Idiopathic bilateral carpal tunnel syndrome in a 9-month-old infant presenting as a pseudo-dystonia.

BACKGROUND: Carpal tunnel syndrome is the most common focal peripheral neuropathy seen in most electrophysiological laboratories. Although the incidence of carpal tunnel syndrome in adults is 50 to 150 cases per 100,000 people, it is rare in children. There are less than 200 case reports of carpal tunnel syndrome in children, with mucopolysaccharides and mucolipidosis being the most frequent cause. Idiopathic carpal tunnel syndrome with childhood onset occurs in less than 0.2% of cases. PATIENT: We describe a 9-month-old infant who presented with intermittent abnormal posturing movement of bo

An ergonomics study on the prevalence of musculoskeletal disorders among Indian bus conductors.

This study was undertaken among 100 randomly selected bus conductors from 2 routes. A questionnaire study based on the modified Nordic musculoskeletal questionnaire, assessment of physical and physiological parameters, analysis of working postures and a detailed work study were performed. The analysis revealed that conductors had a work schedule of 16-18 h each day; the duration of work could vary from 15 to 20 days at a stretch. Discomfort leading to musculoskeletal disorders mainly affecting the leg (93.3%), knee (83.3%), shoulder (80%) and back areas (56.7%) had the highest 12-month prevale

Comparative Effects of Sertraline and Nortriptyline on Body Sway in Older Depressed Patients.

This study examined effects of nortriptyline and sertraline on the balance and stability of depressed geriatric inpatients. Body sway was measured with a stable force-platform at three timepoints: before starting antidepressant medication, 5-7 days after medication was initiated, and 1 week later. A group of healthy, unmedicated older volunteers was evaluated under the same conditions as patients. In sertraline-treated patients (n = 10), significant differences (P < 0.05) between baseline and the first week of treatment were found in the force-platform measurements of sway length (L) and area

Pure akinesia with low myocardial metaiodobenzylguanidine uptake.

The objective of this study was to delineate the clinical features of patients with pure akinesia and a low heart versus mediastinum ratio below 2.0 in the delayed images of myocardial metaiodobenzylguanidine scintigraphy, a marker for the clinical diagnosis of Lewy body diseases. A total of 13 patients was enrolled. The clinical features of the patients were uniformly elderly (mean age, 73.7 years), a male gender preponderance (10 men and 3 women), relatively mild motor disability (mean Hoehn Yahr grade 2.4) without laterality, festinating gait (77%) and loss of postural reflex (69%), which w

Intracranial hypotension in a girl with Marfan syndrome: case report and review of the literature.

INTRODUCTION: A 14-year-old girl, followed in our department for Marfan syndrome, presented with postural headache for a month. Neurological examination was normal. The diagnosis of intracranial hypotension syndrome was suspected. DISCUSSION: Bilateral subdural hematomas were found on brain magnetic resonance imaging (MRI), and spinal MRI showed large lumbosacral arachnoid diverticula; no cerebrospinal fluid leaks could be found. Despite bed rest and hydration for 2 weeks, postural headache remained. Epidural blood patching was also performed. Subsequently, the patient became asymptomatic and

Biomechanical assessment of three rebar tying techniques.

The National Institute for Occupational Safety and Health (NIOSH) conducted a study of ironworkers to evaluate their risk for developing back and hand injuries from hand-tying reinforcing steel bar and to investigate whether power tying tools can be an effective intervention for the prevention of work-related musculoskeletal disorders. A field investigation of biomechanical loading when using 3 techniques to tie together rebar was conducted. Researchers measured employees' wrist and forearm movement with goniometers and videotaped and analyzed trunk postures. Manually tying rebar at ground lev

Effects of experimental muscle pain on electromyographic activity of masticatory muscles in the rat.

