Neurophysiology of the lower urinary tract.

The lower urinary tract (LUT) has two functions: (1) the storage of waste products in the form of urine and (2) the elimination of those wastes through micturition. The LUT operates in a simple "on-off" fashion, either storing urine or releasing it during voiding. While this activity may seem simple, micturition is controlled by a complex set of peripheral neurons that are, in turn, coordinated by cell groups in the spinal cord, brainstem, and brain. When this careful coordination is interrupted, the control of the bladder is lost, resulting in incontinence or retention of urine. The purpose o

Dilated intercellular spaces and chronic cough as an extra-oesophageal manifestation of gastrooesophageal reflux disease.

Chronic cough is one of the extra-oesophageal manifestations of gastrooesophageal reflux disease (GORD). It is presumed to occur either directly by microaspiration of acidic gastric contents into the airway or indirectly by a reflex triggered by contact of acidic refluxates with the oesophageal epithelium in GORD. How contact of the oesophageal epithelium with acidic refluxates promotes sensitization for chronic cough is unknown, but like heartburn, which is a necessary accompaniment, it requires acid activation of nociceptors within the oesophageal mucosa. Dilated intercellular spaces within

Bladder and bowel dysfunction in Parkinson's disease.

Bladder dysfunction (urinary urgency/frequency) and bowel dysfunction (constipation) are common non-motor disorders in Parkinson's disease (PD). In contrast to motor disorder, the pelvic autonomic dysfunction is often non-responsive to levodopa treatment. Brain pathology mostly accounts for the bladder dysfunction (appearance of overactivity) via altered dopamine-basal ganglia circuit, which normally suppresses the micturition reflex. In contrast, peripheral enteric pathology mostly accounts for the bowel dysfunction (slow transit and decreased phasic contraction) via altered dopamine-enteric

Effects of in utero meso-2,3-dimercaptosuccinic acid with calcium and ascorbic acid on lead-induced fetal development.

To examine the effects of meso-2,3-dimercaptosuccinic acid (DMSA) on developmental toxicity resulting from exposure to lead in utero, female albino mice were exposed to lead by drinking water contaminated with lead acetate for 4 weeks. After the cessation of lead exposure, female mice were supplemented by gavage with saline solution, DMSA, or DMSA and calcium as well as ascorbic acid from the fourth day of gestation until parturition, respectively. Lead levels (blood, liver, and bone) were measured at birth. Pups were then tested about neural development including surface righting reflex, clif

Cortical tremor (FCMTE: familial cortical myoclonic tremor with epilepsy).

For 15 years, 50 Japanese and European families with cortical myoclonic tremor and epilepsy were reported in the literature under various names. More recently, the acronym familial cortical myoclonic tremor with epilepsy (FCMTE) has been proposed for this new clinical entity based on both clinical and electrophysiological criteria: irregular postural myoclonic tremor of the distal limbs, familial history of epilepsy, autosomal dominant inheritance, and a rather benign outcome. The diagnosis is confirmed by electrophysiological features favoring cortical reflex myoclonus (enhanced C reflex at r

Receptors and transmission in the brain-gut axis: potential for novel therapies. V. Fast and slow extrinsic modulation of dorsal vagal complex circuits.

Vago-vagal reflex circuits in the medulla are responsible for the smooth coordination of the digestive processes carried out from the oral cavity to the transverse colon. In this themes article, we concentrate mostly on electrophysiological studies concerning the extrinsic modulation of these vago-vagal reflex circuits, with a particular emphasis on two types of modulation, i.e., by "fast" classic neurotransmitters and by "slow" neuromodulators. These examples review two of the most potent modulatory processes at work within the dorsal vagal complex, which have dramatic effects on gastrointest

Impulsive testing of individual semicircular canal function.

In order to test the human angular vestibulo-ocular reflex in the dynamic range of normal head movements, we measured 3-dimensional compensatory eye-movement responses to low-amplitude (10-12 degrees), high-acceleration (3000-4000 degrees/s/s), passive, manually delivered head rotations (head "impulses") in the three planes of the semicircular canals in normal subjects, in subjects who had recovered from surgical unilateral vestibular deafferentation, and in patients after acute unilateral peripheral vestibulopathy, that is, from vestibular "neuritis." We found that canal-plane head impulses a

Neural blockade during exercise augments central command's contribution to carotid baroreflex resetting.

This investigation was designed to determine central command's role on carotid baroreflex (CBR) resetting during exercise. Nine volunteer subjects performed static and rhythmic handgrip exercise at 30 and 40% maximal voluntary contraction (MVC), respectively, before and after partial axillary neural blockade. Stimulus-response curves were developed using the neck pressure-neck suction technique and a rapid pulse train protocol (+40 to -80 Torr). Regional anesthesia resulted in a significant reduction in MVC. Heart rate (HR) and ratings of perceived exertion (RPE) were used as indexes of centra

Complex regional pain syndrome of the breast in a patient after breast reduction.

