Osteomyelitis of the clavicle is a rare condition that is difficult to diagnose. We have reviewed five cases of acute staphylococcal osteomyelitis of the clavicle that have been collected over a 3-year period in our center. All patients had pain, fever, and swelling over the clavicle. All had a raised erythrocyte sedimentation rate with positive Staphylococcus aureus blood cultures at presentation. Three children with acute schistosomiasis mansoni developed pyogenic liver abscesses. The abscesses were diagnosed by ultrasonography and confirmed during laparotomy. Sternoclavicular septic arthritis is an uncommon clinical entity that is often misdiagnosed on initial presentation. It has generally been described in IV heroin users and immunocompromised hosts. We report the case of a 43-year-old woman with endstage liver disease who presented with a fever, a painful sternoclavicular joint, and gastrointestinal bleeding. The clinical presentation, diagnosis, and treatment of sternoclavicular septic arthritis are reviewed.

A 38-year-old female patients was found accidentally to have a positive culture of MRSA from a routine CVP catheter tip culture 1 week after she had complete wound closure. She was recovering from a partial skin thickness burn covering 42 per cent TBSA on the trunk and extremities. We examined biological properties of strains of methicillin-resistant Staphylococcus aureus (MRSA) which were isolated in our ward in 1991 and 1992. A total of 47 MRSA strains were isolated in 1991 and 64 in 1992. The majority of these strains of MRSA were highly resistant to DMPPC, CEZ and IPM, and were intermediately resistant to MINO. All these strains were, however, sensitive to VCM. In order to detect subclinical mastitis by means of California Mastitis Test and recounting of somatic cells, 163 cows from the dairies of San Luis city, Argentina, were examined. We describe two patients suffering from bacterial endocarditis with tricuspid valve envolvement. Both had pulmonary embolism, revealed by scintigraphic lung perfusion examination. The diagnosis was made by transthoracic echocardiography in one patient and transoesophageal echocardiography in the other. Both received therapy with antibiotics and heparin given intravenously. In one patient the vegetation disappeared. We discuss the epidemiology, diagnosis and treatment of the condition, with special focus on the possible role of heparin.

An experimental animal model for bacterial joint inflammation has been tested. Using 16 rabbits divided into 4 groups, we injected knee joints of two groups with Staphylococcus aureus and the other 2 with NaCl. Although periosteal reaction in acute haematogenous osteomyelitis is known to occur and is well documented, massive periosteal reaction is rare. Two children with copious periosteal reaction around the shafts of the long bones are presented. This could be a manifestation of staphylococcal osteomyelitis as blood culture grew Staphylococcus aureus in one of the cases.

Toxic shock syndrome (TSS) is associated with a spectrum of Staphylococcus aureus infections and is not just a disease of menstruating females. We report three cases of TSS in adolescent males. In one case the origin of infection was a cauterized wart that did not appear clinically infected. Two cases were associated with bacteremia and had demonstrable acute phase antibody to toxic shock syndrome toxin-1 (TSST-1). One of these patients died. The S. aureus strain from this patient did not produce TSST-1 but did produce enterotoxin D. The historical and clinical features of TSS are reviewed.
