Optic pathways tuberculoma mimicking glioma: case report.

BACKGROUND: Optochiasmatic tuberculomas are very rare lesions. They can occur with concomitant tuberculous meningitis, and pulmonary tuberculosis or as the only manifestation of the disease. The authors present a case of optic pathways tuberculoma with radiologic appearance simulating an optic pathways glioma. CASE DESCRIPTION: We report a case of a 20-year-old man with mental retardation due to anoxic encephalopathy who developed a sudden bilateral amaurosis. He also presented with diabetes insipidus, panhypopituitarism, right proptosis, and chemosis. Computed tomography (CT) and magnetic re

Further investigations of a radiation detector based on ionization-induced modulation of optical polarization.

Optical property modulation induced by ionizing radiation is a promising approach for ultra-fast, lower time jitter detection of photon arrival time. If successful, this method can be utilized in time-of-flight positron emission tomography to achieve a coincidence time resolution approaching 10 ps. In this work, the optical property modulation based method is further developed with focus on a detection setup based on two crossed polarizers. Previous work demonstrated that such an optical setup could be utilized in radiation detection, though its detection sensitivity needed improvement. This w

Mutant NADH dehydrogenase subunit 4 gene delivery to mitochondria by targeting sequence-modified adeno-associated virus induces visual loss and optic atrophy in mice.

PURPOSE: Although mutated G11778A NADH ubiquinone oxidoreductase subunit 4 (ND4) mitochondrial DNA (mtDNA) is firmly linked to the blindness of Leber hereditary optic neuropathy (LHON), a bona fide animal model system with mutated mtDNA complex I subunits that would enable probing the pathogenesis of optic neuropathy and testing potential avenues for therapy has yet to be developed. METHODS: The mutant human ND4 gene with a guanine to adenine transition at position 11778 with an attached FLAG epitope under control of the mitochondrial heavy strand promoter (HSP) was inserted into a modified s

Asymmetric choroidal vascular pattern in tilted disc syndrome.

PURPOSE: To observe the choroidal vasculature in patients with tilted disc syndrome (TDS) using en face optical coherence tomography (OCT) and to evaluate the symmetry of the choroidal vascular pattern in the macular area. METHODS: En face OCT was performed using a Plex Elite 9000 (Zeiss) with a 12 × 12 mm image of the macula, which was flattened with retinal pigment epithelium and segmented into choroidal layers. The middle/large choroidal vessels were evaluated for vertical symmetry qualitatively by the retina specialist coauthors and quantitatively by binarization analysis of choroidal ves

Superior Segmental Optic Nerve Hypoplasia: A Case Report and Literature Review.

Superior segmental optic nerve hypoplasia (SSONH) is a rare congenital anomaly in which there is underdevelopment of the superior part of the optic nerve. This leads to a reduction in the total nerve fibres constituting the optic nerve and can result in inferior visual field loss. SSONH is classically associated with maternal type 1 diabetes mellitus. We report an atypical case of SSONH associated with maternal gestational diabetes but with no prior history of maternal type 1 diabetes. We comment on the associated retinal nerve fibre layer (RNFL) changes, and also the corresponding ganglion ce

Visual improvement after long-standing central serous macular detachment associated with an optic disc pit.

BACKGROUND: Approximately 50% of cases of congenital optic disc pits are associated with serous macular detachment. Long-standing serous detachments (over 1 year) with poor initial presenting acuity are usually associated with cystic degeneration of the macula and loss of pigment from the underlying retinal pigment epithelium, with resultant poor visual outcome. METHODS: A 43-year-old male presented with an optic disc pit and associated serous elevation of the macular region with cystic changes at the fovea. RESULTS: Over the following 2 years, the patient was aware of an improvement in visi

Blindness after blow-out fracture repair.

Blindness following uncomplicated repair of orbital blow-out fracture has been reported previously. Mechanisms that have been implicated in this devastating complication include direct intraoperative injury to the optic nerve, increased orbital pressure, retrobulbar hemorrhage, and bone spicule impingement on the optic nerve. Two additional cases of blindness following uneventful surgical repair of a blow-out fracture are described herein. The first patient had sustained severe blunt trauma to the forehead after discharge from the hospital. The computed tomography (CT) scan of his orbit sugges

Non-arteritic anterior ischemic and glaucomatous optic neuropathy: Implications for neuroretinal rim remodeling with disease severity.

