A 67-year-old white man with a history of right orbital trauma and restrictive strabismus underwent right superior oblique tenectomy for superior oblique restriction. Bleeding was noted from the superior rectus intraoperatively, despite the fact that the muscle was neither exposed nor hooked for the procedure. Subsequent exploration of the muscle revealed a full-width tear across the superior rectus muscle approximately 5 mm posterior to its insertion. The remaining residual distal muscle was removed from the insertion and the proximal end of the torn superior rectus muscle was identified and

BACKGROUND: The latissimus dorsi (LD) is often used for tendon transfers to treat massive irreparable posterosuperior rotator cuff tears. The operation requires the LD tendon to be mobilized to reduce tension on the tendon. In that respect, any connection between the LD tendon and contiguous muscles may hamper tendon mobility and affect the surgical outcome. The goal of this study was to document the occurrence of connections between the LD and adjacent muscles and nerves.
METHODS: We studied the scapular region on 48 embalmed cadavers. The skin and superficial fascia were removed according t

The spring ligament complex is an important static restraint of the medial longitudinal arch of the foot and its failure has been associated with progressive flatfoot deformity. Reconstruction of the spring ligament complex is most appropriate in stage II posterior tibial tendon dysfunction, before severe peritalar subluxation and rigid deformity develops. Although an understanding of the spring ligament complex and its contribution to medial arch stability has grown, there is no unanimously accepted surgical technique that has consistently demonstrated satisfactory outcomes. This article revi

Chronic instability is a common complication of lateral ankle sprains. If nonoperative treatment fails, a surgical repair or reconstruction may be indicated. Today, endoscopic techniques to treat ankle instability are becoming more popular. This article describes an endoscopic technique, using a step-by-step approach, to reconstruct the ATFL and CFL with a gracilis graft. The endoscopic technique is reproducible and safe with regard to the surrounding anatomic structures. Short and midterm results confirm the benefits of this technique.

A case of posttraumatic checkrein deformity of the hallux is presented. This deformity is most often caused by scarring of the muscle belly or tethering of the tendon. A 22-year old woman developed a hallux checkrein deformity after a bimaleolar fracture. Intraoperatively, a linear scar tethering the muscle belly to the posterior tibia was observed. Resection of the scar allowed for full flexor hallucis longus mobility. Full hallux range of motion as well as foot function was restored. The cause of the checkrein deformity in our patient was a scar tethering the flexor hallucis belly to the pos

BACKGROUND: Bilateral quadriceps tendon tears are infrequent conditions. Surgical repairs are the most appropriate treatments and they involve several months of recovery.
METHODS: We report the case of a patient with bilateral quadriceps tendon tear resulting from low energy trauma. He was treated with surgical transpatellar suturing reinforced with plasma rich in growth factors (PRGF-Endoret).
RESULTS: Short-term results showed functional and radiological recovery at the two-months follow-up. The use of PRGF together with usual surgery may contribute to early rehabilitation.

This article presents a 46-year-old man with bilateral atraumatic quadriceps rupture that occurred while he was descending stairs. The patient underwent surgery the day after the accident. In the left knee, quadriceps reinsertion was performed using a conventional technique. In the right knee, platelet-rich plasma (PRP), both in its liquid and semisolid patterns, was added intraoperatively. Ultrasonography and magnetic resonance imaging evaluations were performed 1, 6, and 24 months postoperatively. At 6 and 24 months postoperatively, clinical and functional evaluations also were performed. Cl

Osteochondroma of the bone can irritate the overlying soft tissue, resulting in pain and discomfort. Rarely, a tendon tear can occur by an impinging bony mass. We report a case of osteochondroma of the distal tibia in a 23-year-old female patient that was complicated by a partial tear of the tibialis posterior tendon. She was treated with excision of the bony mass combined with tendon debridement and repair. The follow-up examination at 5 years postoperatively showed successful results.

The distal tibiofibular syndesmosis provides stability to the ankle mortise, and it is composed of the anterior inferior tibiofibular, posterior inferior tibiofibular, interosseous, and inferior transverse tibiofibular ligaments and the interosseous membrane. Subacute or chronic syndesmosis injuries can present after missed diagnosis in the acute period or after failed or inadequate nonoperative management. It can result in chronic ankle pain and progressive degeneration of the ankle. Reconstructive options for chronic syndesmosis disruption include arthroscopic debridement and screw fixation,

Tendon transfer surgery has evolved over the past decade. Unique muscle properties have been elucidated in terms of potential force generation, excursion, and metabolic properties. The choice of an appropriate donor muscle is becoming more of a science than an art. The measurement of sarcomere length has progressed from the laboratory into surgery. Methods of tendon coaptation have advanced to allow early mobilization. This article reviews recent advances in our understanding of tendon transfer surgery of the hand and upper extremity.

OBJECTIVE: To describe the use of a single-ring transarticular fixator construct for use in immobilisation of the talocrural joint in order to protect the healing of surgical repair of ruptures of the common calcaneal tendon.
METHODS: Ten repairs were performed in nine dogs. The age, breed, sex, details about the duration of pre-existing lameness, and the location and severity of tendon rupture at presentation were recorded for each dog. Recheck examinations were performed at one, two, four, six, eight, and 12 weeks postoperatively. At each visit up to and including six weeks, radiographs of

Understanding the leg and joint stiffness during human movement would provide important information that could be utilized for evaluating sports performance and for injury prevention. In the present study, we examined the determinants of the difference in the leg stiffness between the endurance-trained and power-trained athletes. Seven distance runners and seven power-trained athletes performed in-place hopping, matching metronome beats at 3.0 and 1.5Hz. Leg and joint stiffness were calculated from kinetic and kinematics data. Electromyographic activity (EMG) was recorded from six leg muscles.

BACKGROUND: Skin ulcerations are rarely reported in dermatomyositis of the adult. We report on a case of perforating ulcers resistant to classical treatments in a woman with dermatomyositis.
CASE REPORT: A 34 year-old woman, treated for typical dermatomyositis, developed a few weeks after starting her treatment, multiple perforating skin ulcers on her shoulders, elbows and wrists though general signs had completely disappeared. In spite of the increase in oral steroids, the patient presented again with worsened lesions and a new perforating ulcer on the hand which lead to the breaking of a te

The unique function of the boxer's hand requires persistent, forceful punching in a constantly clenched fist posture, therefore, the metacarpophalangeal joints are continually exposed to blunt trauma and highly vulnerable to injury. This injury is traditionally termed boxer's knuckle. Although a myriad of metacarpophalangeal joint derangement is apt to result from isolated or repetitive blows inflicted and absorbed by the hand, the most serious and disabling type of boxer's knuckle is extensor hood disruption. Based on experience with 27 surgical cases, this article describes characteristic ex

Repair of a pectoralis major muscle rupture at the tendinous insertion into the humerus was successfully performed 13 years after the initial injury. Repair was possible only because the ruptured sternal portion of the muscle was scarred to the intact clavicular portion and therefore had not retracted. The attachment of the avulsed sternal head to the intact clavicular head enabled successful restoration of strength and function, as well as normal contour and appearance of the pectoralis major muscle complex.

Radial nerve palsy results in a lack of extrinsic extensors of the wrist, fingers and thumb. The degree of dysfunction depends on the level of trauma to the radial nerve. If the wrist is not stable there is a loss of prehensile grip. Patients have great difficulty picking up large or heavy objects. In high radial nerve palsy the loss of triceps function can cause additional problems. Based on our knowledge of anatomy and biomechanics, radial nerve function can be restored sufficiently. The indications, preoperative planning, technique of transposition and postoperative management are explained