Complex regional pain syndrome (CRPS) is characterized by devastating pain, swelling, and cutaneous discoloration that result from vasomotor dysfunction caused by an abnormally accelerating sympathetic loop reflex after trauma or surgery. Although in the extremities CRPS is well documented as reflex sympathetic dystrophy, it only has been reported anecdotally in the breast after modified radical mastectomy and never reported after breast reduction. We report CRPS in the right breast of a 27-year-old woman after revision breast reduction surgery. The patient had signs of CRPS and symptoms of pa

[Epiphyseal pain from cyclosporine in renal transplants].

In patients with renal transplant, cyclosporin has been implicated in the occurrence of osteoarticular pain. This syndrome, which we illustrate by two of our observations, is fairly stereo-typed. Osteoarticular pain begins around the second month post-transplant and in a symmetrical pattern involves, knees, ankles, tarsi, less frequently hips, and almost never upper limbs. Pain arises on standing and walking, which is severely impeded. Clinical examination is usually normal. Radiographs show patchy, subchondral osteopenia. Bone scintiscan documents numerous foci of increased uptake and MRI mul

[Evaluation and treatment of dysphagia in patients with cerebrovascular lesions and cranial injuries].

This study was designed to evaluate swallowing disorders and their evolution after rehabilitation treatment in a group of pts with cerebrovascular accident (CVA) localized in the brainstem (8 pts), cerebral hemisphere (8 pts) and in a group of pts with cranial trauma (8 pts). The diagnosis of swallowing disorders was based on videofluorography (VFG); rehabilitation treatment on Logemann's technique. All pts exhibited a variety of disturbances usually occurring in combination rather than as isolated disorders. The most frequent problem was a delayed swallowing reflex which was found in all pts

Food-procuring behavior of rats under the conditions of chronic activation and blockade of the neostriatal dopaminergic system.

Using the technique of intracerebral microinjections, the features of food-procuring behavior (the realization of a situational instrumental conditioned reflex in a Skinner box) were investigated in rats in experiments under the conditions of a pharmacological influence on the dopaminergic system of the neostriatum. Amphetamine in a dose of 15 and 45 micrograms and haloperidol in a dose of 5 mg were injected daily over the course of three weeks bilaterally into the rostral division of the neostriatum. A amphetamine dose of 15 micrograms was ineffective, but a increase in motor hyperactivity, a

Varied influence of damage to the motor cortex on precision avoidance and escape reactions in dogs.

The influence of local extirpation of the representation of the forelimb in the motor area of the cortex on the performance of precision elevations of this limb was investigated. Even imprecise avoidance reactions to an acoustic signal did not recover spontaneously. Irregular motoric reactions which were insufficient in amplitude could easily be restored in the course of retraining, but precise movements (both rapid fused, as well as relatively slow stepwise movements) did not recover for at least a half year after the operation. By contrast with this, precise rapid reactions in response to el

Contralateral suppression of TEOAE. Attempts to find a latency.

Latency of the contralateral suppression of transient-evoked otoacoustic emissions (TEOAE) has been investigated using the ILO88 system for ipsilateral stimulation and recording, and a laboratory computer interface producing contralateral noise bursts. Reduction of rms amplitude in the last 10.24 ms of the response was found to produce the most consistent indication of suppression. Onset latencies varied from less than 40 ms to 140 ms, offset latency from 20 ms to more than 80 ms. Acoustic reflex latencies have not been reported with such short latencies and this makes it unlikely that this re

The alien hand and related signs.

Alien limb sign includes failure to recognise ownership of one's limb when visual cues are removed, a feeling that one body part is foreign, personification of the affected body part, and autonomous activity which is perceived as outside voluntary control. Although the hand is most frequently affected, any limb or combination of limbs may fulfil the alien limb criteria. Alien hand sign should be reserved for cases in which the hand feels foreign together with observable involuntary motor activity. To characterise this phenomenon, seven patients with alien hand sign and other motor or behaviour

[Neuromotor dyskinesia occurring during repeated injections of enkephalins into the rat striatum].

The effect of 3-week daily injection of 5 or 15 mg enkephalin-like tetrapeptides (TP) into the rostral neostriatum was studied in chronic experiments on rats with implanted microcannulas. According to the type of TP, hyperkinesia or a pathological cataleptic status was registered in disorder of conditioned reflex behavior of rats. Myoclonus of the muscles of the face and the muscles of the front limbs was encountered from the 10th till the 120th minute after the injection and was characterized by dynamic changes during chronic administration of the agent and clearly defined stage of generaliza

Hearing in the elderly: the Framingham cohort, 1983-1985. Part I. Basic audiometric test results.

Many studies have documented the decline in auditory function with age. We broaden that data base in this the first of a series of reports emanating from the auditory testing of the Framingham cohort during biennial exam 18. The results of the auditory questionnaire, hearing sensitivity, acoustic compliance measures, and word recognition tests obtained from 1662 men and women in their 60th through 90th decades are presented. Pure-tone thresholds increased with age but the rate of change with age did not differ by gender even though men had poorer threshold sensitivity. Maximum word recognition
