Surgical abdomen masked by metabolic crisis: fulminant ischemic colitis in severe diabetic ketoacidosis - a case report and literature review.

BACKGROUND: Abdominal pain and gastrointestinal symptoms are common in diabetic ketoacidosis (DKA) and are generally reversible with metabolic correction. However, persistent or worsening abdominal findings should prompt concern for an underlying intra-abdominal emergency. Intestinal ischemia is an uncommon complication of severe hyperglycemic crisis in young patients but may progress rapidly to transmural necrosis due to profound hypovolemia and compromised microvascular perfusion. CASE PRESENTATION: A 30-year-old woman with type 1 diabetes mellitus presented with a 3-day history of diarrhea

Inflammation-Based Prognostication in Advanced-Stage NSCLC: A Retrospective Cohort Study.

Background/Objectives: Neutrophil-to-lymphocyte ratio (NLR), a marker of systemic inflammation, has prognostic value in non-small cell lung cancer (NSCLC), but its longitudinal performance in routine care is unclear. We evaluated baseline and 12-month changes in NLR and hemoglobin in a single-center, Eastern European cohort. Methods: In this retrospective study, 180 adults with histologically confirmed NSCLC, diagnosed May 2022-April 2024 at a Romanian tertiary center, were followed until 30 April 2025. Baseline demographics, tumor characteristics, molecular profiles, laboratory parameters, an

Effectiveness of a Long-Term, Home-Based Aerobic Exercise Intervention on Slowing the Progression of Parkinson Disease: Design of the Cyclical Lower Extremity Exercise for Parkinson Disease II (CYCLE-II) Study.

OBJECTIVE: Previous short duration studies have demonstrated that high-intensity aerobic exercise improves aspects of motor and non-motor function in people with Parkinson disease (PwPD); however, the effectiveness of a long-term exercise intervention on slowing disease progression is unknown. The primary aim of this study is to determine the disease-altering effects of high-intensity aerobic exercise, administered on an upright stationary cycle, on the progression of PD. A secondary aim is to develop a prognostic model for 12-month changes in the Movement Disorder Society Unified Parkinson's

A Pilot Study of Lenalidomide Maintenance Therapy after Autologous Transplantation in Relapsed or Refractory Classical Hodgkin Lymphoma.

For patients with relapsed or refractory classical Hodgkin lymphoma (cHL), salvage chemotherapy followed by consolidation with autologous stem cell transplant (ASCT) remains the standard of care. Even with this aggressive treatment strategy, 5-year progression-free survival is ≤50%, and there remains interest in maintenance strategies to improve long-term disease-free survival. Lenalidomide is an immunomodulatory agent with demonstrated activity in multiple subtypes of lymphoma including cHL, and has also been shown to improve both progression-free and overall survival as maintenance therapy a

Clinical Outcomes in Initial Hirudotherapy versus Standard Treatment in Idiopathic Granulomatous Mastitis: A Comparative Cohort Study.

OBJECTIVE: Idiopathic granulomatous mastitis (IGM) is a chronic inflammatory disease characterized by a high risk of recurrence and fistula formation. Spontaneous remission has been reported in up to 50% of cases, particularly in mild forms, within 6-24 months without intervention, but inadequate management may allow disease progression. The aim of this study was to compare the clinical outcomes of patients who initially preferred hirudotherapy (leech therapy) with those managed with evidence-based stepwise conventional care. MATERIALS AND METHODS: We retrospectively analyzed histopathologica

Cost-Effectiveness of Pembrolizumab With Chemoradiotherapy for Locally Advanced Cervical Cancer.

IMPORTANCE: The KEYNOTE-A18 trial demonstrated that adding concurrent and adjuvant pembrolizumab to chemoradiotherapy and brachytherapy significantly improved survival in patients with newly diagnosed, locally advanced cervical cancer. However, considering the annual global incidence of 660 000 cases of cervical cancer, including 13 820 in the US in 2024, incorporating this regimen into the standard of care could have substantial health care economic implications for both patients and the health care system. OBJECTIVE: To determine the cost-effectiveness of adding pembrolizumab to the first-l

Pattern of disease recurrence and outcomes after progression of high-risk renal cell carcinoma (RCC) patients treated with adjuvant immunotherapy.