PURPOSE: Post-acute non-arteritic ischemic optic neuropathy (NAION) and glaucomatous optic neuropathy (GON) can be difficult to differentiate clinically. Our objective was to identify optical coherence tomography (OCT) parameters to help differentiate these optic neuropathies. METHODS: We compared 12 eyes of 8 patients with NAION and 12 eyes of 12 patients with GON, matched for age and visual field mean deviation (MD). All patients underwent clinical assessment, automated perimetry (Humphrey Field Analyzer II; Carl Zeiss Meditec, Dublin, CA, USA), and OCT imaging (Spectralis OCT2; Heidelberg

Measurement of retinal nerve fiber layer thickness with a deep learning algorithm in ischemic optic neuropathy and optic neuritis.

This work aims at determining the ability of a deep learning (DL) algorithm to measure retinal nerve fiber layer (RNFL) thickness from optical coherence tomography (OCT) scans in anterior ischemic optic neuropathy (NAION) and demyelinating optic neuritis (ON). The training/validation dataset included 750 RNFL OCT B-scans. Performance of our algorithm was evaluated on 194 OCT B-scans from 70 healthy eyes, 82 scans from 28 NAION eyes, and 84 scans of 29 ON eyes. Results were compared to manual segmentation as a ground-truth and to RNFL calculations from the built-in instrument software. The Dice

Prevalence and diagnostic ability of β-zone parapapillary atrophy in open-angle glaucoma: a systematic review and meta-analysis.

BACKGROUND: β-Zone parapapillary atrophy (β-PPA) is a common sign in patients with open-angle glaucoma (OAG). Some studies have suggested that β-PPA can aid in the diagnosis of OAG. We performed a systematic review and meta-analysis of the prevalence and diagnostic ability of β-PPA in OAG. METHODS: We performed a literature search in PubMed, Web of Science, Embase and Google Scholar from inception to 1st November, 2021. Both hospital-based and population-based studies that reported details of β-PPA in OAG were included. RESULTS: We screened 1404 articles from these databases and ultimately i

Vitreoretinal Findings in Nonarteritic Ischemic Optic Neuropathy.

BACKGROUND: Although nonarteritic anterior ischemic optic neuropathy (NAION) is considered a disorder that primarily affects the optic nerve head, optical coherence tomography (OCT) shows peripapillary and foveal subretinal fluid associated with optic disc swelling from NAION. We sought to further evaluate retinal and vitreous changes in patients with NAION. METHODS: Patients diagnosed with NAION at the New England Eye Center between 2013 and 2017 were evaluated using OCT. The presence and distribution of subretinal fluid was analyzed. Evidence of other vitreoretinal changes, including vitreo

Bilateral drusenoid deposits with subretinal fluid and cystoid macular edema in a serum anti-phospholipase A2 receptor antibody positive patient: A case report.

INTRODUCTION: Phospholipase A2 receptor (PLA2R) is a major target antigen in idiopathic membranous nephropathy (IMN). IMN was often reported to have retinal vascular abnormalities secondary to hypercoagculability or hypertension. We describe a case of a serum anti-PLA2R antibody positive patient with bilateral drusenoid deposits and subretinal fluid (SRF) and cystoid macular edema (CME) to stress the necessity of fundus examination in anti-PLA2R antibody positive or IMN patients. OBJECTIVE: To report a case of unique fundus manifestations in a serum anti-phospholipase A2 receptor (anti-PLA2R)

Structural changes in retina (Retinal nerve fiber layer) following mild traumatic brain injury and its association with development of visual field defects.

BACKGROUND: Mild traumatic brain injury (mTBI)is the most common form of traumatic brain injury accounting for 70-80% of all brain injuries annually. There is increasing evidence that long lasting morphological and functional consequence can be present in visual system following mTBI. Among all the visual manifestation, awareness of Visual field defects is important because it may compromise the social, personal or professional life of any individual. Retinal structural changes such as thinning of Retinal nerve fiber layer (RFNL)captured using optical coherence tomography have emerged as a pos
