Disseminated histoplasmosis is a condition with nonspecific clinical signs. In case of fever in a patient receiving anti-TNF alpha therapy, a thorough history that includes travel to areas of high endemicity can raise suspicion of infection. When suspected, diagnosis is made by detecting antigens in urine and blood (if this analysis is available), conducting serology, and obtaining a specimen for culture, direct microscopy, and histopathological analysis. BACKGROUND: Bangladesh's health system is largely financed through out-of-pocket (OOP) payments. We aimed to quantify the direct OOP expenditure incurred by hospitalised patients with dengue during the 2023 epidemic.
METHODS: A cross-sectional study was conducted among 428 hospitalised adult patients with dengue at a major public hospital during the 2023 epidemic, collecting detailed OOP expenditure data.

A 36-year-old male with no significant medical history presented to the ED with progressive left-sided neck and facial pain for the last 7 days. The patient also reported subjective fevers, chills, difficulty opening his mouth, and anorexia for the last 1 week. He denied cough, chest pain or tightness, shortness of breath, skin rashes, dysphagia, or odynophagia. He reported use of two to three 59.15 mL beers daily for the last 20 years. There was no recent travel or sick contact exposure. He did not report any TB exposure, IV drug use, or recent sexual encounters.

A 69-year-old man consulted for a 3-day history of fever, wet cough, and yellow-green phlegm. He denied having any dyspnea, chest pain, hemoptysis, swallowing disorders, choke, chills, asthenia, anorexia, or weight loss. He reported a continuous dry cough and three episodes of pneumonia in the past 4 years. He was a nonsmoker, without any other personal or familial medical history. We report a case of imported pulmonary coccidioidomycosis caused by Coccidioides posadasii in a patient who was misdiagnosed as tuberculosis and mistreated with antituberculosis medications for 18 months. The symptoms were not relieved until antifungal treatment was started. An extensive review of the coccidioidomycosis cases occurring in China reveals 38 cases, 16 of which had no associated history of travel to any traditional endemic areas.