BACKGROUND: Radical or partial nephrectomy followed by adjuvant pembrolizumab is the standard of care for high-risk localized renal cell carcinoma (RCC), yet around 40% of patients relapse within 5 years. We investigated patterns of disease recurrence and the clinical management of RCC patients treated with adjuvant immunotherapy. MATERIALS AND METHODS: We collected patients with high-risk RCC who received adjuvant immunotherapy after radical surgery in our Institution. The primary endpoint was the rate and pattern of disease recurrence. Secondary endpoints were disease-free survival (DFS), o

Telotristat ethyl for the treatment of carcinoid syndrome diarrhea not controlled by somatostatin analogues.

Telotristat ethyl (Xermelo), developed by Lexicon Pharmaceuticals, is an oral tryptophan hydroxylase inhibitor blocking peripheral conversion of tryptophan to serotonin (5-hydroxytryptamine [5-HT]). It was approved by the U.S. Food and Drug Administration (FDA) in February 2017 and by the European Commission in September 2017 for patients with carcinoid syndrome in whom diarrhea is not adequately controlled by somatostatin analogues (SSAs). Diarrhea, secondary to the release of serotonin, is the predominant gastrointestinal symptom in patients with carcinoid syndrome and has a significant impa

The mucopolysaccharidoses and anaesthesia: a report of clinical experience.

Patients with the mucopolysaccharidoses show widespread, progressive involvement and derangement of many organs and tissues which can have profound implications for the anaesthetist. These disorders are uncommon and few anaesthetists care for these patients on a regular basis although individual patients often undergo multiple anaesthetics for procedures intended to improve their quality of life. There is a relative paucity of literature dealing with clinical anaesthetic experience with these patients. We report a retrospective review of the recent experience at the Children's Hospital of East

Labor Intervention and Outcomes in Women Who Are Nulliparous and Obese: Comparison of Nurse-Midwife to Obstetrician Intrapartum Care.

BACKGROUND: Women who are obese have slower labors than women of normal weight, and show reduced response to interventions designed to speed labor progress like oxytocin augmentation and artificial rupture of membranes. The optimal labor management for these women has not been described. METHODS: This retrospective cohort study compared 2 propensity score-matched groups of women (N = 360) who were healthy, nulliparous, spontaneously laboring, and obese (body mass index ≥ 30 kg/m2 ). Labors were managed by either a certified nurse-midwife (CNM) or an obstetrician at one hospital from 2005 thro

Multimodal treatment with thiotepa, bevacizumab, teniposide, and tunlametinib in a patient with neurofibromatosis type 1-associated oligodendroglioma: a rare case report.

Neurofibromatosis type 1 (NF1) is a common autosomal dominant disorder associated with central nervous system gliomas, most frequently optic pathway gliomas; however, oligodendrogliomas in the setting of NF1 are exceedingly rare, with no prior documented cases and no established treatment strategies to date. A 53-year-old female with NF1-associated oligodendroglioma experienced multiple recurrences following surgery, radiotherapy, chemotherapy, and targeted therapy. Upon further disease progression, she was treated with a novel combination of thiotepa, bevacizumab, teniposide, and the mitogen-

Mechanistic overview and suggested strategies to overcome BCL-2 inhibitor resistance in TP53-mutated acute myeloid leukemia.

Venetoclax is a selective BCL-2 inhibitor that has transformed the treatment landscape for elderly and unfit patients with acute myeloid leukemia (AML). Its use has also been progressively extended to include TP53-mutated AML based on clinical outcomes comparable to other available standards of care. However, TP53-mutated AML is associated with high rates of primary and acquired resistances to venetoclax combinations, which have not afforded any meaningful gains to overall survival. The main causes for these limitations include profound genomic instability, loss of p53 pleotropic function, an

Rethinking diabetic foot ulcer care: Integrating exercise and physical activity.

Because repetitive trauma and pressure contributes to diabetic foot ulcer (DFU) development and progression, offloading is one of the main therapies used in treatment. The term offloading has become conflated with a reduction in movement altogether, increasing sedentary time and, with it, the risk of associated adverse health outcomes. The aim of this narrative review is to evaluate the literature on movement-based interventions (reducing sedentary time, increasing physical activity (PA), or increasing exercise) in DFU patients. We found 16 articles that looked at the effect of exercise in ind
