting with vague non-specific symptoms, can impose great diagnostic difficulties and may masquerade several infectious, rheumatological or neoplastic conditions. Here, the authors report a case of ENKL presenting as a palatal perforation, naso-oral fistula and fever in a 21-year-old man, which was initially misdiagnosed as bacterial osteomyelitis, invasive fungal infection and Wegener's granulomatosis. The case report underlines the possibility of ENKL occurring at a young age, its rapidly progressive and locally

Primary frontal sinus carcinoma with extradural anterior cranial fossa involvement.

Paranasal sinus carcinoma is rare, with an estimated annual incidence of less than 1 per 100,000 population. Primary frontal sinus involvement is extremely rare, accounting for only 0.3% of all paranasal sinus malignancies. A frontal sinus cancer may be mistaken for a mucocele, pyocele, or osteomyelitis. We report the case of a 48-year-old woman with a carcinoma that originated in the frontal sinus. Computed tomography and magnetic resonance imaging demonstrated bony destruction with intracranial extension but no involvement of the bulbus. The patient underwent a frontal craniotomy, tumor exci

Septic rupture of the ascending aorta after aortocoronary bypass surgery.

We describe an exceptional case of non-fatal septic rupture of the ascending aorta in a patient with sternal dehiscence, deep sternal wound infection (DSWI) and pleural empyema after aortocoronary bypass surgery. Routine follow-up computed tomography (CT) detected a mediastinal pseudoaneurysm originating from the ascending aorta. Thereby, massive and irregular sternal bone defects and contrast-enhancing mediastinal soft tissue suggest osteomyelitis and highly-active and aggressive DSWI as initial triggers. Urgent thoracotomy 1 day later included ascending aorta reconstruction, total sternum re