The aim of the present study was to investigate the effects of noxious chemical stimulation of a jaw muscle on postural electromyographic (EMG) activity from several masticatory muscles in lightly anesthetized rats. Unilateral injection of a substance known to induce acute muscle pain (5% NaCl) or longer duration of pain with inflammation (mustard oil) was made into the masseter muscle. The changes in EMG activity following the injection were recorded from the injected and contralateral masseter muscles and the ipsilateral digastric muscle. The algesic chemicals produced a significant but tran

Musculoskeletal problems among VDU workers in a Hong Kong bank.

A survey of musculoskeletal problems among visual display unit (VDU) users was carried out in a bank using a self-administered questionnaire. The prevalence of complaints in various body parts were: neck--31.4%, back 30.6%, shoulder--16.5%, hand and wrist--14.9% and arm--6.6%. Frequent users of VDU had significantly more musculoskeletal problems in the neck and shoulder regions than infrequent users. Individual musculoskeltal complaints were associated with various risk factors including personal attributes, working posture, repetitive movements and work station design. Back, neck and shoulder

Using self-view television to distinguish between self-examination and social behavior in the bottlenose dolphin (Tursiops truncatus).

In mirror mark tests dolphins twist, posture, and engage in open-mouth and head movements, often repetitive. Because postures and an open mouth are also dolphin social behaviors, we used self-view television as a manipulatable mirror to distinguish between self-examination and social behavior. Two dolphins were exposed to alternating real-time self-view ("mirror mode") and playback of the same to determine if they distinguished between them. The adult male engaged in elaborate open-mouth behaviors in mirror mode, but usually just watched when played back the same material. Mirror mode behavior

Familial paroxysmal kinesigenic ataxia and continuous myokymia.

A large family with paroxysmal ataxia and continuous myokymic discharges is described. The disorder is of autosomal dominant inheritance. During attacks coordination of movements and balance are disturbed; often a postural tremor of the head and the hands and fine twitching in some of the facial and hand muscles are present. The attacks usually last a few minutes and may occur several times per day. They first appear in childhood and tend to abate after early adulthood. The attacks are frequently precipitated by kinesigenic stimuli similar to those in paroxysmal kinesigenic choreoathetosis. Th

The loads on the lumbar spine during work at an assembly line. The risks for fatigue injuries of vertebral bodies.

This study was performed in an attempt to determine the total spinal compressive load during assembly line work to find a possible association with the many complaints of back pain. A flexion analyzer was used to register trunk movements, and analysis of postures and lifted weights was done from video recordings. The load on the spine at the L3 level was calculated through a biomechanical model, meant for analysis of static, sagittally symmetric postures and lifting tasks. Maximum lift tests were performed before and after a full work day. The peak load on the L3-L4 level when lifting correspo

Effect of neuroleptics and of potential new antipsychotic agents (MJ 13859-1 and MJ 13980-1) on a monkey model of tardive dyskinesia.

Two adult female cebus apella monkeys with persistent tardive dyskinesia (TD) were given acute i.m. injections of reference neuroleptics (chlorpromazine, haloperidol, thioridazine, and clozapine) or of potential new antipsychotic agents (MJ 13859-1 and MJ 13980-1). The drugs were assessed for their ability to modify TD symptoms or to produce acute neurologic reactions. Effects of three doses of MJ 13859-1 administered orally were also examined. At the doses used, thioridazine and clozapine had little or no effect. Chlorpromazine, haloperidol, MJ 13859-1 and MJ 13980-1 reduced or abolished TD a

Autonomic nervous dysfunction in severe pre-eclampsia.

We evaluated autonomic nervous function in 14 subjects with severe pre-eclampsia and 11 subjects with normal third-trimester pregnancies using standard cardiovascular tests, i.e. heart rate responses to deep breathing and standing up, and blood pressure response to standing up. Both of the heart rate responses were lower in subjects with severe pre-eclampsia than in the control group (P less than 0.01 and P less than 0.001, respectively). Four subjects with severe pre-eclampsia had postural hypotension, defined as a systolic blood pressure fall of at least 30 mmHg on standing. Our results show

[Normal and abnormal response of the arterial pressure to standing (author's transl)].

