In vitro and in vivo antibacterial activities of a novel glycylcycline, the 9-t-butylglycylamido derivative of minocycline (GAR-936).

The 9-t-butylglycylamido derivative of minocycline (TBG-MINO) is a recently synthesized member of a novel group of antibiotics, the glycylcyclines. This new derivative, like the first glycylcyclines, the N,N-dimethylglycylamido derivative of minocycline and 6-demethyl-6-deoxytetracycline, possesses activity against bacterial isolates containing the two major determinants responsible for tetracycline resistance: ribosomal protection and active efflux. The in vitro activities of TBG-MINO and the comparative agents were evaluated against strains with characterized tetracycline resistance as well

Linezolid as a potential alternative to glycopeptide in treating Enterococcus faecium suspected cyst infection in a hemodialysis patient with autosomal-dominant polycystic kidney disease: A case report.

Autosomal-dominant polycystic kidney disease is a clinically significant disorder associated with progressive renal impairment. In these patients, cyst infection is an important but variably occurring complication, and optimal antibiotic selection remains challenging owing to the limited available evidence and restricted antibiotic penetration into cysts. An 81-year-old Japanese man with autosomal-dominant polycystic kidney disease undergoing maintenance hemodialysis presented with fever and a suspected cyst infection. Blood cultures showed the fluoroquinolone-resistant Enterococcus faecium. V

Refractory Clostridioides difficile Colitis in a Patient With Hidradenitis Suppurativa: Consequences of Long-Term Clindamycin and Multi-agent Anti-TNF Therapy.

Clostridioides difficile infection (CDI) is a significant cause of morbidity and mortality worldwide, and its treatment is often challenging in patients with complex medication regimens. We report a case of refractory CDI in a 70-year-old woman with Hurley stage II hidradenitis suppurativa (HS) involving the right axilla and perianal region. Her HS was managed with chronic antibiotic use (clindamycin) and anti-tumor necrosis factor (TNF) biologics, initially infliximab (5 mg/kg every eight weeks) followed by adalimumab (40 mg every two weeks) due to inadequate response to infliximab. Despite m

Design, Synthesis, and Antibacterial Activity of Long-Chain Alkyl-Substituted DemethyloxyAaptamine Derivatives.

Demethyloxyaaptamine, isolated from the marine sponge Aaptos aaptos, features a 1H-benzo[de][1,6]naphthyridine core and exhibits potent antibacterial activity. To systematically investigate its underexplored antibacterial properties and facilitate structural optimization, we constructed a focused library of 28 C-3 alkylamino-substituted derivatives of demethyloxyaaptamine via regioselective functionalization. In vitro evaluation against Staphylococcus aureus revealed that several derivatives possess minimum inhibitory concentrations (MICs) superior to vancomycin. Structure-activity relationshi

Antioxidant and antibacterial hydrogel formed by protocatechualdehyde-ferric iron complex and aminopolysaccharide for infected wound healing.

To better treat bacteria-infected wounds and promote healing, new wound dressings must be developed. In this study, we obtained PA@Fe by chelating iron trivalent ions (Fe3+) with protocatechualdehyde (PA), which has a catechol structure. Subsequently, we reacted it with ethylene glycol chitosan (GC) via a Schiff base reaction and loaded vancomycin to obtain an antibacterial Gel@Van hydrogel with a photothermal response. The as-prepared Gel@Van hydrogel exhibited good injectability, self-healing, hemostasis, photothermal stability, biocompatibility, and antioxidant and antibacterial properties.

Lactococcus garvieae in Rural Alabama: A Case Report.

Lactococcus (L.) garvieae is a gram-positive coccus that has been found in various aquatic and terrestrial animals, as well as in dairy products, and is considered a potential zoonotic bacterium. The pathogen has been recognized as an emerging opportunistic human pathogen, often associated with the ingestion of raw seafood. The most common presentation of L. garvieae infection in humans is infective endocarditis, but it has also been found to have associations with other clinical manifestations. The following is a case report of a 6-year-old male with infected bilateral leg abrasions that occu

Characterization of an Enterococcus faecalis bacteriophage SFQ1 as a potential therapeutic agent.

Enterococcus faecalis is a well-established resident of the human gastrointestinal tract and is also a major cause of human infections. Unfortunately, therapeutic options for E. faecalis infections remain limited, particularly with the emergence of vancomycin-resistant strains in hospital settings. Consequently, there has been a renewed interest in phage therapy as an alternative to antibiotics. In this study, we have isolated a bacteriophage, vB_EfaS-SFQ1, from hospital sewage, which effectively infects E. faecalis strain EFS01. Phage SFQ1 is a siphovirus and exhibits a relatively broad host

Rare case of bacteremia due to Lysinibacillus sphaericus in a person living with HIV.

Lysinibacillus sphaericus, as an insect pathogen, is a ubiquitous Gram-positive bacterium present in the environment. It is often considered to be contaminating bacteria. L. sphaericus has been reported to cause infectious diseases in humans relatively rarely. We report a rare case of bacteremia due to L. sphaericus in a person living with HIV, which is also the first reported case of bacteremia caused by L. sphaericus in China. L. sphaericus easily causes infection in immunocompromised individuals. We found that L. sphaericus and Lysinibacillus fusiformis could not be distinguished by their 1

Stalobacin: Discovery of Novel Lipopeptide Antibiotics with Potent Antibacterial Activity against Multidrug-Resistant Bacteria.

A novel lipopeptide antibiotic, stalobacin I (1), was discovered from a culture broth of an unidentified Gram-negative bacterium. Stalobacin I (1) had a unique chemical architecture composed of an upper and a lower half peptide sequence, which were linked via a hemiaminal methylene moiety. The sequence of 1 contained an unusual amino acid, carnosadine, 3,4-dihydroxyariginine, 3-hydroxyisoleucine, and 3-hydroxyaspartic acid, and a novel cyclopropyl fatty acid. The antibacterial activity of 1 against a broad range of drug-resistant Gram-positive bacteria was much stronger than those of "last res

More than Garden Variety: Massive Vegetations from Infective Endocarditis.

Infective endocarditis classically involves non-sterile vegetations on valvular surfaces in the heart. Feared complications include embolization and acute heart failure. Surgical intervention achieves source control and alleviates valvular regurgitation in complicated cases. Vegetations >1 cm are often intervened upon, making massive vegetations uncommon in modern practice. We report the case of a 39-year-old female with history of intravenous drug abuse who presented with a serpiginous vegetation on the native tricuspid valve and methicillin-resistant Staphylococcus aureus bacteremia. The veg

Septic arthritis in children: diagnosis and treatment.

Acute septic arthritis in children is usually hematogenous. It is more common in boys, and it most often affects the large joints of the lower limb. Diagnosis is based on cultures obtained from the infected joint and is supported by C-reactive protein blood test or ultrasound imaging. Staphylococcus aureus is the most common causative agent and is the primary target for empiric treatment. First-generation cephalosporins and clindamycin are suitable antibiotics. Vancomycin is utilized in areas with high rates of clindamycin- and methicillin-resistant S. aureus. After a short intravenous adminis
