INTRODUCTION: People of Ashkenazi Jewish (AJ) ancestry are more likely than unselected populations to have a BRCA1/2 pathogenic variant, which cause a significantly increased risk of breast, ovarian and prostate cancer. Three specific BRCA1/2 pathogenic variants, referred to as BRCA-Jewish founder mutations (B-JFM), account for >90% of BRCA1/2 pathogenic variants in people of AJ ancestry. This is a personal reflection on my and my family's experience with Telomere Biology Disorders (TBDs). This reviews the journey from identifying a familial pattern with idiopathic pulmonary fibrosis to identifying a genetic cause and a specific gene mutation. It also discusses the journey for the author from wanting to not know whether they had the genetic mutation to deciding to test for TBD and the gene. The reflection also discusses how I have dealt with the grief of losing my father and sister to this disease.

Screening with Fecal Occult Blood Test (FOBT) fails to adequately select patients who could have colorectal tumors and should be sent to colonoscopy. The latter investigation should be considered as the first intention examination in every person aged more than 50 years old (or, 40 years if there is a family history of elevated cancer risk). Colonoscopy will allow to identify the numerous patients who, in spite of significant disease, would have a negative FOBT.

A 5 year follow-up of 130 workers exposed to tetrachlorethylene (dry cleaning) was performed, including measuring of exposure conditions (personal dosimetry, exposure tests) and clinical investigations. No signs of hepatoxicity of tetrachlorethylene could be detected, but slight derangements of nerval functions may develop in long-term exposed workers. Nerve motor and sensory conduction velocity shows a tendency to diminution. Recommendations are derived for biological monitoring and health surveillance of workers exposed to tetrachlorethylene.

In 1887, König claimed that loose bodies in the knee joints of young persons had three causes: (1) very severe trauma; (2) lesser trauma causing contusion and necrosis; and (3) minimal trauma acting on an underlying lesion-for which he suggested the name osteochondritis dissecans. His thesis has stood the test of time. We still confuse the second and third categories but osteochondritis dissecans has been identified as an ossification defect.

Neurological evaluation of a family living in rural Louisiana showed that six living members have splasticity, hyperactive stretch reflexes, and pathological reflexes. Ten deceased family members had been similarly affected. All affected persons were offspring of consanguineous marriages, and sex distribution was equal, indicating that inheritance was autosomal recessive. Although undetected clinically, conduction abnormalities suggestive of demyelination in the visual pathways were revealed by special tests.

One-hundred seventy-five persons viewed test letters of 35 and 11% contrast at 0.1 and 0.01 fL chart luminance while lenses (+0.25 to -1.50 D in 0.25-D steps) were placed before their eyes (and glasses, if they wore them).
