Nutritional, metabolic and genetic considerations to optimise regenerative medicine outcome for knee osteoarthritis.

Knee osteoarthritis (KOA) is a multifactorial degenerative disorder of joints, affecting the world's population over the age of 65 and with a higher prevalence in females. KOA is responsible for many age associated joint problems such as stiffness and pain. Conventional methods for managing KOA such as nonsteroidal anti-inflammatory drugs (NSAID) may not improve pain or alter the disease progression and may have adverse side effects. Non-pharmacological management of OA is fundamental to management of functional limitations and provides effective symptom relief but has not shown that disease p

A qualitative exploration of enablers for hepatitis B clinical management among ethnic Chinese in Australia.

An estimated 18% of people living with chronic hepatitis B (CHB) in Australia were born in China. While guideline-based care, including regular clinical monitoring and timely treatment, prevent CHB-related cirrhosis, cancer and deaths, over three-quarters of people with CHB do not receive guideline-based care in Australia. This qualitative study aimed to identify enablers to engagement in CHB clinical management among ethnic Chinese people attending specialist care. Participants self-identified as of Chinese ethnicity and who attended specialist care for CHB clinical management were interviewe

Individual versus team-based case-management for clients of opioid treatment services: an initial evaluation of what clients prefer.

INTRODUCTION AND AIMS: Case-management is a client-centred intervention to improve the coordination and continuity of delivery of services for people with complex needs. This service has been incorporated into opioid treatment programs in various ways. This study was undertaken to compare two case-management models, termed individual case-management (ICM) and team-based case-management (TBCM). This study aims to describe the new TBCM and client attitudes to, and acceptance of, this model compared with ICM. DESIGN AND METHODS: Clients from two opioid treatment programs, one implementing ICM an

Acceptability, feasibility and appropriateness of integrating HPV self-sampling for cervical cancer screening into voluntary family planning services in Malawi.

INTRODUCTION: Despite being preventable, the cervical cancer burden remains high in Malawi and other low-income countries. The World Health Organization recommends cervical cancer screening (CCS) using human papillomavirus (HPV) testing. The coverage of HPV-based CCS is low but may increase with self-sampling of vaginal fluid. We assessed the acceptability, feasibility and appropriateness of two models for integrating HPV self-sampling for CCS into family planning (FP) services in Malawi. METHODS: We conducted a mixed-methods study nested within a 1:1 cluster randomized trial comparing two ser

Mechanical ventilation before and during veno-venous extracorporeal membrane oxygenation.

INTRODUCTION: Veno-Venous Extracorporeal Membrane Oxygenation (VV ECMO) has revolutionized severe acute respiratory distress syndrome (ARDS) management by decoupling gas exchange from native lung function, minimizing ventilator-induced lung injury (VILI). However, optimizing mechanical ventilation before and during extracorporeal support remains unstandardized. AREAS COVERED: Pre-ECMO management heavily influences survival, requiring clinicians to balance the cumulative 'dose' of injurious ventilation against cannulation timing. On ECMO, practice remains divided between an 'open lung approach

Bio-inspired Dual-Mode Cooling Hydrogel Composite for Passive Heat Dissipation of Photovoltaic Panels.

Photovoltaic (PV) panels experience pronounced efficiency loss and reliability degradation at elevated operating temperatures, creating an urgent demand for efficient thermal management. Conventional active cooling approaches, however, are constrained by high energy consumption and system complexity, restricting their applicability. Inspired by the thermoregulation strategy of the land snail Theba pisana, this work reports a dual-mode cooling hydrogel composite that integrates evaporative and radiative cooling in one material system, enabling passive thermal management of PV panels. The hydrog

Anticoagulation monitoring using point-of-care testing combined with mobile remote management in postoperative cardiac valve replacement patients receiving warfarin therapy: A single-center cross-sectional study.

Lifelong anticoagulation monitoring is essential for mechanical heart valve recipients. While conventional hospital-based monitoring creates significant patient and system burdens, combining mobile remote monitoring (MRM) with point-of-care testing (POCT) may provide a more efficient solution. This study evaluated whether MRM + POCT enhances warfarin therapy quality and patient outcomes. This study compared conventional anticoagulation management versus MRM + POCT in 148 post-valve replacement patients (74 per group). The conventional cohort underwent standard in-hospital testing and received

Arthrocentesis versus occlusal coverage splints in the management of disc displacement without reduction: a systematic review and meta-analysis.

Disc displacement without reduction (DDwoR) can cause pain and limitations in mouth opening, with a significant impact on function. The optimal management strategy for DDwoR is unclear. Treatments include conservative management such as mandibular manipulation, occlusal splints, and patient education/self-management, as well as arthrocentesis, which is a minimally invasive procedure. The aim of this systematic review and meta-analysis was to ascertain whether there is a role for arthrocentesis in the management of DDwoR. Studies analysing the outcomes pain and maximum mouth opening (MMO) in pa

Integrated image-based deep learning and language models for primary diabetes care.

Primary diabetes care and diabetic retinopathy (DR) screening persist as major public health challenges due to a shortage of trained primary care physicians (PCPs), particularly in low-resource settings. Here, to bridge the gaps, we developed an integrated image-language system (DeepDR-LLM), combining a large language model (LLM module) and image-based deep learning (DeepDR-Transformer), to provide individualized diabetes management recommendations to PCPs. In a retrospective evaluation, the LLM module demonstrated comparable performance to PCPs and endocrinology residents when tested in Engli

Roles for community health workers in diabetes prevention and management in low- and middle-income countries.

Diabetes prevalence is increasing worldwide, especially in low- and middle-income countries (LMIC), posing the need for improved detection and management strategies. Chronic disease models and lifestyle medicine provide structures for action. Community health workers (CHWs) can significantly contribute to chronic disease care if they are trained and integrated into low-resource health systems. Although most current CHWs worldwide are performing maternal/child health and infectious disease-related tasks, other programs involving CHWs for noncommunicable disease prevention and management are inc
