Methamphetamine promotes methicillin-resistant Staphylococcus aureus subcutaneous infection through innate immune dysregulation and altered expression of biofilm-associated genes.

Methamphetamine (METH) use is associated with an elevated risk of skin and soft tissue infections caused by methicillin-resistant Staphylococcus aureus (MRSA), yet its direct effects on MRSA pathogenesis remain poorly characterized. Using a BALB/c murine subcutaneous infection model, we demonstrate that METH significantly increases abscess area and bacterial burden compared to untreated controls. Histological analysis revealed markedly elevated neutrophil infiltration and a concurrent reduction in macrophage recruitment in METH-treated animals, indicating dysregulation of innate immune coordin…

Limb Salvage in Acute Limb Ischemia and Diabetic Foot Gangrene Associated with Pancreatic Acinar Cell Carcinoma-Related Hypercoagulability.

Malignancy-associated hypercoagulability may cause arterial thrombotic events and acute limb ischemia. When combined with diabetic vasculopathy, rapid progression to gangrene and severe infection may occur. We report a patient with type 2 diabetes who developed acute limb ischemia and progressive diabetic foot gangrene. Despite prior endovascular therapy, severe infection with necrotizing soft tissue infection and osteomyelitis developed. A staged limb-salvage approach involving aggressive debridement, removal of infected bone, pedicled flap reconstruction, antimicrobial therapy, anticoagulati

Pyometrocolpos: A Case Series and Proposed Management Framework for the Pediatric Infectious Diseases Consultant.

BACKGROUND: Congenital anomalies of the female reproductive tract commonly present during adolescence with abdominal pain secondary to hematometrocolpos. At the time of diagnosis, many patients will have signs and symptoms concerning for infection of retained blood products, termed pyometrocolpos. Cases of pyometrocolpos have rarely been described, posing challenges for the pediatric infectious diseases consultant in determining the risk of true infection and appropriate antimicrobial therapy. CASE SUMMARIES: This case series first describes a 12-year-old female with a history of left renal a

Prophylactic use of wound care products as a barrier in high-risk vascular surgery patients with underlying prosthetic materials prone to deep space infections.

ObjectivesProsthetic infection is a feared complication following vascular surgery and is associated with significant morbidity. Recently, biologic wound care adjuncts have been used more given their advantageous effects on wound healing. The goal of this study is to evaluate the outcomes of patients who underwent placement of biologic wound care products over prosthetics with the goal of observing reduced deep dehiscence involving the prosthetic or prosthetic infections.ResultsFrom June 1, 2023 to June 26, 2024, 13 patients met criteria for inclusion. Ten of the 13 (77%) involved placement of

Medial Femoral Condyle Flap as Salvage for Osteomyelitic Clavicle Nonunion.

Clavicle nonunion with osteomyelitis is a challenging condition, combining bone infection and structural defects. Traditional 2-stage procedures address these issues but may be unsuitable for patients unable to undergo multiple operations. The medial femoral condyle (MFC) flap has emerged as an effective option, offering both vascularized bone grafting and mechanical support in a single-stage approach, integrating infection control and bone reconstruction. We report a 21-year-old man with right clavicle nonunion and osteomyelitis following internal fixation for a fracture sustained in a motorc

Epithelium-specific response of cultured keratinocytes to infection with adenovirus type 2.

Adenoviruses are pathogenic for certain stratified squamous epithelia. The sites most frequently involved are the upper respiratory tract and oropharynx. Adenovirus infections of the epidermis are quite rare. We examined the virus-cell interactions of adenovirus type 2 (Ad2) and cultured human keratinocytes grown from a variety of body sites. Our intent was to explore the nature of the apparent epithelium-specific susceptibility to Ad2. In brief, we found that in vitro viral susceptibility of the keratinocytes could be reliably predicted based on whether the cells originated from an epidermal

Reconstruction of chronic Achilles tendon ruptures using a doubled peroneus longus tendon free graft and interference screw fixation: a technical note.

BACKGROUND: The surgical management of chronic Achilles tendon ruptures (CATR) with significant tissue defects (Myerson Type II/III) presents a clinical challenge. Traditional reconstructive methods relying on free hamstring autografts or flexor hallucis longus transfers are often limited by donor-site morbidity, infection risks, or are unavailable in patients with a history of anterior cruciate ligament (ACL) reconstruction. This technical note describes a refined, tissue-preserving surgical technique utilising a doubled free autologous peroneus longus (PL) tendon graft. METHODS: Under regio

Purpura Fulminans Mimicking NSTI: A Limb-saving case of Haemophilus influenzae Infection.

BACKGROUND: Purpura fulminans is a life-threatening thrombotic disorder characterized by disseminated intravascular coagulation and hemorrhagic skin necrosis. Severe cases may be complicated by septic cardiomyopathy requiring veno-arterial extracorporeal membrane oxygenation (VA-ECMO) for circulatory support. The clinical presentation of purpura fulminans closely resembles that of necrotizing soft tissue infections (NSTI), which makes differentiation challenging. Accurate differentiation is crucial for limb preservation strategies. CASE PRESENTATION: A 52-year-old man with a history of hypert…

Reconstruction of a rare case of bilateral juvenile gigantomastia at a tertiary hospital: a case report.

INTRODUCTION: Juvenile gigantomastia is a rare benign breast pathology that predominantly affects adolescents and women younger than 18 years of age. It is characterized by excessive, diffuse enlargement of the breast tissue (more than 1.5-2.5 kg per breast or constituting over 3% of total body weight), involving both glandular and stromal components. The histological picture consists of a juvenile fibroadenoma (JFA), which represents a distinct subtype of fibroepithelial breast tumor. The condition can cause severe physical discomfort, pain, posture abnormalities, recurrent skin infections, d