Redistribution of blood volume during orthostatism is followed by a reduction in venous return and therefore cardiac output. A drop in the blood pressure following postural changes is normally prevented by the action of the baroreceptor reflex, which leads to an increase in total peripheral resistance and heart rate. Orthostatic hypotension may arise from a variety of causes. In some cases, the disorder of postural regulation is due to an autonomic nervous system disease, and various tests are available that enable the diagnosis to be confirmed and the mechanism of action to be elucidated. Incidence of orthostatic hypotension in patients with primary affective disorders treated with tricyclic antidepressants.

Previous studies have indicated that postural hypotension is an uncommon event when tricyclic antidepressants are used in the treatment of depression, and other studies have indicated that postural hypotension is a possible predictor of positive therapeutic response to antidepressant therapy. In this study, 20 depressed patients with the diagnosis of primary affective disorders were hospitalized and treated with tricyclic antidepressants. All patients had been without medication for at least 2 weeks before the study began. Blood pressure recordings were made after a 5-minute resting period and

Ergonomic evaluation of passive lower-limb exoskeletons in simulated industrial tasks at non-optimal workstations.

BackgroundIndustrial tasks often require workers to maintain awkward static postures at non-optimal workstations, leading to physical strain and musculoskeletal issues. Exoskeletons offer potential ergonomic benefits by reducing physical workload. However, the efficacy of passive lower-limb exoskeletons in non-optimal workstation conditions remains unclear.ObjectivesThis study examines the ergonomic efficacy of a passive lower-limb exoskeleton during simulated industrial tasks performed at workstations with varying heights and tilt angles.MethodsSixteen participants performed bolting tasks at

Development and Content Validity of the Movement Compensatory Screening (MoCS): A Structured Framework for Identifying Compensatory Strategies After Stroke.

BACKGROUND: Compensatory movement strategies are common after stroke and vary across individuals and tasks. However, the lack of a structured framework to guide movement analysis during mobility and postural control tasks may limit consistent identification of these strategies across clinicians in clinical practice. OBJECTIVE: To develop and examine the content validity of the Movement Compensatory Screening (MoCS) to identify compensatory movement strategies during mobility and postural control tasks after stroke. METHODS: Following the Delphi method and COSMIN guidelines, the MoCS was deve

A Lightweight Multi-Articular Passive Exoskeleton Using a Single Elastic Band to Improve Crouch Gait Pattern: A Pilot Study.

Crouch gait, characterized by excessive flexion of the lower-limb joints, is a common gait disorder among children with cerebral palsy (CP) that compromises mobility and increases energy expenditure. Although surgical and orthotic interventions can improve alignment, maintaining these benefits requires continued postural support and gait practice. While powered exoskeletons have been developed to support upright and coordinated movement, they are often too heavy and complex for practical use. To address this, we developed a lightweight passive exoskeleton that assists the hip, knee, and ankle

Influence of limb dominance on compensatory foot contacts during unilateral standing in adults with reduced plantar sensation.

BACKGROUND: Postural stability during unilateral stance is an important indicator of functional independence and fall risk in older adults. However, the compensatory strategies used to maintain balance under conditions of sensory impairment remain unclear. This study investigated how limb dominance affects postural control, which was measured by the duration and frequency of contralateral foot contacts during unilateral stance in adults with reduced plantar sensation. METHODS: Thirty-eight right-limb dominant individuals with reduced plantar foot sensation and 33 control participants were inc

High-Frequency Spinal Cord Stimulation for the Treatment of Spasticity: A Preliminary Case Series.

Background: Spasticity is a complex and multifactorial condition resulting from upper motor neuron injury. It manifests through muscle contractions, pain, limited range of motion, and clonus, which significantly impair daily activities and quality of life. High-frequency spinal cord stimulation (HF SCS) has shown optimal results in treating chronic neuropathic pain, but its potential role in spasticity remains underexplored. This study aimed to evaluate the efficacy of HF SCS in patients with spasticity. Methods: From April 2021 to July 2024, six patients with spasticity from various etiologie
